[Shute's suture in mediolateral episiotomy. A technique with many advantages (author's transl)].

A comparison of median with mediolateral episiotomy shows that mediolateral episiotomy offers notable advantages resulting naturally from its anatomical course and the possibility of eventual distension. -- The Shute seam is described as a suturing technique in episiotomy still largely unknown in Europe. Apart from easy and rapid suturing and much cleaner wound healing it offers the advantage of significant reduction of pain during childbed with very satisfactory functional and cosmetic results. -- Reduction of wound pain seems to be a decisive factor for the woman in childbed, especially in view of subsequent births, thus making it easier for the obstetrician to obtain the patient's consent for performing episiotomy.

[Transcervical extraamniotic Rivanol instillation as a method of avoiding complications in pregnancy interruption].

The number of abortions performed is steadily increasing. According to information available to date from the Federal Statistics Office in the Federal Republic of Germany, pregnancies are still largely terminated by means of cervical dilatation with Hegar dilatators and subsequent curettage. Almost half of all abortions are performed in young women who have not yet given birth. In a great number of cases these patients have a gracile, rigid cervical os which has to be opened abruptly in a few minutes with Hegar's dilatators, often under asserting quite some force. This can result in lacerations of the cervix, cervical insufficiency, inflammation and cicatrization. The transcervical extraamnial Rivanol instillation seems to be a method for avoiding these early and the serious late complications ensuing. Using this method--an alternative to the Prostaglandine application, with its even more serious secondary effects and contraindications--the highest possible degree of success can be achieved in avoiding cervical injuries and in reducing inflammation in the pelvis minor to a minimum, which could later on lead to sterility.

[The advantages of the extra-amniotic transcervical instillation of rivanol in therapeutic abortions (author's transl)].

Our experience with the transcervical extra-amniotic instillation of 70 ml 0.1% Rivanol solution in 154 cases of first trimestre, second trimestre abortions and intra-uterine fetal deaths is reported. This simple and safe method has several advantages which render it suitable for out-patient usage should the rate of therapeutic abortions increase even more. The gentle dilatatory effect on the cervix is remarkable and is noticeable after 24 hours. Dilatation with the Hegar-dilators becomes unnecessary. There were no major complications. The stress on the patient is minimized.

[A study about 3rd degree perinatal rupture (author's transl)].

29 cases of 3rd degree perineal rupture occured from 1962 to 1975 at the Städtische Frauenklinik and Hebammenlehranstalt Braunschweig and have been analysed retrospectively. Most of these were observed in the last five years. The increase is undoubtlessly related to the shorter expulsion peried by ocytocin infusion. Investigations for avoidable courses and how to reduce them to a minimum were made in this analysis. Not at least this study was motivated by the lack of informations about obstetrical complications and the eventual consequences in literature. The only prophylaxis is the adequate support of the perineum and the sufficient episiotomy at the right moment. According to the operative and post operative treatment mentioned there could be achieved a good functional result in all cases. The stationary treatment has been reduced by these measures from 19,0 to 10,5 days.
