Protocol for delayed contrast enhancement in computed tomography of cerebral neoplasia.

One hundred cranial computed tomograms of adults with known or suspected intracranial neoplasm were analyzed retrospectively. Rapid high-dose intravenous contrast infusion (84.6 g l) was followed by immediate and 1 1/2-hour delayed scans. Delayed images afforded more information than the initial series in 67% of cases. 11.5% false-negative examinations (i.e., no tumor reported) would have resulted if studies were terminated after the immediate post-infusion scans. No patient experienced clinically detectable renal compromise.

Enhancement of the white matter following prophylactic therapy of the central nervous system for leukemia: radiation effects and methotrexate leukoencephalopathy.

The authors report a case of fatal necrotizing leukoencephalopathy following prophylactic therapy of the central nervous system for acute lymphoblastic leukemia. The clinical, CT, and neuropathological findings are described. The CT scan demonstrated symmetrical white-matter enhancement. Histological analysis was consistent with the effects of irradiation and methotrexate. The differential diagnosis of the clinical and CT findings is discussed. Brain biopsy is the diagnostic procedure of choice.

Atrial myxoma with intellectual decline and cerebral growths on CT scan.

A woman with left atrial myxoma had progressive disturbance of intellectual function, headache, and prominent constitutional symptoms. The absence of stroke is noteworthy. Multiple high-density lesions with contrast enhancement were seen by CT scan, suggesting metastatic neoplasms, hematomas, or hemorrhagic infarcts. Serial study showed progressive enlargement of the lesions. The only cerebral lesion examined histologically proved to be an organizing hematoma, originating most likely from adjacent small arteries found to be plugged by embolic myxoma.
