[Therapy concept in phyllodes tumors of the breast].

The data of 22 women with a phyllode tumor of breast operated in our department from 1980 to 1997 were analysed. 11 benign, 7 borderline and 7 malign phyllode tumors were found in accordance to WHO-classification. In conclusion diameter of tumor is a relevant prognostic factor beside histologic classification. Therefore local excision is recommended for benign and borderline phyllode tumors smaller than 5 cm. A safety distance of 1 cm is claimed. Benign or borderline lesions greater than 5 cm and all malign require a simple mastectomy. Only recurrence smaller than 5 cm after excision of benign and borderline phyllode tumor should excised once more. Metastasis have to be cured by surgical therapy.

[Use of surgical staplers in gastrointestinal surgery].

Reported in this paper is the use of staplers in gastro-intestinal surgery. Three soviet-made types of staplers, UO 40/60, NShKA, and SPTU (comparable to TA 30/55/90, GIA, and EEA) were used on the upper part of the gastro-intestinal tract in 237 cases and in colorectal surgery on 66 patients. The UO stapler worked well, when used for closure of uncomplicated duodenal stumps, though no absolute indication can be claimed for such application. The SPTU type, on the other hand, proved to be an ideal solution in cases of oesophago-jejunostomy with intrathoracic localisation of the anastomosis. Another, nearly absolute indication for the use of the SPTU can be claimed for anterior rectum resection (especially in male patients) with narrow pelvis and tense pelvic floor.

[Electrometry of wound healing].

Methods, advantages, and disadvantages of electrometry on wounds are reported. The author's results were obtained from measurements by help of the DC-Milli-Pico-Meter MV 40 (VEB Präcitronic Dresden) on 20 rats with model wounds during a 22-days period. A relation between average values of wound voltage and time of healing as well as the considerable ranging of the single values could be proved. Because of this ranging the use of the method on patients is questionable. Since the transfer of the results from animal experiments on the human yields problems for several reasons, a clinical checking of the method appears rational.
