[The acute cerebellar ataxia, reccurrence of disease after 5 years--a case report].

Acute cerebellitis, also known as acute cerebellar ataxia, is a rare inflammatory syndrome characterized by cerebellar dysfunction. It typically occurs as a primary infectious, post-infectious or post vaccination disorder, typically in early childhood. The author presents a case of recurrent, acute cerebellar ataxia in a 20-year-old woman. In the described case the first event of acute cerebellar ataxia occured at the age of 15. The disease lasted 3 weeks and symptoms disappeared entirely. The similar symptoms recurred after 5 years but they were more pronounced and accompanied by pleocytosis, high protein level and low glucose level in cerebrospinal fluid. The symptoms disappeared entirely after 2 months of treatment.

[Application of numeric integration in waveform morphology of visual evoked potentials in optic nerve diseases. Numeric integration method].

PURPOSE: Presentation of new waveform analysis of pattern and flash visual evoked potentials (VEPs) in patients with optic neuropathy. MATERIAL AND METHODS: The study involved 48 patients with acute optic neuritis (2nd group), 60 patients with demyelinating (3rd group), 32 patients with compressive (4th group), 24 patients with nonarteritic anterior ischemic optic neuropathy (5th group) and 32 healthy subjects (1st group). Using computer system UTAS E-1000 (pattern and flash stimulation) the VEPs tests were performed. Amplitude, latency of 8 VEP components, 4 gradients, mean waveform and 16 areas of numeric integrals were calculated. Results were analysed with the use of AN-STAT software. RESULTS: Statistical difference recorded under pattern stimulation was obtained in latency of 3rd, 4th, 5th components of VEP in comparing 3th and 4th groups, as well as amplitude of 3rd waveform, second gradient, areas of integrals between cursors number 2 and 4 in comparing 3rd and 5th groups (p < 0.001). Statistical difference recorded under flash stimulation was obtained in latency of 4th component in comparing 2nd and 5th groups, in amplitude of 7th waveform, areas of integrals covering 160 and 224, 224 and 256 ms, between cursors number 2 and 4 in comparing 4th and 5th groups, as well as in a range of areas of integrals covering 160 and 192 ms in comparing 4th and 5th groups (p < 0.001). CONCLUSION: Results prove clinical potency of the presented new comprehensive method in analysis of VEP in optic nerve diseases.

[Evaluation of some clinical parameters in patients with retinitis pigmentosa in relationship to the type of inheritance].

PURPOSE: Analysis of selected clinical parameters in patients with various types of retinitis pigmentosa. PATIENTS AND METHODS: We examined 51 patients aged 8-67 suffering from different genetic forms of RP identified on the ground of pedigree analysis. We analyzed onset of nyctalopia, degree of retinal degeneration, degree of visual field loss, dark adapted electroretinographic testing flicker (ERG) and presence of posterior subcapsular cataract. CONCLUSIONS: XLRP is one of the most severe RP forms. In the groups with ADRP and sRP, the patients have varied expression of disease.

[A case report of interferon-associated retinopathy].

The authors describe signs and symptoms of interferon-associated retinopathy in patient with hepatitis C virus, nephropathy and diabetes mellitus. Necessity to assess visual system before and after therapy was emphasised.

[Application of automated perimetry in diagnosis of macular diseases].

PURPOSE: Evaluation of automated perimetry application in diagnosis of macular diseases. MATERIAL AND METHODS: Examinations were performed in 27 patients with central chorioretinopathy (CSCR), 29 patients with diabetic maculopathy, 12 patients with drusen, 13 patients with macular hole, 55 patients with age-related macular degeneration (AMD) and 23 healthy volunteers. Central visual fields were tested with automated threshold-measuring perimetry using the LED-PERS perimeter. Localisation, depth and spread of defect in central visual field were estimated. All threshold values, mean sensitivity, fixation, fluctuation and visual acuity were calculated. Statistical comparison of examined parameters and multidimensional analysis of variance (MANOVA) was performed using software. RESULTS: Significant differences were found when comparing all threshold values, mean sensitivity, fixation and fluctuation between the control and the other groups, and between patients with CSCR, with drusen and with AMD. Good results of MANOVA were obtained in 3 distinguishable classes between patients with CSCR, diabetic maculopathy and AMD. Positive correlation between threshold values of fields and visual acuity was noticed. CONCLUSIONS: Automated perimetry has been confirmed to be very useful in differential diagnosis of patients with macular diseases.

[Visual evoked potentials and static perimetry in functional visual deficits].

The author presents application of visual evoked potentials and static perimetry in hysterical amblyopia, psychical blindness, malingerers, amblyopia, cortical blindness and visual acuity assessment in infants. The author emphasizes the influence of multiple factors on the results and interpretation of the examination.

[Iridoschisis--case report].

A case of 54-year-old man with a rare degenerative condition of the iris stroma is presented. Described iridoschisis has developed in superior part of iris. Open-angle glaucoma and degeneration of vitreous was associated.

[Use of neural networks for analysis of visual evoked potentials developing a type of "pattern"--preliminary report].

PURPOSE: The authors present the "pattern" visual evoked potentials (VEP) analysis with use of the artificial neural networks (ANN). MATERIALS AND METHODS: The study involved 11 patients with compressive chiasmal optic neuropathy, 20 patients with optic neuritis, 12 patients with anterior ischaemic optic neuropathy, 20 patients with optic nerve atrophy from neuritis, 8 patients with demyelinative neuropathy, 5 patients with oedema optic nerve, 20 healthy persons. The tests of visual evoked potentials were performed with the use of computer system UTAS-E1000. Classification of potentials was made by correlation of outputs of ANN with results of confirmed neuro-ophthalmology conditions. RESULTS: ANN of different architecture were classified correctly in 80-100% of VEP record samples. CONCLUSIONS: The obtained correctness of classification confirms usefulness of VEP analysis as the objective diagnostic method in some neuro-ophthalmological diseases and indicates application of ANN in multifactor analysis.

[Evaluation of visual evoked potentials in partial optic nerve atrophy].

PURPOSE: Parameters of visual evoked potentials in partial optic nerve atrophy in different aetiology were studied. MATERIALS AND METHODS: The study involved 48 patients (68 eyes) with partial optic nerve atrophy from neuritis, toxic, arteriosclerosis and trauma. The tests of visual evoked potentials were performed by the use of UTAS E-1000 computer system in all patients with the use of pattern and flash stimulation. RESULTS: Amplitude of all waves were reduced in 74% patients using "pattern" and in 61% patients using "flash" stimulation. Latency of wave P2 was delayed in all patients with optic nerve atrophy from arteriosclerosis. Latency of P100 wave and N1 wave were not delayed in patients with optic nerve atrophy from toxic neuropathy. CONCLUSIONS: Estimation of latency of the VEP may be useful in diagnosis of the partial optic nerve atrophy. Variety of records of VEP indicate different degree of disturbances of nerve conduction.

[Use of pattern and flash visual evoked potentials in differential diagnosis of optic neuropathy].

PURPOSE: To assess the value of pattern and flash stimulus of visual evoked potentials (VEP) in patients with optic neuropathy. MATERIAL AND METHODS: The study involved 11 patients with compressive optic neuropathy, 19 with acute optic neuritis and 22 with demyelinating optic neuropathy. The control group included 20 persons without history of ocular and neuroophthalmologic diseases. In all patients VEP were performed using pattern and flash stimulus. Multidimensional analysis of variance (MANOVA) was used which allowed to evaluate the differentiation efficacy in respective parameters as well as the whole distinguishable classes. RESULTS: The efficacy of differential diagnosis of examined optic neuropathy with pattern or flash VEP was not sufficient. Better results were achieved when both types of stimulus were applied in the same patient.

[Neodymium-YAG laser iridotomy in subacute and acute angle closure glaucoma].

PURPOSE: To evaluate efficiency of Nd: YAG-laser iridotomy in the treatment of angle closure glaucoma. MATERIAL AND METHODS: 33 eyes of 22 patients were examined before and after the treatment. Intraocular pressure (IOP) was measured 24, 48 and 72 hours after iridotomy and then in (after) 1 and 6 months. Visual acuity, static perimetry and optic disc evaluation was performed before the treatment and after 1 and 6 months. RESULTS: In 90% of the eyes, 24 hours after the laser, IOP was below 22 mmHg and it was normalized during the whole follow-up. There no differences between the results of visual acuity, perimetry and optic disc evaluation before the treatment and during follow-up. CONCLUSIONS: Nd:YAG-laser iridotomy is an efficient method of lowering of IOP in angle-closure glaucoma.

[Evaluation of visual evoked potentials and static perimetry in chiasmal tumors and retrobulbar neuritis].

The author presented results of visual evoked potentials and static perimetry examinations performed in 17 patients with chiasmal tumors and in 27 with retrobulbar neuritis. A comparative analysis of the gathered parameters was done, using computer programs. The results suggest that the applied non-invasive methods of examination could be useful in differential diagnosis of these diseases.

[Cronassial in the treatment of neuropathies and atrophy of the optic nerve].

The study presents the evaluation of treatment by Cronassial of 26 patients including 16 with post-inflammatory optic atrophy, 4 with optic neuritis and 6 multiple sclerosis patients with optic neuropathy. One half of the patients showed an improvement of the visual acuity and visual field, and small improvement of the visual evoked potentials. No improvement of the conductivity of the impulses of the retino-cortical potentials has been observed. The best efficacy of the compound has been seen in the inflammatory conditions of the optic nerve.

[X-linked retinitis pigmentosa].

The study presents a case of a family whose members exhibit an X-linked retinal pigment dystrophy. The attention is called to the fact of an early onset of the disease, full development of the symptoms at an young age and a bed prognosis. The clinical characteristic of inheritance in X-linked retinal pigment dystrophy is discussed.

[Function of the visual system after orbital surgery. Late observations].

Forty six persons with an orbital tumor were operated in the period of 1980-1988. A control examination performed in 39 persons did not show in 38 any characteristics of recurrence of the tumor; the motility of the eye ball was normal. In 29 persons the visual acuity of the eye at the side of the operated tumor was over 5/6.

[Morphology of changes in records of visual evoked potentials in patients with multiple sclerosis].

Statistical analysis of the parameters of the waves of visual evoked potentials were performed in 2 groups: in 32 patients with a sure diagnosis and in 21 with a probable diagnosis of multiple sclerosis. In the 1st group prevailed the reduced record with a delayed latency of peaks of the A, B, C waves and with the interhemispherical and interocular asymmetry of the diagram. In the 2nd group dominated an delayed latency of the B and C waves and interhemispherical asymmetry of those waves. Abnormal characteristic of the record was obtained also after stimulation of the eyes of the examined patients with a full visual acuity.

[Orbital myositis].

Four patients with unspecific myositis of the extraocular muscles were treated in the Ophthalmological Department Medical School in Białystok in the period 1983-1988. The authors discuss the problems connected with the diagnosis and treatment of this pathological condition. Special attention is called to the new diagnostic methods.
