The stratified-population-at-risk (SPAR) model for psychogeriatric nursing: a paradigm for the third millennium?

There is warning of a coming epidemic of older people with psychiatric problems. However, there are already too few specialist nurses to ensure that all older people receive care concordant with accepted professional standards. This problem is an obstacle to the World Health Organization health for all by 2000 objective. The Stratified Population at Risk (SPAR) model is a proactive paradigm for planning psychogeriatric nursing services that includes mental health promotion. The SPAR model assumes that-under certain conditions-all people are vulnerable to psychiatric problems, but differ in their level of risk. The model includes an innovative role for psychogeriatric nurse specialists.

Gender differences in survival of 234 patients referred to a psychogeriatric service.

OBJECTIVE: To explore the survival characteristics of psychogeriatric patients. Participants and settingAn historical cohort of 234 patients consecutively referred to a specialized psychogeriatric service proximal to a general health service in Perth, Western Australia. METHODS: Linked health service data were analysed using relative survival analysis and actuarial methods. RESULTS: Relative survival of the cohort after 40 months was significantly lower than the general population of the same age, sex and calendar period (0.78; 95% CI=0.70-0.86). Male patients experienced twice the mortality rate of female patients after adjustment for age (MRR=2.10; 95% CI=1.37-3.20). Age, dementia, mood disorder and ethnicity had no independent effects on mortality in male patients. Female patients with a diagnosis of dementia experienced twice the mortality of female patients without this diagnosis. The distribution of major underlying causes of death was similar in males and females. CONCLUSIONS: Gender-specific factors appear to affect survival in psychogeriatric patients. Male patients are younger on average, but experience higher mortality than female patients. Ethnic background does not influence mortality in either male or female patients.

Mental health services in Perth nursing homes.

Directors of nursing (DONs) in 42 nursing homes in Perth were asked for information concerning mental health services provided for their residents. A questionnaire similar to those used by other researchers in Sydney and in Ontario was utilised to enable meaningful comparisons among the three studies. According to the DONs' perceptions, the mean proportion of residents in Perth nursing homes with psychiatric and/or behavioural problems was between 50% and 75%. The mental health services provided to nursing homes were significantly less than that desired by the DONs. Other variables assessed in this study included the number of transfers per year because of psychiatric problems; the number of staff with psychiatric qualifications; the provision of psychiatric training for staff; the most frequently occurring psychiatric problems in residents and the most valued mental health services provided to nursing homes. Results were comparable to those of the Sydney and Ontario studies. This study suggests that there is a significant psychiatric population in Perth nursing homes that, like those in Sydney and Ontario are seriously neglected regarding appropriate professional psychiatric care. Existing psychogeriatric community assessment teams could provide more of these services if adequately staffed and resourced.

The determined state of white expression in the Drosophila eye is modified by zeste1 in the wzm family of mutants.

Analysis of the whitezeste mottled (wzm) mutant family suggests that the zeste gene product functions in establishing and stabilizing a transcriptionally active chromatin domain for white locus expression. The z1 mutation reduces expression of paired or proximate copies of white, while single or unpaired copies maintain wild-type levels of expression. The wzm mutation, caused by the insertion of the retrotransposon BEL into the 5' intron of white, alters the zeste-white interaction to produce a mottled eye phenotype in hemizygous z1 wzm males. We have determined the molecular structure of four wzm derivatives. wzl results from the insertion of an additional transposable element into the 5' regulatory region of white. wzvl is a deletion of sequences upstream of the white locus. Two others, whalo and wcres, result from the transposition of wzm plus the entire verticals-roughest region into heterochromatin near the tip of chromosome 3L. They variegate for roughest but not for white; rather, the z1 effect on wzm now causes white expression to become non-autonomous and non-clonal. The analysis of these five mutations shows that the neomorphic zeste1 product, in combination with structural changes imposed by transposons and intercalary heterochromatin, modifies the determination and stability of white expression. We propose that the normal zeste product functions as part of a complex that stimulates transcription by changing chromatin conformation to establish and maintain transcriptionally active domains. The unpairing of homologs is proposed to be one of the initial results of conformational change, providing an explanation for the role of zeste in transvection.

The meaning of change to individuals within a college of nurse education.

Change is an inevitability in all aspects of our lives today, with the prospect of an exponentially increasing rate of change. This study attempts to provide a clearer understanding of the meaning of change to the individual within a college of nurse education. The thoughts and feelings of individuals towards change are often subjective and contradictory in nature. A cultural perspective using a phenomenological approach was therefore adopted. Using the attributes of a 'new paradigm research' enabled a harmony to be achieved between the methodology and the paradoxical elements of change and the individual. The study identified that good change is dependent upon a partnership between manager and managed or teacher and student. It became evident that a true partnership based on open, supportive and trusting relationships was deficient in some areas. Three themes concerning the individual and change evolved as being important: (a) a perception of the individual's need to be self-empowering, self-helping, self-caring, self-motivating; (b) the need for the individual to be resilient to change and reflective in practice; and (c) a perception of change as a learning experience.

Struma ovarii: hyperthyroidism in a postmenopausal woman.

A rare case of struma ovarii producing hyperthyroidism in a postmenopausal woman is reported. The ovarian tumor demonstrated uptake of both [99mTc]pertechnetate and 131I, allowing preoperative diagnosis of the condition. In females with unexplained hyperthyroidism and low 131I uptake by the cervical thyroid gland, imaging of the pelvis should be considered.

Primary amyloidosis of the ureter and renal pelvis.

We present a rare case of primary amyloidosis involving the ureter and renal pelvis. Histologic examination is required to distinguish this condition from transitional cell carcinoma. Intraoperative diagnosis of amyloidosis by frozen section may allow for a conservative surgical approach.

Asymmetrical pairings of transposons in and proximal to the white locus of Drosophila account for four classes of regularly occurring exchange products.

An explanation for the origins of four classes of regularly occurring duplication and deficiency chromosomes is provided through examination of their molecular structures. The duplications and deficiencies occur as the reciprocal products of crossing-over, following two different patterns of asymmetrical synapsis between transposons positioned in and proximal to the white locus of Drosophila melanogaster. Three copies of the retrovirus-like transposon roo are involved in the exchanges. Evidence suggests that transposon-mediated asymmetrical exchange is a general phenomenon in eukaryotes, which adds significantly to the effects of transposons in the restructuring of eukaryotic genomes.
