In this study, 9 patients with primary hypercholesterolemia were treated with pravastatin. They received a daily dose of 20 mg for 12 weeks. Results showed a significant reduction in total and LDL cholesterol, whereas no significant difference was observed in HDL and triglyceride levels after treatment. No adverse effects were observed.
3-Hydroxy-3-methylglutaryl coenzyme A reductase inhibitors, a new class of hypolipidaemic agents, have been recently shown to be effective in lowering both total and low-density lipoprotein (LDL)-cholesterol. In the present study 20 patients were treated with simvastatin for six months. Various dosing regimens of 10, 20, and 40 mg/day were employed. Administration of simvastatin 10 to 40 mg/day resulted in a 24.7 to 35.8% reduction in LDL-cholesterol levels and in a 19.5 to 26.9% decrease of total cholesterol levels. Moreover, simvastatin increased high-density lipoprotein-cholesterol and induced a decrease in plasma triglycerides. The side-effect profile was similar to other observations and included reversible transaminase elevations, myalgia and gastrointestinal complaints. From the present study simvastatin seems to be well tolerated in the short- to medium-term and to be effective; however, its long-term tolerability needs to be confirmed.

The state of the fundus oculi was investigated in 40 insulin-dependent and 20 non-insulin dependent diabetics. In both groups the state of the fundus oculi was correlated with total cholesterol, LDL, HDL, glycosylated haemoglobin, platelet adhesiveness, triglycerides and duration of the disease. In the case of insulin-dependent diabetics a correlation was also sought with daily doses of insulin. The fundus oculi was normal in 21/40 insulin-dependent and 14/20 non-insulin-dependent subjects. Multiple regression showed a positive correlation between insulin-HbA doses and duration of the disease and the fundus oculi. Simple liner regression showed a negative correlation between insulin doses and fundus, a tendentially positive correlation between platelet adhesion and fundus. A positive correlation between LDL-platelet adhesion and fundus was also shown in the non-insulin-dependent subjects. These results suggest that factors held to influence macroangiopathy also have an effect on macroangiopathy, that the check up of diabetics is important for the prevention of retinopathy, that insulin doses should be strictly limited to the necessary amount for the control of diabetes since excess is as harmful as insufficiency.
Biosynthetic Human Insulin (BHI) was administered to 20 diabetics for six months. A clinical examination, glycaemia before and after eating, total glycosuria-ketonuria, and glycosylated haemoglobin tests were performed at fixed intervals, and the results were statistically analysed. It is concluded that human insulin produces no significant side effects. More important that the type of insulin treatment is effective outpatients surveillance of the diabetic, to ensure an adequate metabolic compensation.

Screening data involving uterine neck tumours are reported from the Varese/3 Medical Consortium in the two-year period 1977-1978. Out of 3,929 smears, a neoplasia percentage of 0.23% was observed. This figure differs from that reported elsewhere and is not easy to interpret.

The results of a breast tumour prevention campaign conducted in 1977-78 by the Varese/3 Southern Outer Health Consortium are described. All women aged 30-65 yr were asked to come for a clinical examination of the breast by suitably trained midwives. Periodic self-examination was explained on this occasion and a card was prepared with historical data for evaluation of the risk factors. 4708 of the 8226 women approached came for examination (57%). Of those subjected to further diagnostic tests, 170 are still under out-patient control, while 34 have been hospitalised with a neoplastic incidence of the order of 1.28%, coupled with a pathological picture with various probabilities of degeneration. On the whole, it is felt that the most useful feature of the venture was its contribution towards the health education of the female population, and its bringing out of risk factors. A favourable judgement is passed on the campaign and its continuation is suggested.
A model of computerized clinical history is presented for physicians and anyone else interested in automatic information processing. The questionnaire should be compiled by the patient, controlled and examined by paramedical and medical staff, processed and managed by computer and organized into an output report. In addition to historiddition to historical data, this report includes the results of fun ctional and laboratory findings. In such a context, the purpose of anamnesis is to provide a complete clinical history in the interests of the better "classification" of the patient. Its logic and structure, however, also lends itself well to clinical and statistical uses and to a standardization of the clinical card.
The medical, social and economic problems and implications involved in periodic health check-ups are considered. The starting point is the early experience of the Italian Diagnosis Centre and the examination of what has been done by Centres working in the field for some time. The intention is to stress the importance of defining in detail those pathological factors worthy of investigation and of showing that the purpose of the check-up is to pinpoint the correct diagnosis as quickly as possible. "Early diagnosis" is considered to be an area of primary importance in the struggle against disease. Finally, the social importance of the check-up in reducing the incidence of hospital admittances and average hospitalization is stressed.

The connection between age, sex and some laboratory parameters (blood uric acid, glucose, cholesterol and triglycerides) has been studied on a very wide whole (22025 exams). An important datum which has appeared is that the increase of age is connected with an increase of the values of the parameters studied, and that the sex conditions differences which proved statistically significant.

A statistical analysis was made of the historical questionnaires filled in by 1000 adults subjected to a check-up at the Italian Diagnostic Centre, the aim being to assess the medical and social background of persons taking advantage of services of this kind.
