The occurrence of mechanical nerve injury in connection with anaesthesia and surgery is probably more common than generally believed, and by no means all cases are reported to the patient injury claims department. Both incorrect positioning of the patient on the operating table and pressure from retractors and other instruments can contribute to the occurrence of such injuries. Both neurological and neurophysiological procedures should be used to localise the injury, particularly for the purpose of insurance assessment. Most important of all, however, is prevention of injury. Risk minimisation is dependent on the observation of meticulous routines in surgery units, and clear division of staff responsibilities. The surgical injury rate can be monitored by means of a register of general complications.

Gerontopsychiatric centres are functional settings which provide mainly out-patient care for mentally ill elderly persons. The centres were established in order to transfer the care of mentally ill elderly people from in-patient to out-patient care. This study examines to which extent this goal can be reached and where the limits of this transfer are. Two regions, the city of Bielefeld without gerontopsychiatric centre and the Gütersloh district with such a centre, were compared. It was shown that a transfer from intramural to extramural care was successfully accomplished by gerontopsychiatric centres. The transfer is scarcely limited by kind and severity of the disorder. Often an informal network is necessary for an out out-patient or partially out-patient care. But also with weak networks out-patient care is possible if there is a good-functioning formal network. Gerontopsychiatric centres constitute a central structural element in community-oriented care of the mentally ill elderly. They can help to realize the goal of a self-determinant and independent life.
Twenty-five patients with fractures of the lower leg treated by closed reduction and plaster fixation were examined an average of 7 years and 8 months after trauma. The end result was assessed according to the following: (1) The patient's own opinion; (2) clinical examination including comparing range of motion in knee and ankle in the fractured and non-fractured leg, leg length discrepancy, malrotation of the injured extremity and muscle force analysis; (3) radiological examination. All fractures had healed after a mean of 13.4 weeks. Twelve of the 25 patients had no complaints. Subjective and objective assessment of the end results were in good correlation. Only eight patients had remaining radiological displacement, in only one of which it was severe. The slight remaining malpositionings did not influence the final functional result. The muscle force analysis showed that fractures caused by high-energy trauma also brought permanent reduction of muscular strength to the fractured extremity.
In this pilot study, 14 patients with different symptoms and signs in relation to orthopedic implants were compared with 12 asymptomatic patients to determine the in vitro lymphocyte response to nickel sulfate. We conclude that nickel allergy may explain some, but not most, of the local and generalized symptoms associated with orthopedic implants.
The aim of the present study was to compare sockets for below-knee (BK) prostheses made by Computer Aided Design-Computer Aided Manufacture (CAD-CAM) to those made by hand. The patients in the study were provided with two prostheses each, which apart from the sockets, were identical. One socket was made by the CAD-CAM technique developed at the Bioengineering Centre, Roehampton, University College London and one was made by hand at the OT-Centre, Stockholm, Sweden. The results were based on investigation of eight unilateral below-knee amputees evaluating their own sockets by Visual Analogous Scale with respect to comfort, pressure, and pain. The sockets were evaluated on seven occasions, at two tests, on delivery, after use every second day for six days and every second week for two weeks. All CAD-CAM sockets except one had to be changed once as compared to the hand made of which only two had to be changed. As to comfort it could not be demonstrated that there was any significant difference between the two types of sockets and both types were well accepted by all patients. Differences in pressure and pain were rarely reported. There were obvious differences between the two types of socket with respect to height, width, and inner surface configuration. The authors feel that CAD-CAM will in the near future be an excellent tool for design and manufacture of prosthetic sockets.
In 28 patients the anterior cruciate ligament of the knee was substituted with a bovine bioprosthesis (Xenograft). Five patients underwent reoperations due to synovitis and graft rupture during the first postoperative year; and 2 patients, due to graft rupture after 3 years. After 3 years, 11/21 patients with a remaining graft had a rupture. Totally, 18/28 implanted grafts were considered to be ruptured. Our results indicate that the Xenograft in its present form should not be used.

Fresh tibiae and femora from 22 harriers ranging in age from 8 to 44 weeks have been studied with a computerized torsion-machine. The growth of the dogs studied ceased at an age approaching 30 weeks. The strength and stiffness of the entire bones also ceased to increase at that age, and the increase in stress at fracture and shear modulus of the bone material strongly declined in impetus. This means that no important maturation, as measured in strength of the bone material, takes place after cessation of growth. The fracture twist angle per unit bone length was greatest for the younger bones and decreased to an age of around 25 weeks; the relation, however, between the linear and the non-linear part of the torque-twist curves did not change. Furthermore, the study indicates that juvenile bones, like adult bones, are essentially elastic-brittle, and there was no sign of increased plasticity in the young bones.
A study was carried out of 302 major amputations for vascular insufficiency in the lower limb with respect to levels of amputation, postoperative revisions, re-amputations on a higher level and postoperative mortality. This information was related to vascular disease (diabetes mellitus/arteriosclerosis) and to the experience of the surgeon. There was a high incidence of above-knee amputations both of diabetics and arteriosclerotics and the rate of complications was high for "senior" as well as "junior" surgeons. The amputations were performed during 1978 and the study has shown that there is an urgent need to lower the level of amputation without increasing the rate of complications. The study indicates that there is a need for further information about the problems involved in rehabilitation of above-knee amputees.

By using a newly constructed pin instead of Kirschner wires in the traction absorbing wire (TAW) technique, the clinically observed sliding of the Kirschner wires and resulting skin problems are avoided. The pins and the internal fixation technique are described. In order to demonstrate the simplicity and reliability of the technique a follow-up study 7 years after performed surgery is presented. The late results are given of a series comprising the authors' first 10 consecutive cases of closed transverse olecranon fractures treated by this new technique of internal fixation.

The formation of microcracks in entire canine diaphyseal bones under torsional loads was studied. The acoustic emission technique was used to register the stress waves which were generated in the bones by crack formation and/or growth. Acoustic emission signals, characteristic of crack formation and/or growth, were recorded exclusively during the non-linear deformation. This is a strong indication of the elastic behaviour of diaphyseal canine bone material.
The fracture process of torsion tested entire canine diaphyseal bones was studied with a high speed camera. The speed of the final fracture propagation proved to be incompatible with plastic deformation of macroscopical parts of the bone.
The deformation of entire long bones from dogs has been studied in torsion. The load-deformation relationship is non-linear prior to ultimate failure and repeated successively increased loading and unloading in the non-linear range is found to reduce the stiffness of the bone. The non-linearity of the curve is assumed to be due to a change in the geometry of the bone originating from formation of small cracks in the cortex. The bone material of entire long bones from dogs seems to be linearly elastic for all torques.
