[Osteomyelitis of the spine and its surgical treatment--personal experience].

PURPOSE OF THE STUDY: Evaluation of surgical treatment of osteomyelitis of the spine, determination of efficient diagnostic and therapeutic criteria. MATERIAL: The examined group comprised in total 27 patients. No selection was made, all patients who were operated on and treated at the Department of the authors were followed gradually as they were admitted at the Department. METHODS: Evaluation was based on the monitoring of the clinical condition of patients including laboratory tests. Evaluated was also the achievement of vertebral body fusion three months after the surgery. The surgical procedure always contained a radical debridement of inflammatory focus, a massive autogenous cancellous bone grafting mixed with Gentamycin foam, either separately or in combination with instrumentation. Approach to the inflammatory focus was in 9 cases transforaminal posterior, in 2 cases transforaminal combined with transversectomy, in 2 cases transforaminal combined with laminectomy, in 4 cases transthoracic, in 1 case transabdominal, in 7 cases retroperitoneal and once percutaneous technique was used. RESULTS: All patients operated on healed without complications. Local pain and temperature subsided rapidly (in terms of days). Also objective inflammatory criteria returned to normal (in terms of weeks). Seven patients show improved neurological status. X-ray examination 3 months after the surgery always shows consolidation and ingrowth of cancellous bone grafts without local relapse or loss of correction. DISCUSSION: The numbers of patients of the authors' group correspond with the numbers of patients presented in literature. There is also agreement with other authors that a modern treatment of spinal osteomyelitis should include a radical debridement of the focus combined with instrumentation, neurological decompression and application of cancellous bone grafts completed by local antibiotics. As compared to other authors who present a prevalence of abscess the histological examination of our cases shows predominantly inflammatory granuloma. CONCLUSION: Radical surgical treatment of spinal osteomyelitis including debridement, cancellous bone grafting, internal fixation (instrumentation) and decompression together with a general treatment and orthotic management seems to be an efficient way of the treatment of this serious disease.

Oxidation of methanol, formaldehyde and formic acid by methanol-utilizing yeast.

Methanol-utilizing yeast, Candida boidini 11 Bh, characterized by high tolerance to methanol during growth, displays even higher tolerance when the oxidation rate by intact cells is tested. Low respiration activity is found even at 22% v/v of methanol. The half-saturation constant was 17-18 mM. The half-saturation constants for the two oxidation intermediates, formaldehyde and formic acid were 3.5-4.0 an .d 30-33 mM, respectively. When applied together with standard concentration of methanol, very low concentrations of both intermediates stimulated the oxidation rate. These results are discussed in connection with the relationship between growth and oxidation, the tolerance to high concentrations of inhibitory products and the mechanism of inhibition.

Application of mathematical optimization methods in microbiology.

Mathematical methods of experiment design have so far found little use in the optimization of microbiological processes. The conventional optimization procedure is still the transformation of functional relationship of n variables into n unidimensional optimizations; furthermore, the Box-Wilson gradient method is often used. This paper presents a review of methods used in other fields, and their application in microbiological practice. The methods are classified according to whether they require, besides the simple determination of the objective function (direct search methods), also the finding of its first (gradient methods) or second derivative (Newton-Raphson method). A modified Rosenbrock's method of direct optimum search and the gradient Box-Wilson method were used in parallel for the optimization of yeast growth on methanol. Their comparison showed that Rosenbrock's method is more suitbale for multiparameter systems.

The effect of methanol, formaldehyde, and formic acid on growth of Candida boidinii 11 Bh.

The dependence of the specific growth rate of Candida boidinii 11 Bh on methanol concentration follows Monod-type kinetics only in a narrow concentration range (0--0.8 v/v), with a saturation constant of about 19 mmol. With a further increase in methanol concentration of up to 3% v/v, the specific growth rate remains constant. Beyond this level, the growth rate gradually drops to zero, reaching the threshold value at 7.8% v/v. The specific growth rate is also strongly dependent on the size of the inoculum. The biomass yield decreases with an increase in the methanol concentration. Formaldehyde and formic acid, oxidative products of methanol, approximately 100 and ten times, respectively, more toxic to growth than the original substrate.

Application of modified Rosenbrock's method for optimization of nutrient media used in microorganism culturing.

The Rosenbrock's procedure has been modified for optimization of nutrient medium composition and has been found to be less tedious than the Box-Wilson method, especially for larger numbers of optimized parameters. Its merits are particularly obvious with multiparameter optimization where the gradient method, so far the only one employed in microbiology from a variety of optimization methods (e.g., refs, 9 and 10), becomes impractical because of the excessive number of experiments required. The method suggested is also more stable during optimization than the gradient methods which are very sensitive to the selection of steps in the direction of the gradient and may thus easily shoot out of the optimized region. It is also anticipated that other direct search methods, particularly simplex design, may be easily adapted for optimization of medium composition. It is obvious that direct search methods may find an application in process improvement in antibiotic and related industries.
