[Acute fatty liver of pregnancy--differential diagnosis and supportive therapy].

A case of clinically diagnosed acute fatty liver of pregnancy (AFLP) is reported. Hypoglycemia and coagulopathy were predominant in the clinical course. Serial studies on blood chemistry, especially blood glucose and coagulation parameters, in association with virus serology and ultrasound, can rule out common causes of jaundice in pregnancy. This contributes to reliability of the clinical diagnosis of AFLP in the absence of liver biopsy, and ensures adequate management of the patient.

Immuno- and flow cytometric analytical methods for biotechnological research and process monitoring.

In this article, the applications of immunoanalysis and flow cytometry for research and process monitoring in biotechnology are discussed. Brief reviews of the two analytical methods are followed by descriptions of actual applications in various areas of biotechnology. In the case of immunoanalysis, emphasis is placed on systems for on-line bioprocess monitoring, and examples are given for a thermostable pullulanase, a mouse IgG, and antithrombin III. Although flow cytometry is not currently an on-line analytical technique, its value as an off-line method is illustrated by examples of the measurement of shear stress effects, lipid content, and sterol content.

Nasal intubation in the presence of frontobasal fractures: a retrospective study.

In most cases, surgical management of craniofacial fractures involves correction of occlusion. This requires nasal intubation. In the case of frontobasal fractures with simultaneous cerebrospinal fluid (CSF) fistula, nasal intubation is thought to increase the risk of meningitis. An analysis of the records of 160 patients with frontobasal fractures and CSF fistulae showed that the route of intubation had no influence on the postoperative complication rate. Nasal intubation is therefore not contraindicated in frontobasal fractures with CSF fistulae.

Anti-Bacterial Agents

Intraoperative histological evaluation of tumor resection borders without prolonging surgery.

Frozen sections permit an intraoperative check for margin clearance during tumor surgery. However, much time is usually lost while waiting for the evaluation of the specimens, especially if they are many. In 24 patients with oral cancer, biopsies were taken from the regions of the intended resection borders before beginning with the lymphadenectomy. While histological evaluation was being performed by the pathologist, the surgical team proceeded to remove the regional lymphatic drainage. This procedure offers the advantage of being able to evaluate a large number of frozen sections without substantially prolonging the operation.

AO reconstruction plate systems for the repair of mandibular defects 3-DBRP versus Thorp-system.

The AO-maxillofacial division offers two different reconstruction plate systems for the repair of mandibular defects--the conventional 3-dimensionally-bendable reconstruction plate (3-DBRP) and the titanium hollow screw and reconstruction plate (THORP). Both systems were investigated and compared under identical conditions in an experimental study. The results of this investigation show a higher incidence of instability using the THORP-system. We believe that for the THORP, the anchorage applying only two screws in each bone stump as recommended by Raveh et al. (1984), is not sufficient to bear full functional loading immediately.

[Fatal course of peracute fatty liver of pregnancy].

A fatal case of acute fatty liver of pregnancy (AFLP) is reported. After admission, the patient was delivered within 3 hours. Routine laboratory investigation revealed acute liver insufficiency with advanced coagulopathy. Despite substitution therapy, the severe coagulation defect progressed to lethal intracerebral bleeding. Advanced AFLP can only be satisfactorily diagnosed in time, if non-specific symptoms or icterus lead to studies of blood chemistry, especially liver function tests, coagulation parameters (including platelet count, fibrinogen, AT III), blood glucose and renal function (including uric acid). This will enable an adequate management of the patient. The clinical problem of AFLP still remains that of early diagnosis.

Acute Disease

[Caries prevention in radiotherapy of the head-neck area].

Following radiotherapy of malignant head and neck tumours, tooth damage is observed in many cases, which is frequently referred to as "radiation caries". This is not the immediate result of radiation, but has a number of causes. The damage to the salivary glands caused by radiation leads to xerostomia, which affects the self-cleaning capacity in the mouth. As a result of the very painful mucositis which occurs during the course of radiotherapy as well as the restricted mastication caused by the operation, patients prefer a soft diet, which in turn promotes plaque formation. The acidogenic coating causes erosion of the enamel. If preventive measures are not taken, all of the teeth can be completely destroyed within a short time. All efforts should be made to prevent this from happening, particularly in young patients who have undergone curative radiotherapy. This should include intensive fluoride treatment and meticulous oral hygiene.

Cerebrospinal Fluid Rhinorrhea

Pre-tapped and self-tapping screws in children's mandibles. A scanning electron microscopic examination of the implant beds.

One hundred 2 mm AO miniscrews were inserted into the mandibles of eight fresh cadavers aged 8 to 12 years. Scanning electron microscopic examination of the implant beds showed cracks and accumulated bone material, as well as signs of crushing and shearing stress, regardless of whether the screws were pre-tapped or not. When, during screw insertion, the axis of the screw deviated by at least 10 degrees from the axis of the tap, two intersecting threads resulted. It is concluded that during osteosynthesis in child mandibles pre-tapping is not recommended.

Bone Screws

Increase of temperature during osteotomy. In vitro and in vivo investigations.

Direct temperature measurements using copper-constantan thermocouples were performed during in vitro and in vivo bone cuttings while the load of the oscillating saw and irrigation were varied. Very high temperatures during continuously applied load were observed. Consequent irrigation may reduce temperature elevation, but only intermittent sawing combined with irrigation diminishes temperature elevation adequately, which allows for an osteotomy without discernible bone damage. In practice, the available oscillating saws should be handled with great care, which implies that the osteotomy should be performed with maximum irrigation and intermittent use.

Short-term effect of whole milk and milk fermented by Pseudomonas fluorescens on plasma lipids in adult boars.

The short-term effects of whole milk and milk fermented by Pseudomonas fluorescens, of the amino acid composition of the diet and of feeding frequency on the level of plasma lipids, were investigated in six 1-year-old adult boars. The experimental diets contained equal amounts of protein, carbohydrates, fat and cholesterol. After an adaptation period of 5 d for each experimental treatment, blood was collected at regular intervals during 48 h and plasma levels of cholesterol, triacylglycerol, high-density-lipoprotein (HDL)-cholesterol and low-density-lipoprotein (LDL)-cholesterol were examined. All variables except HDL-cholesterol showed distinct diurnal fluctuations, which were substantially influenced by feeding frequency. Variations in the amino acid composition of the experimental diets, which were within a physiological range, had no effect on the level of plasma lipids. Plasma lipid levels were significantly lower when the animals received the diets containing milk instead of the diet without milk: cholesterol, triacylglycerol, and LDL-cholesterol were reduced by 5.6, 5.8 and 10% respectively (pondered means) while HDL-cholesterol remained unaffected. Fermentation of whole milk by P. fluorescens reduced the lipid-lowering effect. Our findings suggest that the intake of diets containing milk results in a lower plasma cholesterol and LDL-cholesterol level than the intake of diets with a similar nutrient content which do not contain milk.

Amino Acids

Two methods of assessment of methotrexate hepatotoxicity in patients with rheumatoid arthritis.

Serial liver biopsy specimens from 18 patients with rheumatoid arthritis receiving a weekly dose of methotrexate 7.5-20 mg for a minimum of 12 months were assessed semiquantitatively and by a microcomputer image analysis system. The semiquantitative histological method showed a significant increase in pericellular collagen and in overall disease while morphometry showed a significant increase in pericellular, perivenular, and portal tract collagen. There was a significant correlation between the two methods, but morphometry had the advantage of objectivity and efficiency. There was no correlation between the increase in collagen and the accumulated dose of methotrexate, which suggests that other factors in addition to methotrexate may contribute to liver injury.

[Chievitz's organ (the juxtaoral organ): morphological and immunohistochemical studies].

The Chievitz's organ (juxtaoral organ) consisting of a long continuous mass of epithelial cells is located with its medial border by the lateral face of the buccinator muscle. It was successfully identified in 10 out of 40 corpses and histologically examined. Through immunohistological methods using markers like S-100, TPA, PAN, CK-13, NSE and vimentin the epithelial character in the centre of the parenchyma could be demonstrated as well as the marked innervation and vascular supply of the direct surroundings.

[Is it possible to differentiate between odontoameloblastoma and fibro-odontoma? Critical position on basis of 18 cases in DOSAK list].

An attempt was made to differentiate between odontoameloblastoma and ameloblastic fibro-odontoma by means of histological criteria based on 29 cases referred to the DOSAK-bone tumor register. After critical revision 18 appropriate cases have been selected for evaluation. We divided them into 4 odonto-ameloblastomas and 14 ameloblastic fibro-odontomas in accordance with the WHO classification. With respect to localisation, clinical symptoms, radiography, frequency of recurrencies and age no major difference was found between the two "entities". In our sample a decisive histological differentiation was not possible. This supports our hypothesis that there is only one entity.

Ameloblastoma

The use of computer-assisted video image analysis in the enumeration of immuno-stained cells in tissue sections.

A novel method of computer-assisted video image analysis (VIA) was used to determine the number of immunostained cells in tissue sections. This method permitted an accurate and objective quantification of cells of a particular phenotype. This enumeration was achieved by measuring the area stained by a test monoclonal antibody (such as the T cell marker, CD3) and comparing it with the area stained by a leukocyte common (LCA) monoclonal antibody (CD45). The proportion of T cells within the total leukocyte population in a particular tissue was then calculated. The differentiation of positive (stained) and negative (unstained) cells was uniformly maintained by setting the computer to detect a threshold for staining intensity. This enabled consistency to be maintained within a tissue section as well as between sections stained with the same antibody. In the present study, we determined the phenotype of leukocytes in colonic carcinomas by VIA and compared this with results obtained by normal visual analysis. The VIA method showed distinct advantages over normal visual analysis especially in sections which contained moderate numbers of stained cells.

Antibodies, Monoclonal

Radiation and osteosynthesis. Dosimetry on an irradiation phantom.

An irradiation phantom was used to measure dose increases using the backscatter of different materials (titanium, steel, lead, aluminium). Telecobalt-60 and 8-MV photons were used for the irradiation. The irradiation dose was measured by means of a defined X-ray film blackening. The most important parameter was to find out whether under simulated conditions, undesirable hot spots occur. We were able to demonstrate that a 12.5 to 16% increase in the radiation dose can be observed for titanium and steel at a distance of 0.45 mm from the metal specimen. A comparison between titanium and steel did not demonstrate a relevant advantage for titanium. Therefore, adjuvant percutaneous radiation therapy should not have any influence on the life of the implant, if the soft tissue layer is of sufficient thickness. The available literature is reviewed and presented in tables.

Aluminum

Gas supply across membranes in bioreactors for hepatocyte culture.

The conditions required for hepatocyte cultures is a main topic in the development of bioreactors for hybrid liver support systems. The detoxification of ammonia and the synthesis of urea due to primary isolated hepatocytes was measured in order to compare two different models of gas supply in bioreactors: (a) indirect medium oxygenation and (b) direct membrane-contact oxygenation of the hepatocytes using polypropylene membranes. Increasing oxygen pressure promoted cell function. At day 6 of culture, urea synthesis was 0.8 +/- 0.3 mM in 21% of O2 cultures and 1.5 +/- 0.1 mM in oxygenated cultures. Alkalosis due to CO2 loss decreased ammonia metabolism. The direct membrane-contact oxygenation resulted in enhanced cell metabolism in comparison to medium oxygenation: urea synthesis at day six was 1.42 +/- 0.2 mM in 21% O2 cultures. Polypropylene oxygenation membranes proved to be sufficient for hepatocyte adhesion. Two functions can be integrated in one element in liver support systems using the investigated polypropylene membrane and the direct membrane-contact oxygenation: oxygenation with physiological oxygen pressure in bioreactors due to gas supply across the membrane and adhesion of hepatocytes in bioreactors on the membrane.
