[Logistics in the pharmaceutical service].

The conception of the field of pharmaceutical service defines pharmaceutical service as the basic part of pharmacy, the principal task of which is to provide pharmaceutical care as an inseparable part of providing health care. It represents a set of professional activities of the pharmacist oriented to securing human and veterinary pharmaceutical products and health care products and to optimising effective, safe and quality pharmacotherapy. Technically, pharmaceutical service is an applied discipline, as it makes use of knowledge gained in other pharmaceutical, medical, psychological, social, and economic sciences. Because of its interdisciplinary character it is necessary to extend the theory of pharmaceutical service in such a way so that it may reflect all aspects of its sphere of activity. One of the possibilities is to define the pharmacy premises as an independent functional unit which operates on the basis of valid legal standards in such a way that on the one hand it secures the provision of health (pharmaceutical) care, and on the other hand it maintains its cost-effectiveness. To keep the quality of care of the patient and the economic aspect in balance, it is necessary to define the term pharmaceutical logistics also under the conditions of Slovak (Czech) pharmacy as early satisfaction of the requirements of the patient (client) in the pharmacy premises, which means that the appropriate pharmaceutical product or health care product and the appropriate information must be at the right time in the required amount and required quality in the right place.

[Patient-oriented advice on pharmaceuticals and its position on pregraduate education of pharmacists].

Patient-oriented information on pharmaceuticals has gradually become the dominant activity of the pharmacist in the community pharmacy. It is therefore necessary to devote sufficient attention to the quality of patient oriented advice already in the course of the pregraduate education of pharmacists so that new graduates who start practising may be prepared both theoretically and practically to cope with the requirements they encounter when working with pharmaceuticals and patients under the contemporary conditions. This paper aimed to monitor the present state of instruction of undergraduates as far as the items of information given to patients are concerned. The results of the study propose to the instructor possible ways of motivating and guiding the undergraduates to the correct approach of the use of information given to patients are concerned. Myocardial free fatty acid and glucose use after carvedilol treatment in patients with congestive heart failure.

BACKGROUND: Use of beta-adrenoreceptor blockade in the treatment of heart failure has been associated with a reduction in myocardial oxygen consumption and an improvement in myocardial energy efficiency. One potential mechanism for this beneficial effect is a shift in myocardial substrate use from increased free fatty acid (FFA) oxidation to increased glucose oxidation. METHODS AND RESULTS: We studied the effect of carvedilol therapy on myocardial FFA and glucose use in 9 patients with stable New York Heart Association functional class III ischemic cardiomyopathy (left ventricular ejection fraction </=35%) using myocardial positron emission tomography studies and resting echocardiograms before and 3 months after carvedilol treatment. Myocardial uptake of the novel long chain fatty acid metabolic tracer 14(R, S)-[(18)F]fluoro-6-thia-heptadecanoic acid ([(18)F]-FTHA) was used to determine myocardial FFA use, and [(18)F]fluoro-2-deoxy-glucose ([(18)F]-FDG) was used to determine myocardial glucose use. After carvedilol treatment, the mean myocardial uptake rate for [(18)F]-FTHA decreased (from 20.4+/-8.6 to 9.7+/-2.3 mL. 100 g(-1). min(-1); P<0.005), mean fatty acid use decreased (from 19.3+/-7.0 to 8.2+/-1.8 micromoL. 100 g(-1). min(-1); P<0.005), the mean myocardial uptake rate for [(18)F]-FDG was unchanged (from 1.4+/-0.4 to 2.4+/-0.8 mL. 100 g(-1). min(-1); P=0.14), and mean glucose use was unchanged (from 11.1+/-3.1 to 18.7+/-6.0 micromoL. 100 g(-1). min(-1); P=0.12). Serum FFA and glucose concentrations were unchanged, and mean left ventricular ejection fraction improved (from 26+/-2% to 37+/-4%; P<0.05). CONCLUSIONS: Carvedilol treatment in patients with heart failure results in a 57% decrease in myocardial FFA use without a significant change in glucose use. These metabolic changes could contribute to the observed improvements in energy efficiency seen in patients with heart failure.

An evaluation of myocardial fatty acid and glucose uptake using PET with [18F]fluoro-6-thia-heptadecanoic acid and [18F]FDG in Patients with Congestive Heart Failure.

UNLABELLED: Understanding the metabolic consequences of heart failure is important in evaluating potential mechanisms for disease progression and assessing targets for therapies designed to improve myocardial metabolism in patients with heart failure. PET is uniquely suited to noninvasively evaluate myocardial metabolism. In this study, we investigated the kinetics of 14(R,S)-[18F]fluoro-6-thia-heptadecanoic acid (FTHA) and [18F]FDG in patients with stable New York Heart Association functional class III congestive heart failure and a left ventricular ejection fraction of no more than 35%. METHODS: Twelve fasting patients underwent dynamic PET studies using [18F]FTHA and FDG. From the dynamic image data, the fractional uptake rates (Ki) were determined for [18F]FTHA and FDG. Subsequently, serum free fatty acid and glucose concentrations were used to calculate the myocardial free fatty acid and glucose uptake rates, respectively. Uptake rates were compared with reported values for [18F]FTHA and FDG in subjects with normal left ventricular function. RESULTS: The average Ki for [18F]FTHA was 19.7 +/- 9.3 mL/100 g/min (range, 7.2-36.0 ml/100 g/min). The average myocardial fatty acid use was 19.3 +/- 2.3 mmol/100 g/min. The average Ki for FDG was 1.5 +/- 0.37 mL/100 g/min (range, 0.1-3.3 mL/100 g/min), and the average myocardial glucose use was 12.3 +/- 2.3 mmol/100 g/min. CONCLUSION: Myocardial free fatty acid and glucose use in heart failure can be quantitatively assessed using PET with [18F]FTHA and FDG. Myocardial fatty acid uptake rates in heart failure are higher than expected for the normal heart, whereas myocardial glucose uptake rates are lower. This shift in myocardial substrate use may be an indication of impaired energy efficiency in the failing heart, providing a target for therapies directed at improving myocardial energy efficiency.

Salivary gland tumours treated in the stomatological clinics in Bratislava.

During the period of 1951-1996 a group of 1021 patients (484 men and 537 women, mean age 53 years, range 2-87 years) with salivary gland tumours were analysed retrospectively. The mean follow-up period was 8 years (range 10 months-25 years). The frequency of benign tumours was 74% (n = 755) and malignant tumours 26% (n = 266). Lesions were sited in the parotid gland 83% (n = 847), in the submandibular gland 10.8% (n = 110), in the sublingual gland 3.2% (n = 33) and in the minor salivary glands 3% (n = 31). The most common benign tumours were pleomorphic adenoma in 53.9% (n = 550) and Warthin's tumour 9.7% (n = 99). Of the malignancies, the adenoid cystic carcinoma was most common (6.4% of cases, n = 65) and mucoepidermoid carcinoma occurred in 5.2% (n = 53). The predominant therapy was surgery alone or in combination with postoperative radiotherapy in 93.7% (957 cases), radiotherapy alone after fine needle aspiration biopsy for 4.7%, and 19 patients remained untreated.

[Treatment of salivary gland tumors in the past and at the present time at the 1st and 2nd Stomatology Clinics of the Charles University Medical School in Bratislava].

The retrospective study analyzes a group of 1021 patients (484 men and 537 women) with salivary gland tumours treated at the 1st and 2nd Departments of Stomatology in Bratislava within the years 1951-1996. This time period is divided into the past (1951-1980) and recent (1981-1996) periods. The trends in the diagnosis and treatment with the main emphasis on surgical procedures are mentioned. The most common operation were lateral parotidectomy (409 cases = 40.1%) and conservative parotidectomy (105 cases = 10.2%), enucleations were indicated rarely. Authors stress the necessity of team approach and cooperation of maxillofacial surgeons, radiologists, histopathologists, otorhinolaryngologists and oncologists. (Tab. 1, Fig. 2, Ref. 6.)

[Functional liver disorder in relation to decreased titratable acidity of milk].

The internal environment of 17 dairy cows was observed by help of 23 variables of metabolic profile in blood and milk at lowered titratable acidity of milk (Tab. I) Average values of the observed variables were compared with reference values in a histogramme (Fig. 1). We calculated the statistical significance of differences of arithmetical means from reference average. Using correlation analysis we calculated the correlation coefficients for the observed variables. We represented the chosen correlation relations in correlogrammes (Fig. 2). We found significant correlation relations between AST and titratable acidity (degrees SH) (r = 0.791), AST and milk pH (r 0.617), AST and lactose (r = 0.69), AST and glucose (r = -0.56), blood pH and milk pH (r = -0.608), milk pH and lactose (r = -0.89). Mutual relations of titratable acidity with chosen variables of blood serum and milk were evaluated by help of regression coefficients, calculated for each parameter from the mathematical model (Fig. 3). The nearest relations were found for blood pH (regression coefficient /k/ kPH = -11.9), for AST (kAST = -3.1), for milk pH (k milk pH = -1.72) and for Mg (k Mg = -1.98). We also compared the recorded results of titratable acidity with theoretical values of titratable acidity calculated from the mathematical model (Tab. II). The differences between them were not statistically significant. It is evident from the results that the acid-base balance and functional liver condition influence the titratable acidity of milk.

[Carcinoma of the oral cavity--analysis of local metastases and results of therapy].

A series of 453 patients admitted to the 2nd Clinic of Stomatology in Bratislava over the ten-year period from 1977-1986 was statistically evaluated. The series comprised patients with the diagnosis of epidermoid carcinoma of the oral cavity, with the exception of carcinoma of the lip. The condition of the regional lymphatic apparatus at the first examination and over the succeeding period was studied. The relationship between the treatment used, clinical stage and prognosis of tumorous disease in the orofacial region was evaluated. The possibilities of different types of cervical dissection are discussed along with the histological findings recorded. In the light of the survival period a direct relationship between the clinical stage of the disease at the onset of treatment and prognosis has been confirmed.

[Interrelation between profile test indicators in the decreased milk acidity syndrome in cows].

The internal environment of high-yielding dairy cows was investigated in relation to 36 variables of the blood metabolic profile, blood serum, milk and urine, with the incidence of reduced titration acidity milk syndrome. The levels of variables were confronted with references values. At the same time the statistical significance of differences of arithmetical means from the reference mean was calculated. Correlation analysis of all variables yielded 630 correlation coefficients. Certain correlations were represented in form of correlogrammes. It has been demonstrated that the reduced titration acidity milk syndrome with the lowest values of 1.9 ml 0.25N NaOH per 100 ml occurred after administration of the feed ration deficient both in digestible crude protein and starch units (nutritive ratio 1:7.5) which contained unsafe grass haylage with a high content of ammonia (232.9 mg per 100 g). A highly significant negative correlation was proved to exist between lactose concentration and pH value of milk (r = -0.913); the regression equation (lactose) is 36.997--4.7536 (pH).

[Use of the computer in the study of relations of indicators in the metabolic profiles of dairy cows].

Fifty dairy cows with metabolic alkalosis and forty-eight dairy cows of a control group were subjected to a metabolic profile test with 30 parameters. The results of analyses were confronted with reference values in histograms. We calculated mutual correlation coefficients for all parameters of dairy cows of the two groups. The correlations between selected pairs of parameters were represented diagrammatically in a form of correlograms. We demonstrated that the values of certain parameters being approximately identical, the correlations in group A of dairy cows (with alkalosis) varied in a different way from the correlations of the control group. It is seen from the results that the confrontation of the values of a parameter with reference values need not be in all cases a reliable criterion of evaluating the internal environment in dairy cows. We processed the metabolic profiles of the two groups by a multidimensional analysis of principal components. The dairy cows of the two groups clearly differentiated from one another according to the internal environment regarding the used parameters. We draw attention to a possibility of computerizing the diagnostic significance of the metabolic profile parameters.
