Pegylated protein encapsulated multivesicular liposomes: a novel approach for sustained release of interferon alpha.

Hepatitis C viral chemotherapy suffers from a relatively short half-life of the interferon alpha-2a (IFN alpha). To address this issue, we investigated the effects of polyethylene glycol modification and their subsequent encapsulation in multivesicular liposomes (MVLs), on the release properties of IFN alpha. In the present study, interferon-alpha was conjugated with methoxy-polyethylene glycol (mPEG, MW 5000). Prepared IFN alpha-mPEG5000 conjugate (IFN alpha-mPEG5000) was purified with size exclusion chromatography. The relative in vitro anti-viral activity of pegylated interferon alpha-2a was found to 87.9% of the unmodified IFN alpha. Pegylated IFN alpha encapsulated multivesicular liposomes were prepared by double emulsification technique followed by evaporation of organic solvents from chloroform ether spherules suspended in water. Prepared MVLs were then characterized for shape, size, vesicle count, encapsulation efficiency, and in vitro release rate. In process stability studies of pegylated IFN alpha protein exhibited better stability when exposed to chloroform: diethyl ether (1:1 ratio) mixture as well as variable vortexing time as compared to native IFN alpha. Relatively high percentage of encapsulation of protein ( approximately 75%) was achieved. In vitro release profile of pegylated IFN alpha-mPEG5000 containing MVLs in the PBS showed lower initial burst release with sustained and incomplete release over a period of 1 week. In contrast, native IFN alpha entrapped MVLs were observed as higher initial burst release, i.e., nearly 35% followed by almost complete release. The results confirmed the possibility of multivesicular liposomes as a long-acting or sustained-release delivery system using a combination of pegylation and encapsulation technique for controlled delivery of interferon alpha.

Monteggia fracture dislocation in a neonate resulting from birth trauma: a case report.

Birth-related injuries in an uncommon location may be missed initially and may result in significant morbidity. We report for the first time a case of a Monteggia fracture dislocation in a neonate resulting from birth trauma. The delayed presentation at 7 days made open reduction necessary in this otherwise nonoperatively managed injury because of the accelerated rate of bony union in infants. All high-risk births, including 2nd-born twins, should be thoroughly screened to avoid initial misdiagnosis.

Non-ionic surfactant based vesicles (niosomes) for non-invasive topical genetic immunization against hepatitis B.

DNA vaccines are capable of eliciting both humoral as well as cellular immune responses. Liposomes have been widely employed for DNA delivery through topical route; however, they suffer from certain drawbacks like higher cost and instability. In present study, non-ionic surfactant based vesicles (niosomes) for topical DNA delivery have been developed. DNA encoding hepatitis B surface antigen (HBsAg) was encapsulated in niosomes. Niosomes composed of span 85 and cholesterol as constitutive lipids were prepared by reverse phase evaporation method. Prepared niosomes were characterized for their size, shape and entrapment efficiency. The immune stimulating activity was studied by measuring serum anti-HBsAg titer and cyokines level (IL-2 and IFN-gamma) following topical application of niosomes in Balb/c mice and results were compared with naked DNA and liposomes encapsulated DNA applied topically as well as naked DNA and pure recombinant HBsAg administered intramuscularly. It was observed that topical niosomes elicited a comparable serum antibody titer and endogenous cytokines levels as compared to intramuscular recombinant HBsAg and topical liposomes. The study signifies the potential of niosomes as DNA vaccine carriers for effective topical immunization. The proposed system is simple, stable and cost effective compared to liposomes.

Intrapartum coccygeal fracture, a cause for postpartum coccydynia: a case report.

Coccydynia can result from a varying number of causes, parturition being one of them. Although strains and sprains of the ligaments attached to the coccyx have been thought to be the usual cause for coccydynia occurring after childbirth, an intrapartum coccygeal fracture dislocation can result in the same. A 28-year-old female presented to the orthopaedic department 4 weeks after the birth of her first child with the complaint of coccygeal pain. Examination revealed marked local tenderness over the coccyx but no crepitus was felt. Radiographs established the diagnosis of fracture and posterior dislocation between the second and third coccygeal fragments. Conservative treatment in the form of rest, doughnut ring, local heat, and avoidance of direct pressure over the area resulted in considerable improvement over the next 4 weeks. Coccygeal fracture dislocation may result in introital dyspareunia and tension myalgia of the pelvic floor. Pain from this lesion may become recurrently symptomatic. The diagnosis must be established at the outset and appropriate treatment instituted to avoid these complications.

Fracture of the posterior process of talus.

Fracture of posterior process of talus is quite rare, but is associated with significant morbidity as it involves two articular surfaces. We report two such cases in which open reduction and internal fixation were done with good functional results. One of the cases had a concomitant medial malleolus fracture which to the best of our knowledge has not been reported so far.

Tuberculosis of the clavicle presenting as an expansile lytic lesion: a case report.

An unusual case of skeletal tuberculosis, presenting as an expansile osteolytic lesion in the lateral end of the clavicle is presented. Diagnostic confusion delayed appropriate medical therapy, leading to development of a discharging sinus with secondary infection, which further confused the picture. With re-emergence of tuberculosis as an important infection worldwide, and the ability of this disease to mimic many skeletal pathologies, this has to be included in the differential diagnosis, especially at unusual sites.

Immunoglobulin subclass distribution and diagnostic value of Leishmania donovani antigen-specific immunoglobulin G3 in Indian kala-azar patients.

Visceral leishmaniasis, or kala-azar, a fatal tropical disease, remains problematic, as early diagnosis is difficult and treatment often results in drug resistance and relapse. We have developed a sensitive enzyme-linked immunosorbent assay (ELISA), using leishmanial membrane antigenic extracts (LAg) to detect specific antibody responses in 25 untreated Indian visceral leishmaniasis patients. To investigate the pathogenetic significance of isotype markers in kala-azar, relative levels of specific immunoglobulin G (IgG), IgM, IgA, IgE, and IgG subclasses were analyzed under clinically established diseased conditions. Since LAg showed higher sensitivity for specific IgG than lysate, the immunoglobulin isotype responses were evaluated, with LAg as antigen. Compared to 60 controls, which included patients with malaria, tuberculosis, leprosy, and typhoid and healthy subjects, visceral leishmaniasis patients showed significantly higher IgG (100% sensitivity, 85% specificity), IgM (48% sensitivity, 100% specificity), and IgE (44% sensitivity, 98.3% specificity) responses. Low levels of IgA in visceral leishmaniasis patients contrasted with a 13-fold-higher reactivity in sera from patients with leprosy. Among IgG subclasses, IgG1, -3, and -4 responses were significantly higher in visceral leishmaniasis patients than in the controls. IgG2 response, however, was significantly higher (twofold) in leprosy than even visceral leishmaniasis patients. The rank orders for sensitivity (IgG = IgG1 = IgG3 = IgG4 > IgG2 > IgM > IgE > IgA) and specificity (IgM = IgG3 > IgE > IgG4 > IgG2 > IgG > IgG1 > IgA) for LAg-specific antibody responses suggest the potentiality of IgG3 as a diagnostic marker for visceral leishmaniasis.

Improved renal function following aggressive treatment of urolithiasis and concurrent mild to moderate renal insufficiency.

To our knowledge the prevalence of urinary stone disease and concurrent mild to moderate renal insufficiency has never been reported. A review of our last 2,000 urinary stone patients identified 33 (1.65%) with serum creatinine levels of 2.0 mg./dl. or greater at presentation. Chemical composition, location and complexity of the calculi, types of procedures required to render the patient stone-free and the effect of surgical intervention (independent of relief of obstruction) on renal function were evaluated. Mean serum creatinine level before surgical intervention and after placement of a ureteral stent or percutaneous nephrostomy tube in patients with evidence of obstruction was 3.2 mg./dl. (range 2.0 to 7.5). Complete or partial staghorn calculi were found in 21 of the 33 patients (64%), including 8 with bilateral staghorn calculi. Seven patients required 1 procedure, 1 required urinary alkalization alone and the other 25 required an average of 3.5 procedures each. Stone analysis revealed struvite, mixed calcium and uric acid to be the most common types. Followup creatinine values (the latest available within 1 year) and corresponding creatinine clearances showed remarkable improvement. The mean decrease in serum creatinine level was 1.2 mg./dl. (p < 0.001). There was no statistically significant difference in the rate of decrease between patients with pretreatment serum creatinine levels of 2.0 to 2.9 mg./dl. and those with initial values of 3.0 or more. Renal calculi and concurrent mild to moderate renal insufficiency warrant aggressive treatment. Patients demonstrate significant improvement in renal function independent of relief of obstruction.

Clinical course of endomyocardial fibrosis.

The survival pattern, morbidity, and clinical course of 145 patients with endomyocardial fibrosis who were followed up between November 1975 and June 1987 were studied. The diagnosis was confirmed in all cases by cardiac angiography, or echocardiography, or necropsy. Percentage survival at the end of one and 9.5 years was 76.11 and 26.35 respectively. History, physical examination, electrocardiography, and cardiac catheterisation were studied at the first presentation. The determinants of early mortality were studied by univariate Kaplan-Meier estimates compared by the log rank test and Cox proportional hazards multiple regression analysis. Significant univariate predictors of early mortality were QRS axis above +90 degrees, intraventricular conduction delay (QRS duration greater than 0.12 s), duration of symptoms before presentation, New York Heart Association functional classes III and IV, presence of embolic episodes, right atrial mean pressures greater than 20 mm Hg, right ventricular end diastolic pressure greater than 20 mm Hg, and aortic oxygen saturation less than 85%. The significant multivariate predictors of mortality were cyanosis, New York Heart Association functional class at first presentation, and right atrial mean pressure greater than 20 mm Hg. The bleak prognosis of endomyocardial fibrosis did not substantially improve despite advances in the medical management of congestive cardiac failure during the period of the study.

Antileprotic action of an extract from "Anantamul' (Hemidesmus Indicus R. Br.).

Evaluation of antileprotic activity of indigenous drugs has of late become important. With this idea "Anantamul' an indigenous drug was for the first time tested on mice infected with M. leprae. The results are interesting and encouraging. There seems to be a definite evidence in support of the drug causing a delay in multiplication of organisms in the mouse foot-pads.

Search for potential anticonvulsant agents. Synthesis of 2-phenyl/methyl-3-[o-, m- or p-(benzimidazol-2'-yl)phenyl]-6 or 6,8-substituted/unsubstituted quinazolin(3H)-4-ones.

Sixteen new quinazolones were synthesized by the condensation of substituted 4-oxo-3,1-benzoxazines and 2-(o-, m- or p-aminophenyl)benzimidazoles. All the sixteen compounds were tested for anticonvulsant activity in mice against pentylenetetrazole induced convulsions. Except for five compounds in which there was no protection, the others showed protection ranging from 20 to 80%.
