Using platelet-rich plasma to accelerate soft tissue maturation in esthetic periodontal surgery.

The transposition of connective tissue to increase the zone of keratinized tissue and/or for root coverage has become an integral part of the surgical dental practice. In an effort to reduce the incidence of postoperative complications occasionally observed with these procedures, the incorporation of platelet-rich plasma and its growth factors has been introduced into the surgical protocol. Documentation of several cases requiring esthetic soft tissue grafting procedures and the use of regenerative barriers to obtain root coverage with the incorporation of platelet-rich plasma is presented.

Reconstruction of severely resorbed atrophic maxillae and management with transitional implants.

The reconstruction of the severely resorbed maxilla requires complex surgical treatment sequencing. Often, multiple grafting procedures are required either before or in conjunction with implant placement. Regardless of the surgical modality, the grafting procedures and the placement of implants in poor quality bone require undisturbed healing during which no pressure is placed on the grafted implant ridge. The use of transitional implants allows the surgeon to provide stable temporary prostheses throughout the healing phase, while preventing pressure from being placed on the grafted or implant reconstructed ridge throughout the maturation. These transitional implant-supported temporaries allow the implant team to maintain vertical dimension, and they provide the patient with the benefits of implant-supported restorations during the time leading up to final prosthetic reconstruction.

Using transitional implants during the healing phase of implant reconstruction.

The use of dental implants to reconstruct the fully and partially edentulous jaw has been well-documented in the literature. Simplification of the healing phase with transitional implants is becoming a routine step in the management of the integration period for the patient undergoing implant reconstruction. Restoration and maintenance of vertical dimension with transitional implants in conjunction with implant surgical therapy is an effective method to provide the patient with an immediate and comfortable transitional appliance. This approach facilitates the uneventful reconstruction with the definitive prosthesis.

Fixed temporization and bone-augmented ridge stabilization with transitional implants.

Utilization of dental implants in full-mouth restorations is now a well-accepted treatment modality, with numerous modifications and implant systems documented in the literature. The efficacy of the treatment procedure generally requires an extended postplacement healing period prior to loading the implant fixture with the stress of mastication. Until recently, clinicians have not been able to address patient comfort requirements during the healing period. The teaching objective of this article is to present and evaluate a transitional implant system used to provide function during the healing phase. The system consists of thin titanium transitional implants and a three-component overdenture that is intended to absorb the pressure during function and protect the augmented implant site and the definitive implant fixtures from the stress of immediate loading. Treatment objectives for the transitional and definitive implants are made during the initial treatment planning. Three cases are presented to document and illustrate the clinical procedure.
