Dietary supplementation with n-3 polyunsaturated fatty acids (n-3 PUFA) has been shown to reduce proteinuria in experimental models of renal diseases, but their potential role in the treatment of human renal disease is unknown. We administered n-3 PUFA in the form of triglycerides [with eicosapentaenoic (EPA)+docosahexaenoic (DHA) = 3 g/day into 4 patients] and of ethyl esters (EPA+DHA = 7.7 g/day) into 10 patients (one patient twice) with chronic glomerular disease (membranous glomerulonephritis and focal glomerular sclerosis), all diagnosed histologically. Serum albumin was > 2.4 g/dl and serum creatinine < 2.5 mg/dl in all patients. Treatment was given for periods of six weeks, followed by a prolonged follow-up for 27 weeks in 10 cases. Dietary supplementation with n-3 PUFA caused the expected reduction in platelet generation of thromboxane B2 (mean +/- SEM, from 490 +/- 70 ng/ml at baseline, to 342 +/- 147 ng/ml at 6 weeks, P < 0.05) of serum triglycerides (from 236 +/- 60 to 170 +/- 43, P < 0.01), and a prolongation of the bleeding time (from 5.8 +/- 0.4 min to 7.7 +/- 0.4 min, P < 0.01) in patients treated with ethyl esters. A modest but significant reduction in serum total cholesterol was noticed (from 275 +/- 27 to 252 +/- 24 mg/dl).(ABSTRACT TRUNCATED AT 250 WORDS)

The introduction of hemodiafiltration in the substitutive treatment of chronic renal failure has arisen from the need to find new techniques that could improve the efficiency of the traditional systems and, therefore, to treat adequately chronic uremia in the shortest possible time. 61 months of experience at our institution have shown that such a goal can be achieved, offering also practical advantages such as a better quality of life for the patients as well as the possibility to treat a greater number of them.
Soft and Hard Hemodiafiltration (HDF). A 32-month study was made to assess efficiency and clinical tolerance in a comparison between "soft" and "hard" HDF. Soft HDF was performed in 18 patients using a polyacrylonitrile AN 69 S 1.2m2 membrane and hard HDF in 12 patients using a polyacrylonitrile hollow-fibre 1.8m2 membrane. Different technical parameters were used for each of the two methods. Biochemical data registered during a single session showed the efficiency of both kinds of treatment and in this study clinical tolerance and patient well-being were significantly better than with hemodialysis. Hard HDF permits considerable shortening of treatment time.

In 14 patients the kinetics has been studied of two contrast media labelled with 131I, and namely of MGM diatrizoate and of diatrizoate of sodium and MGM. The clearance was determined both at basal concentrations and in double concentration by means of external counting. The results were statistically studied by means of the linear regression method. The authors conclude that the MGM diatrizoate is exclusively eliminated through the glomeruli both in basal and double concentration. The diatrizoate of sodium and MGM is eliminated in low concentration through the glomeruli, while in double concentration it is partially eliminated through the tubuli.
1. Postural changes of plasma renin activity (PRA) before and after the administration of a beta-blocking agent (oxprenolol) were studied in nine patients with renal homotransplantation and in ten normal subjects. 2. In normal subjects PRA always increased during upright posture without any correlation with postural changes in mean arterial pressure. Oxprenolol reduced the postural increase of PRA. 3. A postural increase of PRA could be detected as early as 20--25 days after renal transplantation, and appeared with increasing frequency as time elapsed. 4. There was a significant inverse correlation (r = -0.794, P less than 0.001) between the postural changes of PRA and those of mean arterial pressure. 5. These results suggest that in patients with renal homotransplantation the postural increase of PRA can only partly be due to circulating catecholamines or the sympathetic nervous system and may be explained by inverse changes in the mean arterial pressure.
