Parapharyngeal granular cell tumour: a unique surgical challenge.

A granular cell tumour is a rare lesion of probable nerve sheath origin. It is typically benign but up to seven per cent may be malignant. Since its original description in the tongue in 1926, the tumour has been reported to occur at many other sites in the body. The authors report a case of a 49-year-old African woman with an oro-naso-parapharyngeal granular cell neoplasm causing mild dysphagia. The location of this tumour, which has not been reported previously, posed a unique surgical challenge. An initial attempt to remove the lesion transorally was only partially successful because it was too tough and adherent for conventional surgical dissecting instruments. Complete resection, however, was achieved with a carbon dioxide laser via the same approach. This information may be helpful in the management of other similar cases in the future.

Dercum's disease (adiposis dolorosa).

Dercum's disease (adiposis dolorosa) is a rare condition characterized by progressively painful fatty deposits, usually, in menopausal women with obesity, asthenia and mental phenomena. We report a case of a 48-year-old woman with recurrent neck swelling and pain in the neck and parotid region, and a review of management of this uncommon problem.

Effective securing of a drain.

The importance of effective anchoring of drains cannot be over emphasized. In this paper, we describe a safe and effective method of drain fixation used in 118 cases between January 1998-99 in our department. This useful technique has the advantage of minimizing inadvertent drain displacement with its consequent complications.

Acute on-chronic middle ear disease.

The discharging ear is a common presenting symptom to GPs and otologists. The commonest causes are otitis externa and otitis media. Less common is cholesteatoma, which presents with a chronic offensive mucopurulent discharge and deafness. Acute mastoiditis as a presenting symptom of cholesteatoma is rare and, with the introduction of antibiotics, the incidence has declined even further. We present three cases of cholesteatoma presenting as mastoiditis.

Cannabis in the external ear.

ENT surgeons may well be asked to remove cannabis from the external ear where it has been lodged for various reasons. We report two cases and review four other cases we found in the literature to illustrate some of the difficulties. It is important that anyone working with people who abuse drugs are aware that cannabis can be concealed in the external ear.

Prostatic metastases in the nose and paranasal sinuses.

Prostatic metastases in the nose and paranasal sinuses are rare. Seven cases have previously been reported in the world literature. We describe the clinical presentation of a patient with prostatic metastases and the use of prostate specific antigen in confirming the diagnosis. We also review the literature about metastases involving the nose and paranasal sinuses.

Acoustic rhinometry: validation of volume changes following intra-nasal polypectomy.

Acoustic rhinometry measures nasal cavity geometry by analysis of reflected acoustic impulses. Previously, the technique has been applied to normals, patients with septal deviations and rhinitis and to monitor the medical treatment of nasal polyps. To date, no study has been published to validate the application of this technique where nasal polyps are present. In this study, acoustic rhinometry has been used to assess the change in nasal cavity volume following intra-nasal polypectomy in 20 subjects. The volumes of the polyps removed surgically were measured by displacement and compared with the volume change recorded by acoustic rhinometry. A correlation of r = 0.59 (P = < 0.01) has been obtained. Therefore, it is possible to apply acoustic rhinometry as a method of assessing non-surgical treatment of nasal polyps given certain conditions.

Validation of acoustic rhinometry by using the Cavalieri principle to estimate nasal cavity volume in cadavers.

The ability of acoustic rhinometry to provide valid estimates of nasal cavity volume has been tested using cadaveric heads. Values obtained for the cavities in five intact heads were compared with unbiased estimates of the volumes of the same specimens. An unbiased estimate is one which does not depart in a systematic manner from the true value. Here, unbiased estimates were made by applying the Cavalieri principle to uniform random samples of 4-7 macroscopic slices per specimen. Using acoustic rhinometry, the mean volume (left and right sides combined) amounted to 31.3 ml with a co-efficient of variation of 77%. On the same specimens, the Cavalieri estimate of mean volume (both cavities plus the adjacent nasopharynx) was 31.1 ml (66%). There was a significant and positive correlation between the two data sets (correlation co-efficient 0.97). Repeated acoustic measurements on the same cavities gave a measuring error of about 4%. We conclude that acoustic rhinometry can provide valid and efficient estimates of the volume of the nasal cavity plus nasopharynx.

Fish bones and other foreign bodies.

In this prospective study the types of foreign body ingested, sites of impaction, radiographical findings, management and outcome in 81 patients were recorded. We have demonstrated that foreign bodies can be conveniently divided into 'fish bones' and 'others', in terms of their sites of impaction (P < 0.0001), the age groups affected, whether dentures were worn or not (P < 0.02) and their respective management (P < 0.001). Statistical analysis supporting these observations has not been previously reported.

Posture and nasal geometry.

The influence of posture on nasal cavity geometry was studied by acoustic rhinometry in 14 normal adult subjects. On adopting a supine position from sitting the volume of the initially more patulous nasal cavity decreased and the volume of the less patulous side increased. These changes in volume are statistically significant (p = less than 0.01).

Transnasal electrocoagulation of the vidian nerve.

Vidian neurectomy has previously been reported as having a high success rate in alleviating secreto-motor rhinopathies. We report a prospective study of 24 consecutive patients who underwent transnasal vidian electrocoagulation. An overall improvement in symptoms was found in 59.1%. A worsening of overall nasal symptoms was complained of by 27.3%. The possible reasons for this are discussed.

Endoscopic anatomy of the cerebellopontine angle.

This study describes the endoscopic anatomy of the cerebellopontine angle (CPA). Endoscopes were introduced into the basal cisterns and it was found that the neurovascular structures of this angle could be seen in detail. Endoscopy is likely to complement contemporary microsurgical procedures. The need for minimally invasive surgery may mandate further study of the application of this type of endoscopy in surgical practice. It is stressed that considerable cadaver experience should be obtained with this technique prior to any application in the operating room.

Nasopharyngeal tuberculosis.

Isolated nasopharyngeal tuberculosis is a rare disease with very few cases having been reported in recent years. We report a case presenting in the United Kingdom with unilateral otalgia and otorrhoea.

Unusual cause of intranasal injury.

Intranasal injury without external involvement is not common. To our knowledge this is the first reported case of such an injury, caused by an assailant's two fingers forced up the nostrils of our patient causing severe mucosal laceration and excessive bleeding. The patient died two weeks later.

Carcinoma arising in a pleomorphic adenoma of the epiglottis.

Despite the abundance of minor salivary glands within the larynx, pleomorphic adenomas are rarely found. It is therefore not surprising that there are few reports of malignant mixed tumours in this site. We describe a case of carcinoma arising in a pleomorphic adenoma of the epiglottis which, to our knowledge, has not been described previously. The patient underwent a horizontal partial laryngectomy and functional neck dissection and is well one year postoperatively. We feel this represents a suitable approach to treatment for this rare tumour.
