Intrasellar chordomas mimicking pituitary adenoma.

OBJECT: Whereas chordomas involving the sellar region are uncommon, largely or entirely intrasellar examples are rare. The goal in this study was to present examples of these rare tumors as a guide to their proper diagnosis and treatment. METHODS: The authors report three cases in which the chordomas filled the pituitary fossa and presented as nonfunctioning pituitary adenomas. All lesions exhibited the typical histological patterns and immunophenotype of chordoma. One tumor, studied ultrastructurally and subjected to DNA analysis, was shown to have a diploid histogram. The authors present a clinicopathological study of these three cases and review the literature on intrasellar chordomas. CONCLUSIONS: Although these tumors are easily misdiagnosed and therefore may not receive optimal treatment, aggressive surgical resection can yield a favorable prognosis in lesions with a limited extent.

Prognostic significance of proliferating cell nuclear antigen (PCNA) expression in gliomas.

The relationship between proliferating cell nuclear antigen (PCNA) expression and various clinicopathological indices (age, sex, tumour location, histological type and grade and treatment) and post-operative survival were studied in patients with central nervous system gliomas using univariate and multivariate analysis. The expression of PCNA (PC10 score) was examined immunohistochemically using the monoclonal antibody PC10 on paraffin sections from 45 cases. Univariate analysis showed that a high PC10 score as well as older age, high histological grade and the histological type (astrocytoma) were associated with reduced survival. However, multivariate analysis revealed that only PC10 score and histological type had independent prognostic significance. The most important feature influencing PC10 score was the tumour grade. Regarding the patients who relapsed, the survival from the time of original diagnosis was related to the relapse-free period, while the PC10 score of the primary tumour emerged as the only independent predictor of survival following the first recurrence. These results indicate that PCNA expression is an independent prognostic indicator in CNS gliomas.

Malignant astrocytoma containing bone.

A case of a malignant cerebral astrocytoma containing bone in a 56-year old woman is reported. In spite of total removal and radiotherapy the tumour recurred and the patient died 18 months postoperatively. The histological features and immunohistochemical findings are presented. The literature is reviewed and probable mechanisms by which these mesodermal derivatives develop in neuroepithelial neoplasms are discussed.

Cerebrovascular accidents following hormonochemotherapy in cancer patients.

It has become increasingly apparent that the administration of cytotoxic drugs or hormones may be associated with life-threatening vascular episodes. We report on seven women and one man who developed cerebrovascular accident (CVA) during or immediately after the administration of several hormonochemotherapeutic regimens. No patient died from the stroke. Five patients made a complete recovery; one showed remarkable improvement, and two, slight improvement. Several explanations for CVA were proposed, but the pathophysiological basis of the relationship between the arterial occlusive complication and hormonochemotherapy is unclear.

Cerebrospinal fluid carcinoembryonic antigen and alphafetoprotein in patients with central nervous system neoplasia.

A solid phase immunoassay was used to evaluate the levels of serum and cerebrospinal fluid of alphafetoprotein and carcinoembryonic antigen in 33 individuals treated for backache and headache with no evidence of organic neurological disease, 19 patients with primary CNS tumor (benign or malignant) and 22 with CNS metastasis from a solid tumor. AFP serum and CSF levels were found in trace amounts or slightly elevated not exceeding normal limits in all groups. Patients with CNS metastasis were found to have statistically significant higher CEA levels (both in serum and CSF) than the control group, and the patients with primary brain tumors. Patients with leptomeningeal dissemination had statistically significantly higher CEA CSF levels than did patients with primary tumors, and patients with parenchymal metastasis.

Gliosarcoma cerebri with a myxoid matrix of the sarcomatous component.

A case of a cerebral gliosarcoma in a 63-year old female located in the left parietooccipital area of the brain is herewith described. Histology showed the tumour to consist of two components: a gliomatous resembling glioblastoma, and a sarcomatous with a prominent myxoid intercellular substance, a feature reported only thrice before in the medical literature. We report the histochemical and immunohistochemical characteristics of our case, and compare them with those of previous case reports.
