[Recent alcohol and drug consumption in victims of work related accidents].

BACKGROUND: Twenty to twenty two percent of work related accidents have a direct or indirect relationship with recent alcohol use. AIM: To detect recent alcohol or drug use in patients who required hospitalization for severe work related accidents. PATIENTS AND METHODS: Blood and urine samples were obtained from patients admitted to a hospital within 6 hours of an accident severe enough to require hospitalization. Blood alcohol and urinary metabolites of cocaine, marijuana, amphetamines and benzodiazepines were measured. Patient identity was kept confidential throughout the study. Once patients were stabilized, they were interviewed by a psychologist that was blind to laboratory results. RESULTS: Two hundred thirty men and 18 women were studied and 29% (30% of men and 17% of women) had recent use of alcohol or drugs. The most common substances were alcohol in 15% and benzodiazepines in 13%. Use of alcohol or drugs was found in 21% of professionals, in 33% of other occupational levels, half of agricultural workers and 25% of commerce workers. Only two of 60 positive cases, admitted substance use during the interview. Most of negative cases admitted the use of alcohol or other drugs during the month prior to the accident. CONCLUSIONS: Recent use of alcohol and drugs is frequent among severe work related accident victims. Probably, a high proportion of these subjects are problem drinkers. The combination of alcohol and benzodiazepines is an additional risk factor.

A prospective, randomized study comparing highly selective vagotomy and extended highly selective vagotomy in patients with duodenal ulcer.

The recurrence rate of duodenal ulcer after highly selective vagotomy is nearly 10 percent. To diminish this percentage, extended highly selective vagotomy with sectioning the gastroepiploic nerves has been proposed in order to reduce postoperative gastric acid secretion. We have prospectively compared the decrease in gastric acid secretion through measurement of basal acid output, maximal acid output, and peak acid output in patients who underwent highly selective vagotomy or extended highly selective vagotomy. No significant differences in postoperative gastric acid secretion were found and, therefore, no changes in the probability of postoperative recurrence of duodenal ulcer were seen.

A prospective study of intraoperative histologic antrum and corpus boundary in patients undergoing highly selective vagotomy for duodenal ulcer.

The results of the present study demonstrate that, at the level of the proximal branch of Crow's foot of Latarjet, parietal cells are found, and therefore, it must be sectioned when performing highly selective vagotomy. At the level of the distal branch, antral mucosa is found. At the level of the middle branch, in almost one-half of the instances, parietal cells are found, and its section must be evaluated with gastric emptying studies.

[Endoscopic esophagitis findings in patients with gastroesophageal reflux symptoms].

The experience concerning the findings of endoscopical esophagitis in patients with reflux symptoms studied between January 1984, and December 1986, is presented. Endoscopical esophagitis, according to the European classification was found in 5% of the patients. Grade I (mild) and Grade III (Barrett) were the most frequent findings. Symptoms were similar to those reported in the international literature. No difference in age, sex distribution, for each group of esophagitis were seen. The distance of the squamous-columnar limit from the incisors was significantly higher in patients within esophagitis group III or IV.
