[Initial experience with anorectal manometry with the PC Polygraf apparatus in surgery].

The authors present their initial experience with anal manometry using a PC Polygraph of Synectics Medical Co. They discuss the manometric examination of 14 patients with ulcerative colitis, incl. 12 who were operated. In all 12 patients an ilea-pouch-anal anastomosis was made. By manometry a postoperative drop of pressures at rest was found and partly also of pressures during muscular contraction as well as shortening of the zone of high anal pressure. The resulting values, however, did not influence continence, except for temporary soiling, the frequency of bowel movements being 2-8. Anal manometry is an important objective examination of the sphincter function as it makes it possible to compare values before and after therapy. It must be however combined with other anorectal physiological tests such as endosonography, EMG, defaecography etc.

[Comparative study of 2-piece colostomy systems (ConvaTec Combihesive Flexible, Coloplast 2002 and Hollister Guardian in the Czech Republic)].

In three surgical centres in the Czech Republik three modern two-piece colostomic systems where compared which are at present available in the Czech republik, incl. an attempt to assess how they influence the quality of life of the patients. The investigation comprised 117 patients mean age 59 years-93 after colostomy, 13 with ileostomy. The investigated systems were ConvaTec Combihesive Flexible, Coloplast 2002, Hollister Guardian two-piece ostomy systems with a closed sac and inner filter. The main investigated parameters were: tolerance of the products, preference expressed by the patient, the state of the skin beneath the plaster and the index of general health (GWBI). From the investigation ensued that in the Czech Republic the system ConvaTec Combihesive proved very useful and there probably will not be a demand for a change to another system offered by distinguished producers, as such a change would not entail any advantages. It was revealed that when the ConvaTec Combihesive system is used, the skin is maintained in a good condition which has a favourable impact on the feeling of wellbeing and improves the quality of life.

[Subtotal colectomy in emergency situations].

One-stage subtotal colectomy is the most radical solution of ileous conditions caused by an obturating tumour of the left half of the colon. The authors report on their experience with this procedure in 10 patients operated in the course of three years. They emphasize the advantages such as oncological radicality, immediate detoxication of the organism, a favourable postoperative course with a low morbidity (10% dehiscences) and lethality (10%), shorter hospitalization period, life of the patients without a stoma, lower costs and satisfactory functional results. For an experienced surgeon, if perfect intensive postoperative care is available, this operation is the method of choice even in very old patients.

[Evaluation of the quality of life in patients with stomas].

During the past five years in the Czech republic marked improvement of care as regards patients with stomas was achieved and thus also improvement of the quality of their life. The author made a survey, using questionnaires, among patients with stomas operated at the author's department. About half the patients in productive age returned to their jobs, sexual life was discontinued only in one third of the patients under 60 years of age. As regards social life, sports and travel activities in these spheres increased by 9-15%, as compared with a similar survey made in 1990 in the same department. 75% of the patients with stomas consider the stoma bearable. Lack of care of stoma patients involves in particular poor information on the patient's part during the perioperative period and as regards the social sphere.

[Use of the Convatec Wound Manager and Stomahesive flanges in local treatment of external gastrointestinal fistulas].

The authors used in 1989-1992 for local treatment of postoperative external gastrointestinal fistulas the Wound Manager of Convatec Co. This aid ensures perfect protection of the skin, a good access to the fistula and dehiscent wound and accurate removal and measuring of the secretion. The authors used as an alternative method Stomahesive Convatec flanges with permanent suction. In this way during the above mentioned period 16 patients were treated at the Surgical Clinic of the Institute for Postgraduate Medical Training, Faculty Hospital Bulovka in Prague. Thirteen patients were treated by conservative methods only, but also in the remaining three prolonged conservative treatment made the operation possible. The total lethality was 25%. The authors recommend these aids as an integral part of comprehensive treatment of postoperative enterocutaneous fistulas.

[Perianal pyoderma].

Perianal pyoderma (hidradenitis suppurativa-acne conglobata) is a chronic disease which in its progressive form cannot be treated by conservative methods. Extensive suppurative foci must be radically excised incl. all systems of fistulas right down to the fascia and the defects which develop are allowed to heal per secundam or after several weeks they can at a different period of time be covered by a skin transplant. The authors present their 30-year experience with surgical treatment of 39 patients. Treatment was in all instances successful after radical surgical excision. All these patients have fully recovered as regards work capacity as well as social aspects.

[Late results of ileorectal anastomosis in proctocolitis].

In 1961-1986 at the Surgical Clinic of the Institute for Postgraduate Medicine in Prague 49 colectomies were performed with ileorectal anastomosis. The operations were usually made in several stages. The remote results were evaluated after an interval of 10 to 26 years in 33 patients and were good to satisfactory in 82%. Malignant degeneration in the rectum was not observed. The relatively favourable results can be ascribed to careful selection of patients and frequently also the multi-stage procedure. Ileorectal anastomoses are still justified in the surgical treatment of proctocolitis.
