[Palliative treatment of colonic tumors with laser and afterloading irradiation].

Laser photoablation was undertaken in 46 patients (27 women and 19 men, mean age 81 [66-93] years) with malignant tumours of the colon, either for tumour bleeding (n = 7) or stenosis (n = 39), and in 27 patients combined with afterloading 192Ir irradiation. Operation had been refused by 14 patients, 22 were primarily inoperable, one received preoperative radiotherapy for ileus. The radiotherapy was inadequate in only one patient who had to be operated on for subileus. Eleven patients have had a mean survival time of 13.5 months, 22 died after a mean of 7.5 months. No patient died from ileus or any other complication of the palliative tumour treatment. These results suggest that combined laser and afterloading irradiation is an alternative treatment of malignant tumour of the lower intestinal tract or for patients who decline surgical treatment.

[Significance and assessment of the compatibility of mixed solutions with fat emulsion components for parenteral nutrition].

Combined solutions can greatly facilitate the carrying out of parenteral nutrition. The compatibility of their components as well as the stability of such solutions for the duration of clinical use are prerequisites for their application. The necessary and extensive tests of compatibility are illustrated on the basis of a combined solution that is easily produced in the clinic for peripheral and parenteral nutrition. The limits to the production of combined solutions are described.

Amino Acids

[A new possibility in treating stenosing tumors of the upper gastrointestinal tract].

A new method for treating inoperable patients with stenosing malignant tumors of the upper gastrointestinal tract consists in the combination of laser beam followed by direct internal radiation of the tumor using the afterload technique. It brings rapid improvement of dysphagia and resumption of oral feeding. Improvement in quality of life such as this has been achieved in 21 of 24 patients without any signs of complication due to therapy.

[A case of bleeding angiodysplasia of the colon].

Report on a patient with bleeding angiodysplasia of the ascending colon. The clinical course was characteristic for this kind of lesions: long-term, chronic, recurrent intestinal hemorrhages with a gradual increase of anemia. Diagnosis was complicated by lack of success in finding the source of the hemorrhage and by evidence of various pathological findings. Histologically, there were vessel variations typical for angiodysplasia and, furthermore, cholesterol emboli of the submucosal colic arteries. The latter's pathogenetic role as trigger for the hemorrhage is discussed.

[Serum amino acid determination after different blood processing and storage methods].

UNLABELLED: Blood samples of four volunteers were prepared for amino acid analysis in different ways using 39 methods. The result was that after initial deproteinisation of the blood samples different storage temperatures ranging from -18 degrees C to -78 degrees C and different sorting periods from 2 to 6 months did no influence the amino acid spectra. Storage at -4 degrees C did not show a clear result, however slightly decreased glutamine levels. It is possible to allow blood samples to stand at room temperature for further processing for at least 60 minutes, without adverse influence on the analytical result. CONCLUSION: If it is impossible to carry out a quantitative amino acid analysis immediately after blood withdrawal, the blood sample can be stored for several months at not more than -18 degrees C provided that within about 90 minutes after blood withdrawal centrifugation and deproteinisation has been carried out.

[Testing antacid with intragastric Ph measurements and gastroscopy].

In connection with the testing of an antacid in form of a suspension, pH-measurement and gastroscopy are carried out in the same 11 patients. Essential criteria of the efficacy of an antacid are its buffering capacity and its retention in the stomach as a mechanical protective for the gastric mucosa. The Talcid suspension tested in this way showed a good period of stay in the stomach, often exceeding the period of acid-binding capacity. Compared with bicarbonate, its buffering period is about twice as long above pH 4.5 and about 2 1/2 times as long at lower pH's.

[Diet therapy in chronic kidney failure].

The dose of proteins of high biological value and eventually the use of selective diet with protein mixture of highest value such as the potato-egg diet, the dose of essential aminoacids as part of the so-called Sweden-diet and finally the application of ketoanaloges of essential aminoacids, all these measures make possible a removal of all uraemic symptoms as well as a long lasting rehabilitation inspite of a strongly restricted renal detoxication.

Parasitoses of the intestinal tract are rare in Europe. Their diagnosis is difficult since the general symptoms are frequently only little pronounced or completely lacking. On the other hand, the course of the disease may be fatal. Such cases from the literature and our own observations are intended to call to memory the differential diagnosis of intestinal parasitosis.

[Human nitrogen balance under oral administration of egg proteins].

Long-term nitrogen balance periods over 17 and 24 days were performed in 5 healthy volunteers (2 females, 3 males) in order to evaluate the requirements under varying input of whole egg protein. Nitrogen balances are consquently influenced in a positive or negative way by increased or decreased protein intake. The estimation of protein requirements, an important precondition for evaluation of biological value of proteins and/or amino acid mixtures, however, is not influenced as far as experiments are conducted in the linear region between input and balance. In spite of considerable variations of protein intake maximum alterations of requirements of 20, and average ones of 10 per cent points are shown. Even under most unfavorable conditions of comparison potato egg pattern of amino acids administered orally and parenterally exceeds egg protein, given orally, at least by 7, atthe most by 27 points, and reported values of amino acids according to ROSE'S or to the plasma pattern by 36, 23, and 21 points.
