Interaction of IGF-I and 1 alpha, 25(OH)2D3 on receptor expression and growth stimulation in rat growth plate chondrocytes.

Growth plate cartilage cell express receptors for, and are affected by both IGF-I and 1 alpha, 25(OH)2D3. The studies were undertaken to investigate interaction between these two hormone systems, that is, (i) to study effects of 1 alpha, 25(OH)2D3 on IGF-type 1 receptors (IGFIR), on IGF-I stimulated cell replication, colony formation, and on alkaline phosphatase activity (AP), and conversely, (ii) to study the effect of IGF-I on vitamin D receptor (VDR) expression on 1 alpha, 25(OH)2D3 stimulated growth parameters and on AP activity. Freshly isolated rat tibial chondrocytes were grown in monolayer cultures, (serum-free) or in agarose stabilized suspension cultures (0.1% FCS). Vitamin D receptor and IGFIR were visualized by immunostaining with the monoclonal antibody (mAb) 9A7 gamma and mAb alpha IR3, respectively, and quantitated by RT-PCR for mRNA and by Scatchard analysis using [3H]-1,25(OH)2D3 and [125I]-alpha IR3. Cell proliferation was measured by [3H]-thymidine incorporation, growth curves in monolayer cultures, and by colony formation in agarose-stabilized suspension cultures. IGF-I dose-dependently increased [3H]-thymidine incorporation. 1 alpha, 25(OH)2D3, but not 1 beta, 25(OH)2D3 was stimulatory at low ((10-12 M) and slightly inhibitory at high (10-8 M) concentrations. The effect of IGF-I was additive to that of 1 alpha, 25 (OH)2D3 [IGF-I 60 ng/ml, 181 +/- 12.7; 1 alpha, 25(OH)2D3 10(-12) M, 181 +/- 9.8%, IGF-I + 1 alpha, 25(OH)2D3, 247 +/- 16.7%, P < 0.05 by ANOVA] and specifically obliterated by polyclonal IGF-I antibody (AB-1). Interaction could also be confirmed in suspension cultures. IGFIR mRNA and [125I]-alphaIR3 binding was increased by low (10(-12) m) but not by high (10(-8) M) concentrations of 1 alpha, 25(OH)2D3. Homologous up-regulation by IGF-I (60 ng/ml) was specifically inhibited by AB-1 and markedly amplified by coincubation with 1 alpha, 25(OH)2D3 (10(-12)m). Immunostaining with alpha IR3 showed specific IGFIR expression in rat growth cartilage, but not liver tissue. Stimulation of chondrocytes with 1 alpha, 25(OH)2D3 or IGF-I suggested some increase of receptor expression in single cells, but the predominant effect was increased recruitment of receptor positive cells, Vitamin D receptor expression was markedly stimulated (fourfold) by IGF-I (60 ng/ml), but not IGF-II and inhibited by actinomycin D. This study shows that IGF-I and 1 alpha, 25(OH)2D3 mutually up-regulate their respective receptors in growth plate chondrocytes. In parallel, they have additive effects on cell proliferation and colony formation suggesting independent effector pathways.

[Longevity in patients manifesting diabetes in 1962/1963 in the Neustrelitz district in relation to body weight].

Out of 147 of the 166 diabetics detected in the District of Neustrelitz in 1962/1963 (drop-out rate 11.45%) after 22 years lived still 14.3%. In comparison to an age and sex-adapted normal population the further life expectancy of the in the meantime decreased patients was on an average diminished by 6.2 years in the male sex and by 7.0 years in the female sex. In contrast to the expectance diabetics who were obese at manifestation concerning their mean age of death scarcely differed from patients with primarily normal weight (73.2 or 74.1 years) so that the predictive value of the initial body weight is insignificant with regard to the long-term prognosis. 65.1% of the diabetics died from diseases of the circulatory system, only 6.35% died of malignant neoformations. Only in 4.76% the diabetes itself was the actual cause of death.

[Perimortal management--humanistic assignment and responsibility. Attempt at status determination].

The scientific research and practical use of all questions concerning life demands interdisciplinary cooperation. This is the result of neglection of death and dying and new social and medical developments as there are institutionalisation of dying and scientific-technical progress. The foundation of the working group "perimortal medicine" in 1983 in the Association for clinical medicine gives the possibility to discuss theoretical and practical questions concerning the end of life, death and dying. It seems to be important, that also bereavement and sympathy belongs to the topics. The results will help to develop the quality of care for the dying and the survivors.

[Diabetes mellitus in advanced age. 6. On the life expectancy following diabetes manifestation (a 23-year mortality follow-up of diabetic patients in a regional district)].

Of all 166 diabetics of all age groups of a rural district detected 23 years ago, the further course of the disease could be followed in 147 patients up to their death and to the present time, respectively. After 8 years 64.6% were still living, after 16 years 29.9% and after 23 years 13.6% of these patients. The body weight at the manifestation of the disease was prognostically only low of relevance. The prognostic importance of the higher-degree adiposity seems to lie prevailingly in the earlier manifestation of diabetes, less in shortening the remaining life span. The trend of the weight development of the patients presently still alive moved towards reaching or keeping normal weight, independent of the initial body weight, while the contrary tendency in long-term diabetics is exceptional. Test admissions covered by a glycosuria screening and deceased meanwhile lived on average 1 year longer than spontaneous admissions, the same age of manifestation supposed. Only 13.4% of all deceased patients had reached the higher expectation of life of a normal population of the GDR of the same age on manifestation, which underlines the presently still unsatisfactory long-term prognosis of the diabetic patients.
