Crystal-induced inflammation in canine joints. II. Importance of polymorphonuclear leukocytes.

In an experimental model, the polymorphonuclear leukocyte was found to be necessary to the inflammation induced by crystals. This conclusion is based on (a) the invariable migration of polymorphonuclear leukocytes into the synovial fluid of canine joints injected with urate crystals, (b) the experimental suppression of the synovitis by the depletion of polymorphonuclear leukocytes in animals treated with vinblastine, (c) the dependence of the degree of suppression on the degree of leukocyte mobilization into the joint space, and (d) the restoration of the inflammatory response in a leukopenic animal by perfusion with normal blood.

Crystal-induced inflammation in canine joints. I. An experimental model with quantification of the host response.

Injection of sodium urate or calcium pyrophosphate crystals into the stifle joints of anesthetized dogs almost invariably induced an acute exudative response. This response was quantified by serial measurements of intra-articular pressure, pH and leukocyte concentration. Pressure rose progressively and reflected intra-articular volume increase. The hydrogen ion concentration increased as the reaction progressed and correlated in a given exudate with the leukocyte concentration. Analysis of sequential physiologic and biochemical changes occurring in this model of crystal-induced inflammation may provide insight into the mechanisms of acute gouty arthritis in man.

The efficacy and safety of auranofin compared to placebo in rheumatoid arthritis.

This double-blind multicenter study compares the effect of adding auranofin (AF) 3 mg bid or placebo to patients already taking nonsteroidal antiinflammatory drugs for rheumatoid arthritis. The 242 patients who completed 3 months of therapy demonstrated that the group receiving AF responded better than those receiving placebo. Of the 144 patients who completed 6 months coded medication, the efficacy in the AF group was superior to the placebo group in several parameters including a reduction in the number of painful or swollen joints, grip strength, dropout rate, and global efficacy as judged by the evaluating physician. A significant lowering of the Westergren erythrocyte sedimentation rate and immunoglobulin levels was noted in the AF treated patients. This study includes data summarizing the difference between both groups with respect to on-therapy conditions and toxicity.

Demonstration of chemotactic factor in human gout: further characterization of occurrence and structure.

The early events in gout are not clear. In the present studies a chemotactic factor having a molecular weight of 8,400 has been identified in human gouty synovial fluid. Washed crystals from a tophus were shown to generate chemotactic activity when added to polymorphonuclear leukocytes. Amino acid analysis demonstrated this chemotactic factor to be relatively rich in glycine, serine, aspartic acid, glutamic acid and alanine. Evidence from canine experiments suggest that chemotactic activity may also be produced by synovial lining cells. The amount of chemotactic activity that can be demonstrated diminishes over time in joints repeatedly injected with urate crystals.
