[Percutaneous puncture treatment of kidney cysts under echographic control].

Experience had with percutaneous puncture treatment under echographic control in 122 patients presenting kidney cysts is shared. The indications and contraindications for treatment, puncture technique applied and drugs used for cyst epithelium sclerosis are discussed, followed by a description of an original method of cyst cavity draining. In all patients with cyst drainage duely performed the outcome of treatment is successful. Good therapeutic results upon using sclerosing substances are attained in 88.7 per cent, while in cysts not subjected to medication-in 64.3 per cent, or else the total of positive outcomes amounts to 81.8 per cent. After puncture, it is advised to insert a drain into the cystic cavity, and invariably administer sclerosing substances. The readily performed technique of puncture, lack of serious complications, short hospitalization and prompt recovery of the ability for work are unquestionable advantages of the method proposed. In case of cyst recurrence it is possible to perform secondary puncture.

[The treatment of calculi in the iliac segment of the ureter by extracorporeal high-energy shockwave lithotripsy].

Experience is recorded with the treatment of calculi in the iliac segment of the ureter by extracorporal lithotripsy with high-energy stroke waves with lithotriptor of the firm "Dornier", model HM-3. For a period of 2 1/2 years 18 patients at mean age 38 years have been treated. Retrograde catheterization was always performed before lithotripsy. Successful reposition was achieved in 6 patients. In all others lithotripsy was performed in prone position. Special supporting cushions were manufactured and used; they helped for a more stable position of the patient on the stand, facilitate calculus positioning, the load to the knee joints is reduced, the fluoroscopic time is shortened, thus reducing the radiation load to patient and attending personnel. All patients received antibacterial and spasmolytic therapy. The mean length of stay in the clinic was 3.2 days. The results were good in 94.1 per cent of the cases. No early or late complications of urologic or other nature were observed.

[The treatment of coralliform kidney calculosis with extracorporeal lithotripsy, percutaneous lithotripsy and ureteroscopy].

Experience is recorded with the combined treatment of 53 patients with coralliform renal calculosis. Combined application of bypass lithotripsy (BLT), percutaneous lithotripsy (PLT) and ureterorenoscopy (URS) is an alternative to open surgical intervention. Depending on the size, form and position of the calculus, the degree of destruction of the residual concrements, combined therapy was applied, as follows: 17 patients--PLT and BLT, 7 patients--PLT, BLT and URS, 7 patients--URS and BLT, 13 patients--BLT and URS, 8 patients--BLT and URS. Very good results were obtained--complete cure in 51 patients (96.2 per cent), which encourage the authors to recommend the three methods for routine application in the treatment of coralliform renal calculosis.

[Echocardiographic research on the left ventricular function of patients with a complete atrioventricular block before and after pacemaker implantation].

Twenty seven patients with total atrioventricular (AV) block at the age of 29-83 were examined (average age--69 years, females--13, males--14); with cardiac decompensation stage 1-8 patients, stage II--13 patients and stage III--1 patient; without cardiac decompensation--5 patients. During the examination period, no cardiotonics and dehydrating drugs were administered. The examination was carried out prior to and 10 days post the implantation of an electro-stimulator VVI with an echocardiograph M-type in the standard method. The examination prior to the implanted pacemaker manifested moderately to strongly expressed pathological deflection of the mean values of all echocardiographic indices for the pumping and contractile function of the left cardiac ventricle: increased diastolic size, diastolic index and diastolic volume of the left ventricle, mean values of the fraction of contraction, ejection fraction, velocity of circumferential fibers shortening in the lower limits of the norm and increased mean values of stroke volume and reduction of minute volume. The results from the examination are given in Table 1a and 1b. The second examination of the same indices after 10 days gives the improvement of the mean as well as of the final values of the basic echocardiographic indices with a very good correlative coefficient R = 0.92-0.96, P = 0.001. Decrease of the diastolic size with 8.4% was determined as well as of the systolic size with 3.8%, of the diastolic volume with 23.2%, of the systolic volume--with 19.4%, increase of the minute volume with 19.8%.(ABSTRACT TRUNCATED AT 250 WORDS)

[Torsade de pointes ventricular tachycardia].

This kind of ventricular tachycardia presents a special type of tachycardia and the diagnostics is of particular importance in this case. It was described by French authors in 1970 during bradycardia and especially during complete AV block with low ventricular frequency. Arrhythmia present at tachycardial form of atrial flutter, and its treatment with electro-stimulation and sinus rhythm restoration are described.

[The removal of calculi from the pelvic ureter by extracorporeal lithotripsy].

Using high-energy impulses from Dornier HM-3 lithotriptor, iliac ureteroliths were crushed in 18 patients for the period of 2.5 years. The mean age of the patients was 38 years. The lithotripsy was in all cases preceded by retrograde catheterization. Reposition of the stones was achieved in 6 patients. Prone-position lithotripsy was conducted in 12 patients. Specially designed support cushions helped to keep the patient in a steady position, speeded up the stones excretion, relaxed knee joints and reduced the exposure to the impulses. All the patients underwent antiinflammatory and spasmolytic therapy. The mean hospital stay reached 3.2 days. Good results were reported in 94.1% of those treated. Neither early nor long-term complications arose.
