Magnetic resonance imaging in partial epilepsy of childhood. Seventy-nine cases.

A heterogeneous series of 79 children with partial epilepsy was studied by computerized tomography (CT) and magnetic resonance imaging (MRI) to evaluate the contribution of MRI to the aetiological diagnosis of that disease. Both CT and MRI were normal in 13 cases and abnormal in 41 cases showing the same pathology (malformation, atrophy, tumour, scar). In 4 cases of continuous partial epilepsy the lesions observed were either diffuse or focal corresponding to the electrical focus, and they could be due to Rasmussen's encephalitis. In 19 cases where CT gave normal results, MRI showed bilateral high-intensity signal areas of uncertain significance in the white matter of the caudal part of the hemispheres. All considered, it seems that with its greater sensitivity MRI should be the first or only exploratory method to be used in the evaluation of partial epilepsy of childhood.

Cefotaxime in the cerebrospinal fluid and serum in patients with purulent meningitis.

Cefotaxime concentrations in the cerebrospinal fluid and serum were determined in patients with purulent meningitis by means of a simple, rapid and reproducible method in agar medium. The CSF concentrations of cefotaxime fluctuated around 4 mg/l. The pharmacokinetics of the antibiotic in relation to the integrity of the blood-brain barrier was studied by means of an assay of the albumin and IgG present in the cerebrospinal fluid and in the serum.

[Plasma cell acid phosphatase in the differential diagnosis of monoclonal gammopathies].

Acid phosphatase activity was studied cytochemically in bone marrow plasma cells of 32 multiple myelomas, 45 non-myelomatous monoclonal gammopathies and 20 normal subjects. We have found significant differences among these three groups (P less than 0.001). The usefulness of this cytochemical reaction for the study of monoclonal gammopathies is discussed.

In vitro antibacterial activity of thiamphenicol.

This in vitro study of thiamphenicol activity against 247 strains of Gram-positive and 195 strains of Gram-negative pathogenic bacteria showed no significant changes from past activity. Of the 442 strains involved, thiamphenicol proved: - Effective (MIC = less than or equal to 0.5-4 micrograms/ml) against 131 strains, mostly streptococci, pneumococci and enterococci. - Moderately effective (MIC = 8-64 micrograms/ml) against 252 strains, mostly staphylococci, Salmonella, Citrobacter, K. pneumoniae, Enterobacter, E. coli and Proteus. - Ineffective (MIC = 64 micrograms/ml) against 59 strains, mostly Proteus, E. coli and K. pneumoniae. Against 23 strains of Neisseria gonorrhoeae, thiamphenicol generally proved as effective as penicillin G and cefuroxime and more effective than spectinomycin.

[Laboratory diagnosis of acute monocytic leukaemias].

The haemathological features of nine patients with acute monocytic leukaemia are presented. The peripheral blood and bone marrow showed differentiated and apparently undifferentiated blasts. It was necessary to type these blast cells using cytochemical reaction for specific and unspecific esterases. It is very important to differentiate this variety of acute leukaemia from the other non limphoblastic leukaemias for different prognosis and therapeutic assays.

[Data on a comparison between a rapid test and urine culture for the determination of bacteriuria].

A streep test has been employed for bacteriuria involving identification of nitrites in the urine and results are compared with those obtained with classical bacteriological techniques. Parallel performance of the nitrite test and urine culture showed that the latter cannot be replaced as far as diagnosis goes but it also proved the usefulness of the nitrite test for mass screening purposes.

[Considerations on the antibiotic activity (M.I.C.) revealed in 247 serotypes of Salmonella].

247 strains of Salmonella, isolated in Brescia province duriing 1973-1974 have been typed by serological methods. The MIC has been determined, using the following antibiotics: chloramphenicol, tetracycline HCL, ampicillin, doxycycline, rifampicin, cephazolin, carbenicillin, nifuratel, gentamicin, aminosidine, trimetho-prim-sulphamethoazole, nalidixic acid. The pattern of resistence of the various serotypes is quite constant. The relationship between diffusion and epidermiological factors are discussed.
