[3-layer anastomoses after resection of colorectal cancer. A prospective study of 1015 anastomoses].

This study included patients who underwent elective curative resection of adenocarcinoma of the large bowel with primary suture of the anastomosis. The overall incidence of clinical anastomotic leakage was 5%. The incidence of anastomotic leakage was significantly higher in men than in women after low anterior resection. The three-layer anastomosis has no advantage in large bowel surgery when compared with data from other sutured anastomoses reported in the literature.

Adenocarcinoma

Effect of combined prednisolone, epidural analgesia, and indomethacin on the systemic response after colonic surgery.

Twenty patients undergoing colonic resection were randomized to either conventional postoperative pain treatment with morphine chloride and acetaminophen (group 1, n = 9) or methylprednisolone sodium succinate 90 minutes before surgery plus intraoperative neural blockade, with a postoperative analgesic regimen with combined bupivacaine hydrochloride-morphine and indomethacin sodium for systemic effect (group 2, n = 11). Assessments of pain, pulmonary function, convalescence, and various injury factors were done several times until 8 days after surgery. Postoperative pain and hyperthermic response were eliminated in group 2. Conventional reduction in pulmonary function measures was improved in group 2, and fatigue and mobility were less pronounced. Prostaglandin E2, interleukin 6, and C-reactive protein levels increased in both groups, but significantly less in group 2. These results suggest that a combined neural and humoral blockade may more effectively inhibit the global stress response to elective surgery than previously observed with neural blockade with or without indomethacin.

Induction of a putative laminin-binding protein of Streptococcus gordonii in human infective endocarditis.

There is evidence to suggest that the virulence of Streptococcus strains in infective endocarditis might be due to the expression of binding sites for the extracellular matrix proteins of damaged valves. In this communication, we draw attention to one laminin-binding protein from a strain of Streptococcus gordonii isolated from a patient with human endocarditis. This 145-kDa protein was found on the cell wall of the bacterium. The level of expression of this binding protein might be regulated by the presence of extracellular matrix proteins: the protein was lacking after in vitro selection of laminin, collagen I, and fibronectin nonbinding variants, and it was recovered after growth of the variants when laminin or collagen I was added to the growth medium. It was also missing after 10 subcultures in minimal medium, indicating some positive control. Furthermore, the 145-kDa protein was recognized as a major antigen by sera from patients treated for streptococcal infective endocarditis, while sera from patients with valvulopathies gave only slight recognition, suggesting an increase of the expression of this protein during infective endocarditis. It was also shown that the 145-kDa protein carried a collagen I-like determinant detected with anti-human collagen I antibodies.

Complete nucleotide sequence of the sr gene from Streptococcus mutans OMZ 175.

The nucleotide sequence encoding the SR protein of Streptococcus mutans OMZ 175 (serotype f) has been determined. The sr gene consists of 4667 bp and codes for a 171177 Da protein. Comparison of the inferred amino acid sequence with the one of PAc antigen from S. mutans MT 8148 (serotype c) indicates a 88% conservation of amino acid residues which reflects the close relatedness of both proteins. Major differences in amino acid composition are located at the C-terminal part of the sequence where only 298 amino acids of the terminal 420 are conserved.

Non-prostatic malignant disease mimicking prostatic cancer.

Three patients with pelvic tumours (2 cases of pelvic sarcoma and 1 case of metastatic bronchial carcinoma) presented with symptoms and signs suggestive of prostatic carcinoma. Transrectal ultrasonic scanning with perineal biopsy facilitated the diagnosis. The prognosis of pelvic sarcoma was poor following radiotherapy.

Voiding patterns in men evaluated by a questionnaire survey.

A questionnaire on obstructive and irritative voiding symptoms was sent to 572 men aged between 20 and 79 years, selected at random from the National Register; 337 questionnaires were completed. None of the responders had consulted a doctor because of voiding symptoms. There was a significant increase in voiding symptoms and in the obstructive, irritative and total symptom scores between the sixth and seventh [corrected] decades, whereas the increases in the other decades were small. Only the prevalence of terminal dribbling was uniform. The prevalence of single obstructive and irritative voiding symptoms in men aged 60 to 79 years without subjective prostatism was the same as in patients admitted with prostatism, although most of the men had milder symptoms. Only nocturia and urge incontinence were more prevalent in patients admitted with prostatism. About 20% of men in the oldest decades had symptoms equal in severity to those found in men undergoing prostatectomy; 29% and 11% of men in the eighth decade [corrected] had nocturia twice and 3 times or more respectively; 19% complained of urge incontinence. More information on possible treatment is needed.

Voiding patterns and prevalence of incontinence in women. A questionnaire survey.

A detailed questionnaire on the occurrence of irritative and obstructive voiding symptoms, incontinence and the number of childbirths was sent out to 600 women aged between 20 and 79 years, randomly selected from the National Register; 432 (72%) returned the questionnaire and 414 (69%) were evaluable. The frequency of obstructive symptoms such as hesitancy, intermittency and decreased stream was low. Diurnal frequency with voiding intervals less than 3 h were recorded by more than 50% of the women and about 10% voided more frequently than every second hour. Nocturia increased significantly with age; 54 (13.6%) voided at least twice per night. About 40% complained of incontinence but this was troublesome in only 6%; 15.3, 13.3 and 11.5% had stress, urge and mixed incontinence respectively. More than 8% wore nappies or sanitary towels every day to protect against urinary leakage. Although the tendency to wear nappies or sanitary towels increased with age, the increase was not statistically significant. There was a positive correlation between the occurrence of stress incontinence and childbirth in the group as a whole.

Late results of pyeloplasty by the Anderson-Hynes method.

During the period 1979-85, 22 patients underwent operation for hydronephrosis. The surgical method chosen was the Anderson-Hynes pyeloplasty. Nine patients out of 15 with impaired function of the affected kidney showed an improvement in function after surgery. Seventy-seven per cent of the patients were totally free of symptoms, 18% improved following operation. The Anderson-Hynes pyeloplasty is recommended for patients with symptoms of hydronephrosis.

Purification and characterization of the expression product of the sr gene of Streptococcus mutans OMZ 175.

The subcloning in pBR322 of the gene of the S. mutans OMZ 175 74K SR protein, was performed after in vitro reconstitution, from two recombinant EMBL3 phages, lambda SmAD9 and lambda SmAD10. The gene is expressed in E. coli HB101 under the control of its own promoter and produces a protein with a molecular weight of 195 kDa. A strong immunological relationship between the expressed protein and the 74K SR protein was observed in ELISA, Western blotting and immunoprecipitation. The 195 kDa protein was purified by immunoaffinity chromatography to homogeneity as judged by SDS-PAGE and native PAGE. Its reactivity with monoclonal anti 74K SR antibodies indicates that it is probably a precursor form of the 74K SR protein produced in S. mutans. The adhesion properties of the two proteins, tested in solid phase adherence assays, are quite similar. This indicates that the additional peptide present in the precursor protein has little or no role in the adherence properties of protein 74K SR.

Analgesic effect and tolerance of Voltaren and Ketogan in acute renal or ureteric colic.

Fifty-six patients with renal or ureteric colic were entered into a randomised, prospective, double-blind investigation of the analgesic efficacy and tolerance of Voltaren versus Ketogan, both administered intramuscularly. There were no significant differences regarding pain-relief but side effects were fewer in patients treated with Voltaren.

Ofloxacin compared with ciprofloxacin in the treatment of complicated lower urinary tract infections.

In a double-blind, double-dummy study 61 patients with structural or functional abnormalities of the urinary tract and verified infection were treated for 7 days with ofloxacin 100 mg bd or ciprofloxacin 250 mg bd by mouth. Of 62 isolated strains none was resistant to ofloxacin or ciprofloxacin in vitro. Nineteen patients (63%) in both groups were free from infection ten days after therapy. Clinical resolution of symptoms occurred in 83% in the ofloxacin group and 68% in the ciprofloxacin group. Adverse reactions were reported in four patients (6.5%), two with skin rash, one with gastrointestinal disturbance and one with influenza-like symptoms. This study demonstrates that ofloxacin and ciprofloxacin are effective in the treatment of complicated lower urinary tract infection.

No cytogenetic effects of quinolone treatment in humans.

Cytogenetic effects of ciprofloxacin (500 to 2,000 mg daily) and ofloxacin (200 mg daily) were studied in lymphocytes from 31 patients treated for 1 to 10 weeks. Blood samples for cytogenetic analysis were taken before the start of treatment from all patients, after 1 week from 25 patients, and after 2, 4, 6, and 10 weeks from six patients. No chromosome-damaging effect could be demonstrated in any treatment group. The mean aberration yields for each cytogenetic parameter studied and the total number of aberrations were all normal at each sampling occasion.

Carcinosarcoma of the urinary bladder. Case report.

A case of carcinosarcoma in the urinary bladder is presented. On basis of a review of the most recent literature and referring to our own case, radical cystectomy appears to be the most effective treatment of this tumour.

Cloning of the saliva-interacting protein gene from Streptococcus mutans.

Genomic libraries from Streptococcus mutans OMZ175 were constructed in bacteriophage vectors. DNA fragments 1 to 2 kilobases in length were cloned in expression vector lambda gt11. S. mutans DNA fragments 15 to 20 kilobases in length were inserted in the BamHI site of phage EMBL3. Rabbit antiserum raised against an S. mutans saliva-interacting protein with a molecular weight of 74,000, designated 74K SR, was used to screen the lambda gt11 library. A recombinant phage carrying an S. mutans DNA sequence of 1.45 kilobases, lambda SmAD2, was detected and isolated. This fragment, named SmAD2, was used to construct the recombinant expression plasmid pSAD2-4 which encoded for the expression of a 60,000-molecular-weight protein controlled by the beta-galactosidase promoter from plasmid pUC8. The SmAD2 fragment and polyclonal anti-74K SR antibodies were used to screen the EMBL3 library. A total coincidence between the screening with antibodies and the DNA probe was observed, and two phages, lambda SmAD9 and lambda SmAD10, were isolated. They contained a common S. mutans DNA sequence of about 11.8 kilobases and coded for a protein with a molecular weight of about 195,000, which comigrated with a protein of an S. mutans cell wall extract. The expressed protein was purified, and a very strong relationship with the S. mutans 74K SR protein was found by competitive enzyme-linked immunosorbent assay. Thus, cloning of the 74K SR gene allowed us to demonstrate that the saliva receptor appears to be a part of an S. mutans precursor molecule with a molecular mass of 195,000 daltons.
