[Neoplasms of the extrahepatic bile ducts. A clinical study of 44 cases].

44 clinical cases, histologically diagnosed as suffering from extrahepatic bile duct neoplasia, admitted at the "Hospital Foral de Navarra" during the years 1976 to 1985 and at the "Hospital Clinico Universitario de Salamanca" during the period from 1981 to 1987, were analysed. Our results showed a higher incidence in the female sex. The most important antecedent was biliary stone lithiasis (45.45%). Jaundice and abdominal pain were the most frequent symptoms and liver enlargement accompanied by jaundice were the most frequent clinical signs on physical examination. The data obtained did not show specificity at the diagnosis. The ERCP showed a high rate of diagnosis (71.43%) followed by CT scanning (70%) and percutaneous transhepatic cholangiography (52.63%). The types of surgery most frequently performed were cholecystectomy and choledochoduodenostomy, and the diagnosis of well-differentiated carcinoma was found in 63.64% of the cases. Any therapy performed (surgery, radiotherapy and/or chemotherapy) improved the survival rate.

[Round pneumonias].

Two cases of pneumonia in which the initial radiologic sign was the solitary nodule are presented. The symptoms associated permitted the infectious origin. The clinical and radiological evolution after antibiotic therapy supported the diagnosis of rounded pneumonia. We highlight the need to discard benign lesions causing the solitary nodule, before trying to diagnose other diseases.

[N-acetyl-beta-D-glucosaminidase in jaundice syndrome].

The seric activity of the enzyme NAG (N-acetyl-beta-D-glucosaminidase) was studied in 3 groups of patients with jaundice of different etiology (extrahepatic, intrahepatic and acute hepatitis) and in a control group, in order to observe the discrimination capacity of this enzyme in each group. After the use of a fluorimetric method, the results were exposed, applying the statistical test, comparing different variables; the results are discussed and we conclude that NAG is an enzyme of easy detection and low cost to be included in the group of tests to study the biological pattern of jaundice.

[Primary duodenal adenocarcinoma; apropos of a case].

Primary duodenal adenocarcinoma is an infrequent malignant tumor that usually affects persons 60 to 70 years-old. Its clinical manifestations are nonspecific. The diagnosis is suspected on the basis of gastroduodenal contrast radiography and it is generally confirmed by endoscopic biopsy. It is slow to metastasize, so early diagnosis can lead to curative surgical treatment. We present a case seen late, of rapid and fatal course. Statistical data are provided and we comment on some aspects of the classification, clinical manifestations, diagnosis and treatment of these tumors.
