The effect of plasticizers of plastic material bags on biochemical and morphological changes of morphotic blood elements.

The concentration of di-2-ethylhexylphthalate (DEHP) was determined in whole blood, packed red cells and plasma of the blood collected into blood plastic bags manufactured by McGaw, Biotest or Polfa, in samples withdrawn immediately after collection and 1, 2 and 3 weeks later. ATPase activity and electrolyte (K+, Na+, Mg++) composition of erythrocytes were tested and possible morphological changes were inspected in phase-contrast microscope. Platelet aggregation induced by ADP, collagen and adrenaline was also investigated. Blood stored in glass bottles served as a standard. The DEPH concentrations were similar in samples contained in various bags. Maximum DEPH concentration in the whole blood, after 3 weeks of storage, attained 52 microgram/cm3. ATP-ase activity was more rapidly depressed in the blood kept in bags instead of glass bottles. Erythrocyte morphology and results of platelet function tests remained unaffected.

Certain problems of toxicity assessment of plastic materials used for blood preservation and preparation.

Present state of investigations on the noxious effects of contact of blood and its preparations with products made of polyvinyl chloride (PVC) is reviewed. Di-2-ethylhexyl pthalate (DEHP), used for softening of PVC employed for medical purposes, has harmful effect on certain cells and possibly cumulates in the organism. In certain cases (massive transfusion) large amounts of DEHP may penetrate into the organism in quantities exceeding the tolerance of the organism and becoming an additional health-endangering factor. DEHP during storage of blood and its preparations has high migration ability parallelling the storage time, and unequal distribution in different blood components. Although there are no reliable evidence of the risk connected with the using of blood stored in PCV containers, presence of DEHP in the blood during transfusions and its possible effect in interaction with other factors on the patient's condition should be considered. It seems that in making containers for blood and blood preparations PCV should be replaced with other materials or the softeners added to PCV should be replaced with less toxic ones.

[Prolonged storage of erythrocytes].

The authors report the results of biochemical investigations of erythrocytes stored for 70 days at 4 degrees C. For regeneration of phosphate substances in the erythrocytes they were subjected to 2-hour incubation at 37 degrees C parellelly in 3 types of rejuvenating fluids (PIGP, PIGPA, PIGPAd). Regeneration was done on the 28th, 42nd, 56th and 70th days and its results were assessed on the basis of determinations of ATP, 2,3-DPG, erythrocyte osmotic resistance index, percent of their recovery and contents of free haemoglobin. These investigations were carried out immediately after incubation and after 48 hours. The best regenerative properties among these fluids had PIGPAd fluid which raised the levels of ATP and 2,3-DPG in erytrocytes up to the 70th day of storage. In the light of these results it seems that the time of erythrocyte storage can be prolonged o 70 days despite ever increasing erythrocyte loss with prolonging storage.
