Protective effect of Ugni molinae Turcz against oxidative damage of human erythrocytes.

Ugni molinae Turcz, also known as "Murtilla", is a plant that grows in the south of Chile. Infusions of its leaves have long been used in traditional native herbal medicine. The chemical composition of the leaves indicates the presence of polyphenols, which have antioxidant properties. In the present work, the antioxidant properties of U. molinae were evaluated in human erythrocytes exposed in vitro to oxidative stress induced by HClO. The experiments were carried out by scanning electron microscopy (SEM) and hemolysis measurements. The SEM observations showed that HClO induced a morphological alteration in the red blood cells from a discoid to an echinocytic form. According to the bilayer couple hypothesis, the formation of echinocytes indicates that HClO was inserted in the outer leaflet of the erythrocyte membrane. However, a concentration as low as 10 microM gallic acid equivalents (GAE) U. molinae aqueous extract neutralized the shape change effect of HClO applied in a concentration as high as 0.25 mM. The significant protection of U. molinae aqueous extract was also shown in the hemolysis experiments. In fact, very low concentrations of the extract considerably reduced the deleterious capacity of HClO to induce hemolysis in red blood cells. It is concluded that the location of the extract components into the membrane bilayer and the resulting restriction on its fluidity might hinder the diffusion of HClO and its consequent damaging effects. This conclusion can also imply that this restriction could apply to the diffusion of free radicals into cell membranes and the subsequent decrease of the kinetics of free radical reactions.

Human erythrocytes are affected in vitro by extracts of Ugni molinae leaves.

Ugni molinae Turcz, also known as "Murtilla", is a plant that grows in the south of Chile. Infusions of their leaves have long been used in traditional native herbal medicine. The chemical composition of the leaves indicates the presence of polyphenols, which have antioxidant properties. In order to evaluate the mechanisms of their antioxidant properties and the toxicity of the aqueous extracts of leaves, the extracts were induced to interact with human red cells, their isolated unsealed membranes (IUM) and large unilamellar vesicles (LUV) of dimyristoylphosphatidyltidylcholine (DMPC), representative of phospholipid classes located in the outer monolayer of the erythrocyte membrane. Scanning electron microscopy (SEM) observations indicated that the extracts achieved a significant alteration in the shape of the erythrocytes as they changed their discoid shape to echinocytes. According to the bilayer couple hypothesis, the shape change indicates that the polyphenols were located in the outer moiety of the red cell membrane. This conclusion was confirmed by the fluorescence experiments performed in IUM and DMPC LUV. In fact, the extracts produced slight initial increases followed by sharp decreases at higher concentrations in the anisotropy and general polarization parameters. These results imply that the extracts induced structural perturbations in the acyl chain and polar group packing arrangements of the erythrocyte IUM and DMPC LUV lipid bilayers: first ordering and afterwards disordering them as the extract concentration increased.

A terahertz molecular switch.

We present time-dependent results describing the current through a molecular device, modeled as a complex with two active centers connected to leads under bias. We show that, at a properly adjusted external voltage, a passing terahertz electromagnetic pulse may cause a transition between states of finite and negligible current, suggesting that the system might be useful as a nanoscopic switch in the terahertz range. A phase diagram defining the bias region in which the transition takes place within a short time is given. As described, the physical processes involved are of an entirely different nature than those in ordinary photodetectors.

[The pediatric prescription according to its origin and the motive for the consultation in a basic rural health area].

OBJECTIVE: To describe the paediatric prescription in a rural health district in terms of the origin and reason for consultation. DESIGN: Cross-sectional descriptive study. SETTING: Garraf Rural Health District, Barcelona. PARTICIPANTS: The 3 basic care paediatrics units in the district. MEASUREMENTS AND MAIN RESULTS: 1068 attendances of children of both sexes under 15 were collected. Recorded on each visit to the paediatrics clinic were: age, sex, reason for consultation (RC), and, if medicine was given, the product prescribed and the origin of the prescription. The number of children receiving treatment increased progressively with age. 87% +/- 2.8% of prescriptions originated with the paediatrician. Therapeutic groups of respiratory apparatus and systemic anti-infection drugs accounted for 69% +/- 2.9% of all prescriptions. Only 2.3% +/- 0.5% had fixed-dose associations. The most common RC was upper-tract respiratory infection, mainly treated with a single therapy (76.6% +/- 6.1%). In the case of the second most common RC, acute bronchitis, 94.6% +/- 4.2% of all cases received drugs treatment with single therapy or two simultaneous treatments. Acute tonsillitis was mainly treated with antibiotics (86.4% +/- 6.6%). CONCLUSIONS: The frequencies of the pathology groups detected in the Health District correlate with other studies' findings. Analysis of the origin of prescriptions shows the high decision-making capacity of the Health District's paediatricians. The pharmacological profile employed is good, though it could be improved.

Effects of winter supplementation and antiparasite treatment on the productive performance of milk herd in the central-south region of Chile.

During the winter of 1994, a survey of productive and reproductive parameters was undertaken in the south-central part of Chile (8th Region) on 71 cows provided by four small farms with low production (< 3000 l/year). A low food intake was recorded (8.6 +/- 0.2-12.4 +/- 0.2 kg DM/day) and, as a result, the cows calved with poor body condition score (1.9 +/- 0.04 points on the scale of one-to-five, had milk production between 10.5 +/- 0.4 and 12.8 +/- 0.6 l/day, had a delayed start of ovulatory activity (78.2 +/- 4.6 days) and an interval between calving and conception of 120.3 +/- 4.6 days. In addition, low levels of packed cell volume (PCV) and hemoglobin (HEM) were found, which were considered to be a combined effect of low feed ingestion and parastic infection by Fasciola hepatica. To improve the productive and reproductive parameters, treatments were undertaken in three of the four small farms; a parasiticide treatment was used against the F. hepatica at the beginning of the winter and the availability of forage was increased via the use of supplementary green oats. Spring crops were also cultivated: corn for silage and oats with peas to make hay. The antiparasite treatment and supplementation of forage was associated with better body condition score at calving, improved hematological parameters, increased milk production (17%), reduced number of days to conception (120 vs. 91) and increased conception rate at first service (36 vs. 60%). In the fourth small farm where no interventions were undertaken, the parameters obtained during 1994 remained unchanged.

[Chorea induced by oral contraceptives. Report of two cases].

We report two women using oral contraceptives, aged 17 and 33 years old, who presented with hemichorea. In both patients all other possible causes of chorea were discarded and the disease disappeared when contraceptives were discontinued. Four months later, the 33 years old patient used again oral contraceptives and chorea reappeared. This rare complication of contraceptive use has been previously reported in young and mainly nulliparous women.

[99mTc red blood cell scintigraphy for the assessment of active gastrointestinal bleeding].

BACKGROUND: 99mTc red blood cell scintigraphy is a non invasive diagnostic method for low flow gastrointestinal bleeding. AIM: To assess the diagnostic yield of this method in patients admitted with gastrointestinal bleeding in whom upper gastrointestinal endoscopy was negative. PATIENTS AND METHODS: The clinical records of 59 patients, aged 6 to 90 years old (35 male), with active gastrointestinal bleeding subjected to a 99mTc red blood cell scintigraphy were reviewed. All had non diagnostic upper gastrointestinal endoscopic studies, and in 20 a selective arteriography was performed. RESULTS: In 40 patients the scintigraphy was positive for gastrointestinal bleeding, and in 57% of these the exam was positive during the first hour. Fifteen of the 19 patients with a negative scintigraphy did not have an active clinical bleeding at the moment of the examination. In 24 patients, a final etiological diagnosis was reached. In 93% of these patients scintigraphy correctly identified the bleeding site. In one patient with a negative scintigraphy, angiography disclosed a pseudo aneurysm of the splenic artery that was not bleeding actively. CONCLUSIONS: In these patients with negative upper gastrointestinal endoscopy 99mTc red blood cell scintigraphy had a 91% sensitivity for the diagnosis of active gastrointestinal bleeding.

Effect of arginine and ornithine infusions on luteinizing hormone secretion in prepubertal ewes.

Although activation of the hypothalamic GnRH pulse generator is known to initiate the onset of puberty in immature animals, the metabolic cues by which information about the stage of body growth and development are translated into central nervous activity remain obscure. In the present study, the function of L-arginine (ARG) or L-ornithine (ORN) as promoters of pulsatile LH secretion was investigated in 2-mo-old prepubertal ewes. Suffolk ewe lambs (n = 17) fitted with intrajugular catheters were divided into three groups. Saline (200 mL) was infused for 1 h into all lambs. In Group 1, saline infusion was continued for another hour. In Group 2, 200 mL of an ARG solution (350 mM, pH 7.4) was infused for 1 h, and in Group 3, ORN was infused (200 mL, 350 mM, pH 7.4) for 1 h. Blood samples were collected at 15-min intervals from 60 min before until 285 min after the end of infusions. The Pulsar program was used to identify significant LH episodes during 285 min after infusions. Mean LH concentrations during the 285-min period after infusion were greater (P < .05) in Group 2 than in Groups 1 and 3. The mean LH pulse frequencies and amplitudes did not differ among the groups. However, in lambs infused with ARG, 13 of their 17 LH pulses had amplitudes > 1 ng/mL, whereas in control ewes only 5 of 20 pulses had amplitudes > 1 ng/mL. All of the ARG-infused ewes had two or more LH pulses with amplitudes > 1 ng/mL; only 1 of 6 control lambs had two pulses. Results indicate that ARG stimulates LH secretion in prepubertal ewes.

[Nationally produced 153SmEDTMP in the palliative treatment of metastatic bone cancer pain].

153SmEDTMP was obtained from enriched 152Sm irradiated at the 5 MW Chilean Research Reactor and labelled at a molar ratio of 15:1 pH 7.5. Biodistribution, autoradiography, radiochemical purity tests were done for evaluation. 40 patients were treated with 37-55 MBq/kg weight. Bone scans using 99mTcHMDP were obtained prior and after treatment. Bone marrow depression was observed in 37% of them and normal liver function in all of them. In 45% the pain dissapear completely, in 22.5% significantly decreased and partially in 30%. In 17 patients more than one dosis was injected. Our preliminary results indicate that 153SmEDTMP is a promising radiotherapeutic agent for palliative treatment of metastatic bone cancer pain and encourage its use specially because it can be produced in countries with low economic resources, thus a large number of patients can get the benefits of this new procedure.

[Study of volume and kinetics of gallbladder emptying in non-pregnant, pregnant, and puerperal women].

Gallbladder stasis apparently plays an important role in the patho-genesis of cholelithiasis during pregnancy. On the other hand, gallstones diagnosed immediately after delivery disappear spontaneously during late puerperium in one third of these patients. Gallbladder emptying was assessed by biliary scintigraphy and ultrasonography in normal, nulliparous volunteers. The two methods had an excellent correlation: thence, we used ultrasonography to determine gallbladder volume and contraction in pregnant and puerperal women. Fasting and postprandial residual volumes were significantly larger during pregnancy, while the kinetics f gallbladder emptying was similar in nulliparous and pregnant women. During puerperium, gallbladder volume returned to the values observed in nulliparae; but the kinetics of emptying was significantly faster, suggesting an increased sensitivity of gallbladder muscle to physiologic stimuli.

Effects of extracorporeal circulation on renal function in coronary surgical patients.

We prospectively studied perioperative changes of renal function in 12 previously normal patients (plasma creatinine < 1.5 mg/dL) scheduled for elective coronary surgery. Glomerular filtration rate (GFR) and effective renal plasma flow (ERPF) were measured with inulin and 125I-hippuran clearances before induction of anesthesia, before cardiopulmonary bypass (CPB), during hypo- and normothermic CPB, after sternal closure, and 1 h postoperatively. Renal and systemic vascular resistances were calculated. Urinary N-acetyl-beta-D-glucosaminidase (NAG) and plasma and urine electrolytes were measured, and free water, osmolal, and creatinine clearances, and fractional excretion of sodium and potassium were calculated before and after surgery. 125I-hippuran clearance was lower than normal in all patients before surgery. During hypothermic CPB, ERPF increased significantly (from 261 +/- 107 to 413 +/- 261 mL/min) and returned toward baseline values during normothermia. GFR was normal before and after surgery and decreased nonsignificantly during CPB. Filtration fraction was above normal before surgery and decreased significantly during CPB (0.38 +/- 0.09 to 0.18 +/- 0.06). Renal vascular resistance (RVR) was high before surgery and further increased after sternotomy (from 18,086 +/- 6849 to 30,070 +/- 24,427 dynes.s.cm-5), decreasing during CPB to 13,9647 +/- 14,662 dynes.s.cm-5. Urine NAG, creatinine, and free water clearances were normal in all patients both pre- and postoperatively. Osmolal clearance and fractional excretion of sodium increased postoperatively from 1.54 +/- 0.06 to 12.47 4/- 11.37 mL/min, and from 0.44 +/- 0.3 to 6.07 +/- 6.27, respectively. We conclude that renal function does not seem to be adversely affected by CPB.(ABSTRACT TRUNCATED AT 250 WORDS)

[Blood pool in the study of hepatic hemangioma].

Blood pool scintigraphy is a useful tool in the diagnosis of hepatic hemangioma. We reviewed retrospectively the clinical histories of 24 patients with abnormal blood pool scintigraphies and a presumptive diagnosis of hepatic hemangioma, based on clinical or ultrasonographic findings. In 20 of these patients, blood pool scintigraphy was diagnostic for hepatic hemangioma. The diagnosis was confirmed by an asymptomatic clinical evolution of 12 to 78 months in all the patients, CT scan in 8, angiography in 5 and liver biopsy in one. In the remaining 4 patients scintigraphy ruled out the presence of a hepatic hemangioma. Our results are similar to previous reports in the literature and blood pool scintigraphy has a high specificity for the diagnosis of hepatic hemangioma.

[Renal scintigraphy with captopril in the evaluation of renovascular hypertension].

The administration of an angiotensin-converting enzyme (ACE) inhibitor transitorily reduces the GFR in a kidney with renal artery stenosis, effect that can be ascertained with scintigraphic studies using ACE inhibitors. We evaluated the clinical usefulness of captopril renography (CR) in the diagnosis of renovascular hypertension (RVH) in 51 hypertensive patients in which this diagnosis was suspected. All subjects underwent angiography and RVH was diagnosed when renal artery stenosis exceeded 60%, there was lateralization of renal vein renin or there was a concordant clinical outcome. Renography was performed 15 minutes after Lasix administration, using Tc-99m DTPA, before and 60 minutes after the oral administration of 50 mg of captopril. The scintigraphic criteria for a positive test were a decreased split renal function, a delayed peak uptake, a decreased excretion of DTPA and a prolonged transit time. In three of 28 patients in whom angiography discarded RVH, CR was positive. In the 23 patients with confirmed RVH, CR was positive in 10 of 12 with unilateral stenosis, in 3 of 8 with bilateral stenosis and 1 of 3 with stenosis in a transplanted kidney. The overall sensitivity and specificity of the test for RVH was 60.9% and 89.2% respectively. There were no changes in blood pressure or adverse effects after captopril administration. We conclude that CR in RVH is useful to select patients for further studies (angiography) and to perform a functional interpretation of angiographic alterations.

Use of radionuclide renal imaging for clinical followup after extracorporeal shock wave lithotripsy of renal stones.

Patients treated by extracorporeal shock wave lithotripsy (ESWL) are usually evaluated by excretory urography within 1 month after treatment to determine the clearance of stone debris and rule out asymptomatic obstruction. In an attempt to obtain more precise functional information, we used 99mtechnetium-diethylenetriaminepentaacetic acid and 131iodine-hippurate radionuclide renal imaging studies, and a plain abdominal radiograph as the initial followup study after ESWL of 64 kidneys in 55 patients. Of 53 kidneys studied within 60 days after ESWL 42 had abnormal radionuclide renal imaging studies demonstrating pelviocaliceal stasis, excretory delay or poor function, 8 of which required subsequent interventions for obstructing stone debris. Five patients had excretory delay after ESWL that was unexpected based on a pre-ESWL excretory urogram showing normal function without dilation. A subset of 23 patients with large stone burden or anatomical deformity from a prior operation had baseline radionuclide renal imaging studies before ESWL; function improved in 4 and worsened in 5 by radionuclide renal imaging studies after completion of treatment. A total of 19 patients had radionuclide renal imaging studies earlier (within 17 days) after ESWL because of poor function and/or large stone burden, and as expected they had evidence of obstruction from stone debris, which necessitated further followup. Our experience suggests that followup of ESWL by radionuclide renal imaging studies provides specific functional information that is of particular value in the management of patients with obstructing stone debris and/or diminished renal function. Radionuclide renal imaging studies may also reveal unsuspected obstruction or functional impairment after ESWL of uncomplicated stones, and is recommended as routine followup after ESWL.

[Diagnosis of activity in Crohn's disease and ulcerative colitis: scintigraphy with TC-99m glucoheptonate labelled leukocytes].

We evaluated a pre-tinning leukocyte labeling technique using Glucoheptonate-Sn lyophilized kit, in the detection of activity in 15 patients with clinical and laboratory signs of active inflammatory bowel disease (IBD). 25 patients, without gastrointestinal disease were the control group. In 7/8 patients with Crohn's disease and in 7/7 with Ulcerative Colitis the study was positive. The negative study corresponded to a patient who has only involvement of the duodenum, an area of difficult evaluation due to the high liver uptake. 3/25 patients of the control group had activity in the intestine at 4 hour images. In spite of the small group evaluated, we believe that this labeling technique is a promising procedure for evaluation of activity in patients with IBD (Sensitivity 93.3%, Specificity 88.0%).

[Early test of thallium dipyridamole after myocardial infarction: prognostic value and correlation with coronarography].

Fifty three patients were studied with dipyridamole thallium myocardial scintigraphy, 4 to 6 days after a first episode of myocardial infarction. Localization of infarction was anterior in 25 and inferior in 28. Infarction was confirmed by myocardial scintigraphy in 87% of cases. A non q wave myocardial infarction was present in 5 of the 7 patients with negative scintigraphy. Residual myocardial ischemia was suggested by myocardial scintigraphy in 68% of patients. Correlated to coronary arteriography, sensitivity for myocardial ischemia was 80%, specificity 82%. After a mean follow up of 11.2 months, 22 of 36 patients with positive myocardial scintigraphy had new coronary events, 15 of them requiring myocardial revascularization. In contrast, only 2 of 17 patients with negative scintigraphy had new events (p < 0.05). Thus dipyridamole thallium myocardial scintigraphy early after myocardial infarction is a valuable prognostic test.
