Keratinocytes isolated from a small skin biopsy and cultured according to the method of Rheinwald and Green (Cell 1975, 6: 331) are able to undergo rapid expansion in vitro and have been used successfully in the treatment of burn wounds. One of the inconveniences of this method involves the transfer of the epidermal sheet from the culture flask onto the wound bed. One way to facilitate this process is to use fibrin glue (Biocol) as a culture bed for the keratinocytes. Burns are then grafted by simply placing the sheet of fibrin glue and keratinocytes onto the wound bed. This process has been successful in two patients, permanently covering areas of 720 cm2 and 5342 cm2. The newly formed epidermis was fully differentiated and histologically normal after 1 year. The efficiency of this improved, faster procedure could lead to a new approach in the treatment of extensive burn wounds.
We examined the expression and the antigenicity of the major surface polypeptides of Leishmania braziliensis braziliensis and Leishmania donovani chagasi, parasites which commonly coexist in the same endemic areas of Bolivia. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis protein profiles from surface-iodinated promastigotes showed the presence of a unique iodinatable polypeptide of 72 kDa on the L. b. braziliensis surface and of two major components of 65 and 50 kDa exposed at the surface of L. d. chagasi. Comparison of the peptide digestion profiles of the major iodinated polypeptides of both strains showed no similarity between the maps of the 72- and the 65-kDa polypeptides of L. b. braziliensis and L. d. chagasi, respectively. Immunoprecipitation of surface-labeled L. b. braziliensis Nonidet P-40 extracts with 35 serum specimens obtained from Bolivian patients with cutaneous and mucocutaneous leishmaniasis showed that all serum specimens recognized predominantly the 72-kDa antigen and high-molecular-mass proteins in some cases. The recognition patterns were independent of the geographical origin of the patient, the type of lesion, and the serum antibody titer. Serum specimens from children with visceral leishmaniasis did not precipitate the L. b. braziliensis 72-kDa antigen. Hamster hyperimmune serum against L. b. braziliensis also recognized the 72-kDa surface antigen. However, this recognition was inhibited in the presence of the homologous nonlabeled antigen but not in the presence of heterologous (L. d. chagasi and Trypanosoma cruzi) antigens. The specific recognition of 72-kDa surface antigen in both natural and experimental L. b. braziliensis infections suggests that this antigen could be a good candidate for use in the differential immunodiagnosis and prognosis of the disease.
A method to reconstruct the perineum, after excision of a giant sacrococcygeal teratoma, using one anterior V and multiple posterior V incisions, is described. A baby weighing 2,400 g with a 1,000 g sacrococcygeal teratoma was operated using this technique.
Nineteen patients with nasal dermal sinuses were operated over a period of twelve years. 12 patients presented with a fistula and 8 with a cyst. One of them presented with a double sinus ostia. In six patients, intracranial extension was confirmed through a surgical approach. Clinical examination and CT scans determined the choice of surgical approach. All sinuses were removed by two surgical procedures: direct incisions, either vertical midline or horizontal glabellar, and nasofrontal osteotomy via a coronal approach. The main theories of dermoid development are explained and embryogenesis is compared to the operative findings.
Ocular atrophy in prime infancy leads to lack of orbital growth. Current treatment of plastic surgery in severe deformities is unsatisfactory, whereas tissue expanding techniques using intraorbital balloon are effective on orbital cavity growth. This procedure makes use of conjunctival shells or aesthetic ocular prosthesis easier, and allows an improved reconstruction. Description of implants and surgical technique of insertion used.
This study describes the identification of aqueous-soluble antigens in Leishmania promastigotes immunologically and biochemically closely related to the major surface antigen. Proteins from surface-iodinated L. braziliensis braziliensis and L. donovani chagasi promastigotes, extracted and separated by partitioning in the detergent Triton X-114, were analyzed. Immunoblotting of the extracted proteins, using homologous antisera, showed recognition of a 72-kDa labeled, amphiphilic antigen of L. b. braziliensis and a 65-kDa surface antigen of L. d. chagasi. The respective homologous sera also recognized non-labeled hydrophilic antigens, similar in their apparent molecular weights to the major surface antigens. The amphiphilic and hydrophilic antigens of each species were found to share common antigenic determinants, inasmuch as monospecific antibodies that recognized the amphiphilic protein reacted with the hydrophilic antigen. Structural homology was also obtained in the peptide-digestion profiles of the amphiphilic and the respective hydrophilic major antigens. Zymogram assay showed that both amphibilic and hydrophilic fractions displayed proteolytic activity that could be directly attributed to the major L. b. braziliensis and L. d. chagasi antigens. The hydrophilic antigens found in this study are probably not hydrolytic products of the surface antigens and occur in large quantities in the promastigote cytosol.
After recalling the mechanism of action of hyperbaric oxygen (HBO) on healing processes, the authors review the principal indications for this technique in plastic and reconstructive surgery, such as crush injuries and acute post-traumatic ischemia of the limbs, skin flaps and skin grafts, when there is a risk of their not taking, and burns. They stress the importance of strict, stratified therapeutic protocols with control of the hyperoxygenation induced by HBO. In the authors' experience, transcutaneous measurements of the partial pressure of oxygen under the hyperbaric atmosphere is a very useful method with a predictive value to determine the indications for treatment with HBO and to monitor its effects.
Large tumors of the clivus require a combination of several approaches. We review the technique previously described; they cross numerous anatomical structures and include a transcranial approach. We propose to combine transoral and transrhino-septal approaches by means of maxillary bipartition which allows good exposure and an easy removal in a case of chordoma of the clivus.
The authors present a review of 34 clinodactylies. We observe 16 delta phalanx, 12 congenital angular inclination, 6 thumb abnormalities. The procedure is required when the angulation is superior to 40 degrees. In this study, 16 cases had surgical treatment (10 digits, 6 thumbs). Additional osteotomy or subtraction osteotomy were performed. The authors prefer this last one. The age of procedure is 4 years. The surgical treatment is difficult and the authors prefer abstention if there is no functional troubles with the growth.

When Mozart died at the age of 36, was he suffering from the belated complications of a calcified extradural haematoma? This theory took shape during the identification process of the skull owned by the Mozarteum, when the print of calcified extradural haematoma was discovered on the left inner temporoparietal calvarial surface of the skull. This print looks like a rosette, with three distinct concentric areas. The first outer area is striated, the second middle one is granular and scattered with bony deposits, the third central one is marked with vascular grooves.

A 20 year old patient who suffered from contusion of the optic nerve due to a complex craniofacial fracture, developed, after orbital reconstruction for sequelae of the fracture, loss of light perception which was transiently improved by drug treatment. The clinical and electrophysiological course suggest the presence of ischaemic optic neuropathy.
We report the case of a child's Jadassohn sebaceous noevus, localised on the vertex cranii, the trunk and the limb, associated with the presence of ophthalmological and neurological abnormalities. The diagnosis of this syndrome has been difficult because all the signs were not present at birth. The surgical treatment of the vertex cranii lesions will be realised after expansion with sub cutaneous prothesis, allowing a large exeresis and an adequate reconstruction. Only the anatomo-pathological examination allows any malignant degeneration of the lesion.
The authors describe a technique of mobilisation of the frontal sinus using the fractures of anterior and posterior walls. The translesional approach is adapted to cranio-facial borders surgery of traumatisms. It's a skull base approach which minimizes the cerebral retraction and enables or perfect viewing of the top of the ethmoid. The advantages, drawbacks and indications are discussed about 25 cases.
Acridine orange was used for staining and counting micro-organisms obtained from 136 skin biopsies performed in burned patients. The number of organisms per gram of tissue was compared to the number of colony-forming-units (CFU) calculated from cultures of the same biopsies. The staining method was positive in 97 per cent of septic samples, and in 25 per cent of these it proved more sensitive than bacterial cultures, with a 100 to 1000-fold greater number of pathogens detected. Acridine orange also demonstrated bacteria in 69 biopsies which remained sterile after culture. In some cases, the same bacterial species was found in other samples taken a few days later.
