Clinical results of the triangular external fixation device in serious lesions of the lower extremity.

Several types of external fixation devices exist among which those of Judet, Hoffmann, Wagner and Müller can be picked out, each of which has definite applications. Confronted with difficulty in the use and the defect of mechanical rigidity, we have been led to design a new type of the triangular external fixation device. As open fractures of the limbs in stage II and IV of Duparc and Cauchoix, and pseudoarthrosis infected need a considerable stability in addition to careful surgical treatment, so after a comparative mechanical study, and after codifying the most rigid installation, we have used it on several traffic accident victims.

[Abdominal tomometry. 1st results in exploration of the liver and pancreas].

Over a two month period, 120 patients were explored using an Acta Scanner tomometer for various hepatic disorders. In parallel, comparison was made of anatomical slices and computed tomography findings in cadavers and isolated livers. This made it possible to recognise normal structures seen at computed tompgraphy and to identify certain hepatic images which might be sources of error. Serial transverse axial sections were used to reconstruct the whole liver in space, thereby making it possible to eliminate certain cases of pseudotumoral hepatomegaly. Systematic densitometry was carried out using hepatic radio-anatomical and pathological studies. This new type of analysis led to a quantified diagnosis in certain diffuse hepatic disorders and a histological approach to tumour lesions. Hepatography using special contrast agents in very low dosage was used in liver computed tomography after animal experiments. This method, free of untoward effects, makes possible the diagnosis of hepatosplenic tumour lesions less than one centimetre in diameter.

Systematic densitometric analysis was made by radiological anatomical study and in hepatic pathology. This new type of analysis led to a quantified diagnosis of certain diffuse hepatic diseases and a histological approach to tumour lesions. Hepatography using liposoluble agents in very low dosage was used in hepatic tomometry after animal experimentation. The method, free of untoward effects, makes possible the diagnosis of hepato-splenic tumor lesions less than one centimetre in diameter.

[Experimental basis of acupuncture analgesia].

The research realised at INSERM, Unité 103, on the acupuncture analgesia in animal has brought to light certain phenomenon concerning physical, histological and physiological facts. The acupuncture point is a point having the least electrical resistance in relation to the surrounding teguments. The analgesia points studied has an even lower resistance than the other points of the same dermatoma. This is not bound to the phenomenon of surface, as it is also found in cadaver after having cleared the skin with alcohol, ether or acetone. The microscopic analysis of certain points in man and in animal has shown specific elements of the acupuncture point, under the form of a thickness of the epiderm, a modification of the collagen fibers of the derma, of the vascular spiral vessels surrounded by a network of a amyelinic fibers of the cholinergic type, with interlaced myelinic fibers. The acupuncture analgesia of the hind limb can be reproduced experimentally in rabbit, in a proportion of 72 percent in 58 rabbits. The algometric method chosen was the pin prick of the skin. This territorial analgesia can be transmitted by the serum on another rabbit.
