Effect of ponsinomycin on cyclosporin pharmacokinetics.

The influence of treatment with ponsinomycin, a new macrolide antibiotic, on the pharmacokinetics of cyclosporin A has been studied in 10 renal transplant patients. The pharmacokinetics of cyclosporin A was investigated at steady state, before and during treatment with ponsinomycin. On average, the blood levels of cyclosporin A were doubled by the macrolide, possibly due to a decrease in elimination or/and to an increase in absorption. Ponsinomycin should be use very carefully in patients treated with cyclosporin A.

[Primary malignant non-Hodgkin's lymphoma of the breast. A study of 5 cases].

Five cases of primary non-Hodgkin's lymphomas of the breast (stage IE in the Ann Arbor staging system) are described. No clinical or radiological feature has permitted to make the diagnosis before surgery. Histologically, all lymphomas are diffuse and B type. Three cases are high grade lymphomas, one of lymphoblastic type. Two cases are of lymphoplasmacytoid type, one of them associated with an occult invasive ductal carcinoma within the same breast, the other changing in high grade lymphoma at the recurrence time. Three of them reveal histological aspects of lymphoepithelial lesions and appear as mucosa-associated lymphoid tissue lymphomas (MALT). The differential diagnosis with a carcinoma is now easily done by immunohistochemical techniques. The importance of a diagnosis before surgery by cytopunction and drill-biopsy has to be emphasized, to prevent an useless mastectomy.

[Adrenal myelolipoma and Castleman's pseudotumor. A case of association in a retroperitoneal tumor].

The authors report a case of a voluminous retroperitoneal tumor as an incidental finding in a patient with acute urine retention induced by an ovarian compressive cyst. Radiological findings supported the hypothesis of a malignant tumor. The histological examination of the surgical specimen only permitted the diagnosis and showed the association of an adrenal myelolipoma with a Castleman's pseudo-tumor.

[Multiple cancers: a case with 5 different tumors].

We report the very unusual observation of an 85-year-old woman who developed five different tumors in distinct sites over a 12-year period. These tumors were, in the following order of appearance, an infiltrating lobular carcinoma of the breast, an endometrial carcinoma with an ovarian granulosa cell tumor, a clear cell adenocarcinoma of the kidney, and a urothelial carcinoma of the bladder. Two factors may have been involved in the genesis of the multiple carcinomas in this patient, i.e. a hormonal factor (hyperestrogenemia) and radiotherapy.
