Non-traumatic sixth nerve palsy in a young patient.

Non-traumatic sixth nerve palsy in the 20-50 years old group is unusual and a detailed history is often needed to identify serious and potentially reversible causes. We present a case of sixth sense nerve palsy in a young female with eczema for which she was treated with low-dose cyclosporin. We review the indications for and ocular side effects of cyclosporin, a drug with which most ophthalmologists will be unfamiliar.

Iris dialysis and retinal dialysis caused by coin throwing.

A 13-year old boy was hit in the right eye by a coin that was thrown at him. He presented to the accident and emergency (A&E) department with an oblique corneal abrasion, microscopic hyphaema, iris-ciliary body dialysis, commotio retinae with retinal haemorrhages and a large retinal dialysis. The limited amount of available data in the medical literature suggests that eye injuries as a result of coin throwing are serious and should always be referred to an ophthalmologist for evaluation.

Laser trabeculoplasty primary therapy in open-angle glaucoma. A preliminary report.

Thirty-three patients with chronic open-angle glaucoma were treated with argon laser trabeculoplasty as the primary therapeutic modality. In 18 patients (55%) successful normalization of intraocular pressure (less than 21 mm Hg) was achieved. Of these, ten patients underwent bilateral procedures and eight patients underwent unilateral treatment. The IOP was normalized within five weeks in 27 of these 28 eyes. During the follow-up period, ranging from six to 18.5 months, IOP has remained lower than 21 mm Hg in all the successfully treated eyes. In 15 patients (45%) the treatment failed to control IOP by four weeks. In these patients, only one eye was treated. Significant correlations with successful outcome included initial IOP, well-delineated angle landmarks, and date of treatment. The date of treatment was consistent with an improvement in the treatment administration over time.

Familial exudative vitreoretinopathy associated with familial thrombocytopathy.

Two families with familial exudative vitreoretinopathy were studied in which platelet aggregation defects were found in all the affected members. The major defect observed was absent platelet aggregation with arachidonic acid. In addition platelet aggregation with collagen and adrenaline was reduced in one severely affected member. The implication of the platelet aggregation defect in the pathogenesis of this retinal vascular disorder is discussed.

Treatment of radiation retinopathy.

A case of radiation induced retinopathy with neovascularisation arising from the optic disc and retina was treated with panretinal argon laser photocoagulation. Complete regression of the new vessels was observed within 2 weeks of therapy. The pathogenesis of neovascularisation in this condition is discussed.
