Macular coloboma in siblings affected by different phenotypes of retinitis pigmentosa.

Purpose To report the clinical association between macular coloboma (early-onset macular dystrophies/atrophic changes) and different phenotypes of retinitis pigmentosa (RP). Methods Three young-adult siblings, two males and one female, were retrospectively studied. These patients underwent two complete ophthalmologic examinations (27-month follow-up), including orthoptic evaluation, colour vision test, visual field, corneal topography, electronystagmography, fluorescein angiography, and electroretinography. Eye check, automated visual field test, and complete electroretinographic study were also conducted on other asymptomatic members of the same family. Results All symptomatic siblings were affected by manifest congenital nystagmus, poor visual acuity, and progressive visual field impairment in both eyes, bilaterally presenting macular coloboma associated with three different RP patterns: classic RP; mild dystrophy of the retinal pigment epithelium, associated with subnormal electroretinographic findings (subclinical form of RP); and sector RP. The ophthalmologic reports regarding their deceased father documented that he had suffered from the same alterations of ocular movements and visual performances diagnosing, in both eyes, extensive atrophic changes of the macular area completely surrounded by pigmented bone spicules (RP-type tapeto-retinal dystrophy). The other investigated relatives did not show any specific and/or significant ocular disorder. Conclusions In these three adult members of the same family, the concomitance between macular coloboma and different intrafamilial RP phenotypes is described. Recurrent eyelid basal cell carcinoma with sclerochoroidal infiltration: echographic findings.

In an 82-year-old woman with a recurrent right lid basal cell carcinoma infiltrating the upper temporal orbit, a solid yellowish lesion was found ophthalmoscopically in the superotemporal periphery of the right eye. On standardized echography examination, the 2.5-mm elevated subretinal lesion was medium to high reflective with irregular structure, whereas the adjacent orbital mass was medium to low reflective and the sclera was thinned with at least one passage between the orbital and ocular lesions. To our knowledge, this is the first echographic report of sclerochoroidal infiltration from a lid basal cell carcinoma.

Phacotrabeculectomy with or without punch: preliminary results comparing the two techniques.

The authors report their experience in the combined surgery, for glaucoma and cataract, by using the Crozafon-De Laage punch. 24 patients affected by open-angle glaucoma and cataract have been enrolled in the study. 12 underwent a combined operation of phacoemulsification and trabeculectomy according to Cairns, and 12 underwent phacoemulsification and protective filtering surgery executed with the Crozafon-De Laage punch. Comparing the two methods we noted as advantage of the punch its easiness of use, through the sclerocorneal tunnel of the phacoemulsification, to create a filtering fistula; this shortened the duration of surgery. As a disadvantage of punch use we noted a lower filtering gradability as opposed to Cairns' trabeculectomy.

Effects of IAL and Healon on postoperative intraocular pressure after cataract surgery with intraocular lens implantation.

A randomized controlled study was done on 60 patients, divided into three groups, who underwent extracapsular cataract extraction and intraocular lens implantation. The aim of the study was to investigate the effect on postoperative intraocular pressure (IOP) of IAL left in situ compared with IAL or Healon removed after surgery. The IOP was monitored three, six, 12, and 24 hours and seven days after surgery. In general, there was a similar trend in IOP in the three groups with increasing values starting three hours after surgery and normal values reached around 24 hours. No significant statistical difference was found when IAL was removed compared with Healon removed or IAL left in situ compared with Healon removed. No differences were found in the presence of an inflammatory reaction or its severity among the three groups 24 hours after surgery. We believe that IAL can be used in cataract surgery without removing it at the end of the operation, thus simplifying surgical maneuvers and reducing surgical risks.

Electrophysiology of the optic nerve: recent insights.

The introduction of automated recording and processing systems, the development of electro-optical technologies, and the numerous findings deriving from experimental electrophysiology have intensified the interest in electrofunctional non-invasive tests for optic pathways studies. Pattern ERG was demonstrated as the most efficacious stimulus to investigate the ganglion cell activity. The contrast sensitivity measurement by means of electrofunctional methods, contribute to obtain more information about the retinal and optic pathways functionality. The utility of electrofunctional and psychophysic methods in some pathologies of optic pathways (diabetes opticopathy, endocular hypertension, retrobulbar neuritis) is discussed.

After an examination of the anatomical modifications of the retina and optic nerve during the early phases of development, the authors consider variations in some electrofunctional responses: PEV--PERG--ERG and retinocortical time.

Electrofunctional investigations of the optic nerve in diabetes.

After a brief introduction of the principal reports of ocular electrophysiology in diabetic diseases, are described the results of the registration of the visually evoked potential (VEP) and oscillatory potential (OP) in two groups of young patients affected by Type I diabetes. In both groups the VEP latencies increased with respect to the controls and such increase seems evident whether in the group with altered PO or in that of younger patients with PO still normal.

Correlation of ERG and pigment epithelium changes in external progressive ophthalmoplegia (EPO).

Six out of 17 patients with progressive external ophthalmoplegia (EPO) were found to have pigment anomalies with alterations in the electroretinographical (ERG) tracings. However, fluorangiography demonstrated alterations of the retinal pigment epithelium in patients with normal fundus and ERG examinations. We conclude that in our series there was no correlation between retinopathy and tapetoretinal degeneration.

Experiences with a new EOG photostimulator.

A new EOG photostimulator used to provoke ocular pendular and saccadic movements and to evoke optokinetic nystagmus is described. Differences between ocular movements obtained according to various techniques in relation to Arden's index are also evaluated.

Electroretinographic evaluation of a treatment based on retina phosphatides in patients with various types of retinochoroid degeneration.

The authors treated 16 patients with various types of retinochoroid degeneration with a preparation containing phospholipids extracted from pig retina. The evaluation of the efficiency of the preparation was carried out using adaptoelectroretinography. Exogenous phospholipids caused a statistically significant increase at the level of the scotopic components of the ERG.

Effect of pindolol on intraocular pressure.

Pindolol, a strong beta-adrenergic blocking agent, instilled into the conjunctival sac of normal and glaucomatous eyes, produced a significant drop in intraocular pressure. This was not, at first, accompanied by any variation in outflow facility; only after prolonged treatment did an increase in facility appear, which accounted only for one-third of the tension-lowering effect. The drug was well tolerated, and did not affect either pupil motility or corneal sensitivity. It seems suitable for a trial use in the treatment of glaucoma.
