Characteristics of blunt and personal violent injuries.

This study was designed to analyze the characteristics and behavior patterns of individuals involved in nonfatal, traumatic injuries. There were 547 patients included in the study with 363 sustaining blunt trauma injuries, 144 sustaining personal violence injuries, and 40 being burn victims. Motor vehicle accident victims tended to be young, single, white, employed males: substance use was detected in 32%, and 57% were unbelted. Motorcycle accident victims tended to be young, single, unemployed males: substance use was detected in 25% and 90% were not wearing helmets. Pedestrians struck tended to be single, unemployed males. Penetrating injuries involved mostly young, single, black males: substance use was detected in 35% of patients and most incidents occurred from 4:00 P.M. to 8:00 A.M. Assault victims were mostly young, single, black males with substance use detected in 48%. Suicide patients tended to be white males. The incidence of repeat victims was one out of ten for blunt trauma, and one out of five for personal violence injuries and burns.

A prospective randomized study evaluating periumbilical versus infraumbilical peritoneal lavage: a preliminary report. A combined hospital study.

Peritoneal lavage has been proved to be a fairly safe and reliable diagnostic tool in evaluating blunt abdominal trauma. However, the operation is not always a simple procedure. In an attempt to determine the ease and reliability of the procedure, the authors conducted a combined hospital approach, prospectively randomizing the performance of periumbilical and infraumbilical approaches to the peritoneal lavage. To date, 53 lavages have been performed. Reliability of the procedures appears to be equal. The greatest differences in the techniques are with regard to the greater efficacy and performer acceptance of the periumbilical approach. The operative approaches are discussed in detail and literature regarding peritoneal lavage is reviewed.

Evolving changes in the pathogenesis and treatment of the perforated gallbladder. A combined hospital study.

Perforation of the gallbladder occurred in 35 patients in this 6 year review, with a 2.3:1 male predominance in contrast with a female dominance in nonperforated acute cholecystitis. Thirty-three percent of patients with gallstones had a history of symptomatic cholelithiasis which emphasizes that if elective cholecystectomy had been performed, this complication could have been avoided. Further, a large number of cases (40 percent) were found to be of the acalculous variety which suggests a possible changing trend in the pathogenesis of perforated gallbladder. Cholecystectomy with intraoperative cholangiography and adequate drainage appears to be the procedure of choice, and aggressive operative intervention without delay is thought to contribute to the relatively low mortality of 8.6 percent in this series.

Effects of cyproterone acetate on growth and androgen metabolism of rat ventral prostate transplants.

Cyproterone acetate (CA0 (10 or 20 mg kg-1 daily for 5 days) injected into castrate rats supplemented with testosterone propionate (TP) caused a significant decrease in ventral prostate weights in situ; no inhibitory effects were noted in transplant weights. CA failed to significantly inhibit DNA in either the in situ prostate or transplant, but his anti-androgen caused a significant decrease in the formation of 3H-dihydrotestosterone and 3H-androstanediol from radioactive testosterone in the in situ organ. Histologically, CA caused a decrease in epithelial cell secretory activity.

Effects of pesticides on 3H-dihydrotestosterone binding to cytosol proteins from various tissues of the mouse.

These in vitro studies, utilizing cytosols prepared from various tissues (viz. anterior prostate gland, seminal vesicle, kidney and liver) taken from mice 3 days post-castration, were undertaken in order to determine whether or not certain pesticides could alter the binding of 3H-DHT to cytosolic androgen binding components. Estrone, cyproterone acetate, and unlabeled DHT were employed to confirm the presence of specific 3H-DHT binding components in the cytosols. The effects of various pesticides on 3H-DHT binding in these tissue cytosols were then assessed. Parathion (10(-8)--10(-5) M) was found to be an effective inhibitor of total 3H-DHT binding in the prostate, seminal veiscle, kidney and liver. This organophosphate was unable to compete with 3H-DHT for cytosol binding sites in the intestine. Similar in vitro binding studies using dieldrin, DDT, or carbaryl failed to reveal any interference with 3H-DHT binding in any of the tissue studied. The mechanism of parathion's interference with 3H-DHT binding is unclear.
