Carcinoma in situ of the larynx: an evaluation of its natural clinical course.

The outcome of 75 patients with severe dysplasia or carcinoma in situ of the larynx has been reviewed. The patients were divided into three groups according to treatment; Group A (16 patients) had received irradiation. Group B (24 patients) had developed invasive carcinoma less than 1 year after the diagnosis of carcinoma in situ and Group C (41 patients) had been observed for more than 1 year, receiving no treatment other than repeat biopsies. Of the latter group, 46% developed invasive carcinoma of the vocal cords, with a mean observation time of 51 months. Mean age at the onset of the disease was 66 years in patients where the epithelium returned macroscopically to normal during observation compared to 59 years in patients who developed carcinoma (P less than 0.05). This may indicate that subgroups of carcinoma in situ exist with different biological behaviour. No extra benefit was seen for the patients who received irradiation for carcinoma in situ, and close follow-up with, eventually, stripping of the epithelium is therefore advocated. Both altered anatomical site during observation and an increase in area of the lesion should be regarded as a warning signal.

Epithelial distribution of a myelomonocytic antigen LI in relation to cutaneous malignancies and melanocytic naevi.

The distribution of LI antigen in four common skin tumours was examined immunohistochemically in 40 samples of formalin-fixed routine biopsy material. Presence of LI in the dermis depended on the extent and composition of the inflammatory infiltrate; LI was found mainly in reactive histiocytes and neutrophils. Intense epidermal staining was seen adjacent to squamous cell carcinoma, basal cell carcinoma and malignant melanoma, but not adjacent to melanocytic naevi. In specimens of melanocytic naevi, LI staining around pilosebaceous units was weak or absent. Around the three malignant types of lesion, epidermal LI positivity faded distally--thereby indicating the extent of the tumour. LI was, in addition, expressed by the most highly differentiated cells in squamous cell carcinomas, whereas the other neoplasms were negative. Antibody to LI may, thus, be a useful adjunct for distinction between various types of skin tumours.

Ovarian morphology in patients with polycystic ovaries and in an age-matched reference material. A statistical evaluation of 149 cases.

The ovarian morphology in 149 patients with the clinical syndrome of polycystic ovaries (group I) is described. The ovaries from 10 age-matched women (group II) with no signs of ovarian disorder were investigated as a reference material. The number of cysts documented for group I was twice that of the reference group and the ovarian volume three times that of the normals. No significant difference in follicle number was seen between the two groups. The tunica albuginea was thicker in group I with more pronounced atretic changes. The duration of symptoms for group I correlated significantly with body mass index, thickening of tunica, degree of stromal hyperplasia and the calculated means of ovarian volume. A correlation was also seen between ovarian volume, degree of follicular atresia and stromal hyperplasia, as well as between the thickness of the tunica albuginea, number of follicles and degree of follicular atresia.

Carcinoma in situ of the larynx. Results with different methods of treatment.

A series of 81 cases of severe dysplasia or carcinoma in situ of the larynx was retrospectively analysed to evaluate the results of radiation therapy and repetitive stripping/biopsies. An average of almost five years elapsed between the diagnosis of severe dysplasia/carcinoma in situ and carcinoma. Of patients primarily treated with irradiation 19% developed invasive carcinoma compared to 15% in the group primarily treated with repetitive stripping/biopsies, and radiation therapy for failures. The mean age at diagnosis for patients in whom the epithelium normalized was 66.1 years, compared to 59.3 years for those in whom the process progressed to carcinoma (p less than 0.05). It thus appears that carcinoma in situ may comprise at least two different lesions with different biological behaviour.

Epidermal and dermal distribution of a myelomonocytic antigen (L1) shared by epithelial cells in various inflammatory skin diseases.

The L1 antigen is a major cytosol component of human granulocytes that may also be expressed by macrophages and epithelial cells. Its epidermal and dermal occurrence was investigated in formalin-fixed routine biopsy material from eleven different inflammatory skin disorders. Localization was performed with a rabbit antiserum to L1 applied in an unlabeled antibody peroxidase-antiperoxidase method. L1 antigen was not found in normal skin except in epithelial cells of pilosebaceous units. However, epidermal L1 antigen was demonstrated in every biopsy specimen from lupus erythematosus, lichen planus, dermatitis herpetiformis, and atopic dermatitis, whereas granuloma annulare test results were usually negative. The occurrence of dermal L1 antigen depended on the composition of the inflammatory infiltrate; specimens rich in neutrophilic granulocytes (e.g., dermatitis herpetiformis) were particularly strongly stained. Extracellular dermal staining was also seen, especially in areas adjacent to accumulation of positive leukocytes. The varying epidermal occurrence of L1 antigen in skin diseases probably signified different degrees of proliferative activity of the epithelial cells and could apparently not be ascribed to uptake from the dermis.

Vulval lichen sclerosus et atrophicus treated with etretinate (Tigason).

In an open uncontrolled study eight patients with vulval lichen sclerosus et atrophicus were given etretinate (Tigason) 1 mg/kg/day. The dose was then gradually reduced according to effect and side effects. Six patients showed improvement in symptoms and in clinical morphology based on the physician's evaluation and photographic documentation. In four of five patients where biopsies were taken, there was a marked change in the histological picture towards normalization. Two patients did not respond to the treatment. All patients experienced the well-known side effects of etretinate in various degree. We conclude that treatment with etretinate should be tried in vulval lichen sclerosus et atrophicus if the result of other therapeutical efforts are unsatisfactory.

Primary biliary cirrhosis associated with coeliac disease and dermatitis herpetiformis.

A 69-year-old man who 1 year previously got the diagnosis primary biliary cirrhosis presented a highly pruritic rash on the knees, elbows and in the inguinal region. Both clinically and histologically this was compatible with dermatitis herpetiformis, and dapsone treatment confirmed this. He was found to have jejunal atrophy, and after 1 year with gluten-free diet, biopsy showed almost full restoration of the villous pattern. He then only rarely had any skin problems.

Oral abnormalities in the Saldino-Noonan syndrome.

The oral and dental features of a case of the Saldino-Noonan lethal short rib-polydactyly syndrome (SNS) are described. Natal teeth were noted. The anterior maxillary and mandibular fornices and the central labial frenula were absent. The tongue appeared larger than normal and lacked the sulcus terminalis and the vallate and foliate papillae. The tooth anlagen were abnormal. Microscopic studies revealed small tooth buds, in which hard tissue formation was more advanced than gestational age; abnormalities were noted primarily in the most recently formed dental tissue, indicating that the biochemical defect responsible for this disorder had acted abnormally on dentinogenesis mainly shortly prior to birth. Studies of oral and tooth development should be important to better understanding the abnormal function of the chondrodystrophy genes.

Lethal acute gamma radiation accident at Kjeller, Norway. Report of a case.

On September 2, 1982, a 64-year-old man accidently received a heavy dose of gamma radiation in a plant for sterilization by radiation. He developed an acute radiation syndrome. The leukocyte count rapidly diminished to low values, and extensive chromosome injuries could be demonstrated in cultured lymphocytes. He was hospitalized in an isolated room, and received large doses of antibiotics, and transfusions of leukocytes, platelets and blood. He died anuric on the 13th day. From spectroscopic analyses of electron-spin resonance in irradiated material the mean dose in the whole body was estimated to be 22.5 +/- 2 Gy. The course of the illness and the findings at autopsy are briefly described.
