Complications in patients with uveal melanoma after stereotactic radiosurgery and brachytherapy.

Malignant uveal melanoma is the most common intraocular tumour with the incidence about 0.7-1.0 in 100.000 inhabitants. Enucleation is still a standard treatment method for posterior uveal melanoma, but conservative methods are an adequate alternative. Brachytherapy has been used more than 20 years in our Department, stereotactic radiosurgery was introduced in 1999. In years 1991-2004, 145 patients with uveal melanoma were treated at the Department of Ophthalmology, Comenius University, Bratislava. Conservative treatment/brachytherapy is indicated in tumours staged T1-T3. Survival is comparable for all conservative methods. After stereotactic radiosurgery (Linac), the necessity of enucleation because of complications (e.g. secondary glaucoma), after brachytherapy (Ru106 plaques), it was 5%. Conservative methods and combined techniques are effective methods for the therapy of uveal melanomas (Fig. 5, Tab. 1, Ref. 6).

Shell structure of the near-dripline nucleus 23O.

Breakup reactions were used to study the ground-state configuration of the neutron-rich isotope 23O. The 22O fragments produced in one-nucleon removal from 23O at 938 MeV/nucleon in a carbon target were detected in coincidence with deexciting gamma rays. The widths of the longitudinal momentum distributions of the 22O fragments and the one-neutron removal cross sections were interpreted in the framework of a simple theoretical model which favors the assignment of Ipi = 1/2+ to the 23O ground state.

[Pars plana vitrectomy for diabetic pseudophakic macular edema].

PURPOSE: In retrospective study to evaluate the anatomical and functional results of pars plana vitrectomy (PPV) with peeling of the inner limiting membrane (ILM) on pseudophakic eyes with diabetic macular edema. MATERIAL AND METHOD: PPV with peeling of the ILM was performed in 5 eyes (5 diabetic patients) after cataract surgery with posterior chamber intraocular lens implantation. Group of patients enrolled 4 female and one male aged from 44 to 62 years (average 56.2 y.). 2 eyes had undergone focal fotocoagulation previously for clinically significant macular edema. The macular edema was diagnosed by clinical examination and by retinal fluorescein angiography from 3 to 8 months (mean 6 m.) after cataract surgery. Visual acuity (VA) and angiographic changes were monitoring. RESULTS: In pseudophakic eyes with diabetic macular edema that underwent surgery, the angiography documented decrease of hyperfluorescens in all cases. VA improved in 3 eyes (80%) more than +2 lines of Snellen test, stabilized in one eye (20%). VA detoriated in one eye by -1 line of Snellen test. CONCLUSION: Our results demonstrated that PPV with ILM peeling is effective surgical procedure on pseudophakic diabetic macular edema.

[The therapy for acute acquired syphilitic uveitis].

PURPOSE: In retrospective study (analysis) to report the effectiveness of therapy on anatomic and functional outcomes in patients with acute acquired syphilitic uveitis. DESIGN: Retrospective case-control study. PARTICIPANTS: 3 patients (4 eyes) with symptoms of ocular acute inflammation which occurred in latent stage of syphilis are included in our study. In all patients uveitis was the first clinical symptom of syphilis. METHODS: In all 3 patients the infection of acquired syphilis was diagnosed using specific and non-specific tests for treponemal antibody. In 2 eyes we performed diagnostic and therapeutic pars plana vitrectomy. The management of ocular syphilis in all patients consisted of mega units of aqueous penicillin intravenously or intramusculary, in 2 patients we used the combination with steroids. RESULTS: In 2 eyes with perioperatively obtained material of vitreous (during pars plana vitrectomy) specific treponemal antibody was detected using specific (TPHA) test. In the third patient infection was verified serologically. The final visual acuity of all 4 eyes is better than or equal to 6/12. The visual acuity of one exe is 6/6. CONCLUSION: Uveitis and neuroretinitis may be the first symptom of acquired syphilis in latent stage. Early antibiotic treatment significantly improves anatomical reparation of infection and visual outcomes.

[Surgical treatment of traumatic macular holes].

PURPOSE: To review the anatomic and visual outcomes of patients with traumatic macular hole, who underwent vitreoretinal surgery with membrane limitans interna (MLI) removal and using autologous serum. METHODS: The retrospective review consists of 6 eyes of 6 patients with traumatic macular hole, who underwent pars plana vitrectomy with the removal MLI and with using autologous serum. This study includes 4 male and 2 female. Ages ranged from 10 to 34 years and all patients underwent surgery between February 1997 to October 2000. The time between the injury and surgery ranged from 3 days to 3 months. The aetiollogy was 5-times blunt trauma with ball during field games and once it was shot injury. The initial visual acuity and campimetry were recived and anatomical, functional results and campimetry every 3 months after surgery were reviewed. RESULTS: Anatomical closure of the macular hole was achieved in 4 (66.6%) of the eyes (in 1 eye the closure was with pigmented scar). Two patients who have a persistent macular hole, underwent this surgery 2 or 3 months after injury. In all 4 eyes with anatomic closure of the macular hole was reduction of central scotoma. Postoperative Snellen visual acuity improved 3 or mole lines in the 2 eyes. CONCLUSION: Our results suggest that the period between injury and vitreoretinal surgery is very important for final anatomical and functional results after traumatic macular hole.

Retrobulbar neuritis associated with Borrelia afzelii infection.

PURPOSE: To report retrobulbar neuritis caused by Borrelia afzelii culturally proved from cerebrospinal fluid (CSF). METHODS: A 23 year old female underwent ophthalmologic, laboratory and other auxilliary examinations. RESULTS: CSF cultures grew spirochetal microorganisms, serotyped by monoclonal antibodies as Borrelia afzelii. Following the serological and cultural results, treatment with doxycycline 200 mg daily was started and kept for three weeks. Gradual improvement of the visual acuity of the right eye was observed with full recovery to 20/20. CONCLUSIONS: Borrelia infection should be considered in the differential diagnosis of retrobulbar neuritis. CSF should be examined also culturally. (Ref. 5.)

[Antimetabolites in glaucoma surgery--long-term results].

PURPOSE: To observe the long term success of antimetabolites used in glaucoma filtering surgery in eyes with high risc of failure. MATERIAL AND METHOD: Two groups of patients undergone trabeculectomy with the use of antimetabolites 5-Fluorouracil (5-FU) and Mitomycin C (MMC) are included in this retrospective study. 5-FU was administered postoperatively 1-10 times in the dose of 5 mg/ml in subconjunctival injection on 32 eyes. Follow up time ranged from 8 months to 9 years. Mitomycin C was administered intraoperatively in the dose of 0.2 mg/ml, on subconjunctival sponge with application time from 3 to 5 minutes on 15 eyes. Follow up time ranged from 7 month to 4.5 years. RESULTS: Intraocular pressure (IOP) before operation with the use of 5-FU was in average 28.8 mmHg (22-50 mmHg) at the end of therapy with 5-FU was 17.6 mmHg (10-30 mmHg) and in the last examination was on average 19.5 mmHg (14-28 mmHg). IOP before operation with the use of MMC was in average 30.3 mmHg (22-40 mmHg), the seventh day after administration of MMC 12.7 mmHg (6-20 mmHg) and in the last examination was in average 18 mmHg (9-28 mmHg). During the long term observation of IOP values in the 5-FU group there was 79.2% success and in the MMC group there was 83.3% success (IOP up to 20 mmHg with or without therapy). Decrease of IOP was statistically significant. CONCLUSION: Antimetabolites improve the success of filtering surgery in high risc glaucomas. The long term follow up shows that failure of filtration in these types of glaucomas is still a serious clinical problem.

Event-by-event fluctuations of the Kaon-to-Pion ratio in central Pb+Pb collisions at 158 GeV per nucleon.

We present the first measurement of fluctuations from event to event in the production of strange particles in collisions of heavy nuclei. The ratio of charged kaons to charged pions is determined for individual central Pb+Pb collisions. After accounting for the fluctuations due to detector resolution and finite number statistics we derive an upper limit on genuine nonstatistical fluctuations, which could be related to a first- or second-order QCD phase transition. Such fluctuations are shown to be very small.

[Long-term monitoring of visual function after anatomically successful surgical treatment of idiopathic macular holes].

PURPOSE: The purpose of the study is to determine the outcomes of long-term changes and evolution of visual acuity in patients undergoing anatomically successful macular hole surgery. MATERIALS AND METHODS: Into prospective study are enrolled 20 eyes (20 patients). All patients were successfully operated for idiopathic macular hole between September, 1996 and August, 1998. Time of follow up ranged from 20 to 43 months (mean, 29.5). The best corrected visual acuity was examined before operation and every 3 months after surgery. 12 eyes were operated for complicated cataract. The subgroup of 16 eyes was selected from the group of 20 eyes. The subgroup consisted of patients, who in the first examination observed in Watzke-Allen test the interruption of ray of light and there was a central scotoma on the campimetry. These tests were realised before operation and in the 3rd, 9th and 15th months after operation. Fisher's test and Student's t test were used for statistical analysis. RESULTS: Mean corrected visual acuity before operation was 0.21. Best corrected visual acuity was 0.44, reached in the 12th months. Improvement in +2 or more lines was reached in 55%. In 15th and 18th months the mean corrected visual acuity decrease because of the progression of nuclear sclerosis. 12 eyes were operated for complicated cataract. The best corrected visual acuity before and after surgery of idiopathic macular hole was 0.18 respectively 0.39 and improvement in +2 or more lines was reached in 6 (50%) eyes. After surgery of complicated cataract the mean visual acuity was 0.49. Further improvement in +2 or more lines was reached in 2 (16.7%) eyes, together in 8 (66,%) eyes. Statistical analysis (Fischer's tests, Student's t test) was significant (P < 0.05). In the subgroup of 16 eyes, where were followed changes in Watzke-Allen test, the interruption of ray of light was in all 16 (100%) patients. In the 3rd month 6 (37.5%) patients observed interruption of ray of light and other 10 (62.5%) observed deformation in the central part of ray of light. In 9th and 15th months the interruption observed 5 (31.25%) patients. Deformation in 9th and 15th months observed 8 (50%) respectively 5 (31.25%) patients. Gradual decrease of central scotoma observed in the 3rd, 9th and 15th month 10 (62.5%), 13 (81.25%) and 14 (87.5%) patients. During the follow up in 12 (75%) patients was reached total disappearance of central scotoma. CONCLUSIONS: The evolution of visual acuity after anatomically successful surgery of idiopathic macular hole is dynamic, long-standing and continual process. Improvement of visual acuity can be masked by progression of complicated cataract-nuclear sclerosis. Further improvement of visual acuity can be observed after of the surgery complicated cataract.

[Comparison of ultrasonographic examinations with surgical findings after surgery of idiopathic macular hole].

PURPOSE: In retrospective study to compare echographic features of posterior vitreous face and vitreo-macular region with preoperative findings by pars plana vitrectomy of idiopathic macular hole. MATERIAL AND METHOD: The group includes 27 eyes with idiopathic macular hole. 11 in II. and 16 in III. stage by Gass's classification. The spectrum of identifiable echographic features included: 1. partial posterior vitreous face separation; 2. presence of a pseudooperculum; 3. high spike of posterior vitreous face over the macula and/or optic nerve head and 4. combination of these findings, eventually some other pathology of vitreous. Peroperative clinical findings included: 1. presence of a pseudooperculum; 2. facility of elevation and separation of posterior vitreous face. RESULTS: The echographic findings showed: 1. 5-times (18.5%) partial posterior vitreous face separation; 2. 5-times (18.5%) the presence of a pseudooperculum; 3. 4-times (14.8%) the high spike of posterior vitreous face in front of the retinal surface spike; 4. 10-times (37.0%) the combination of 1. -3. and/or some other pathology of vitreous. 3 eyes (11.2%) were without relevant pathologic echographic findings. Preoperatively was 1. the presence of a pseudooperculum confirmed in 9 eyes (33.3%). 6-times (22.2%) the posterior vitreous face had separated by core vitrectomy, no active elevation was needed. In 21 eyes (77.8%) the active elevation of posterior vitreous face was performed, 6-times (28.4%) was the elevation more difficult. CONCLUSION: Echographic features of posterior vitreous face and vitreomacular region correlate with preoperative findings by pars plana vitrectomy and determine a strategy of surgical intervention.

[Microsurgical treatment of posterior capsule opacification].

PURPOSE: To evaluate irrigation/aspiration and posterior capsulectomy in the treatment of the posterior capsule opacification (PCO). SETTING: Department of Ophthalmology, School of Medicine, Comenius University, Bratislava, Slovakia. MATERIAL AND METHODS: Between 1.1.1999 and 31.7.2000 we treated 75 patients (79 eyes) for PCO. Cataract surgery was performed in the years 1990-1999. The artificial intraocular lens (IOL) was implanted in 62 eyes. The interval between cataract surgery and PCO surgery ranged from 4 m.-10 y. The regenerative type of PCO was present in 41, fibrotic in 23 and combined in 15 eyes. Irrigation/aspiration (I/A) was performed in 44 eyes. Capsulectomy with a vitrectomy probe was performed in 35 eyes. I/A procedure was 42x performed with a bimanual system. Capsulectomy was performed in combinations of various insertions of the infusion line (without, anterior chamber maintainer-AChM, pars plana) and capsulectomy (limbal or pars plana approach). It was evaluated: occurrence of peri- and postoperative complications; achieved best corrected visual acuity. RESULTS: Complications during I/A: posterior capsule tearing with vitreous prolaps 4x, IOL subluxation 3x, partial zonular dialysis 2x-1x with vitreous prolaps. During capsulectomy performed through the limbus was observed vitreous prolaps in two eyes. After capsulectomy, performed through pars plana without infusion line, was observed the chronical corneal oedema in one eye. The best corrected visual acuity improved for 2 and more lines in 79.5% of eyes after I/A procedure and in 74.3% of eyes after capsulectomy. The achieved visual acuity lower then 6/60 was caused by changes not related to the cataract and PCO surgery. After testing of various combinations, the most suitable capsulectomy procedure seems to be: infusion line through the limbus, capsulectomy through the radial scleral tunnel incision. In this fashion it is possible to perform the procedure as no-stitch surgery. CONCLUSIONS: Instead of possibility of occurrence of various peri- and postoperative complication after I/A and capsulectomy used for treatment of PCO these procedures complete possibilities in the treatment of PCO. For the regenerative type of PCO is suitable I/A procedure. For the fibrotic type of PCO is suitable capsulectomy through pars plana with AChM. By this procedure it is possible to remove also nearly all vitreous opacities.

[Intraocular foreign bodies in the posterior eye segment].

BACKGROUND: A penetrating eye injury due to an intraocular foreign body (IOFB) may result in a poor vision and even in loss of the eye. A proper analysis of the cause of injury and of the injured eye enables a correct decision to be made concerning timing and method to be used not only for foreign body removal but also concerning of all sight saving surgical procedures. PATIENTS AND METHODS: Over the years 1989-1993, 51 patients with an IOFB in the posterior segment of the eye were treated at the Department of Ophthalmology of the Comenius University in Bratislava. One patient was lost from the long-term observation and in two patients was an intraocular foreign body not removed. We evaluate results in 48 patients. The operative techniques used by foreign body removal and by reoperations are mentioned. The occurrence of peroperative and postoperative complications and the final anatomical and functional results are evaluated. An average follow-up period was 40.7 months. The value of the following prognostic factors was considered in relation to the final visual acuity: size and location of the laceration of the eye, size of IOFB and time of IOFB removal. The statistical significance was tested by Chí-square. Fischer's coefficient for tetrachoric tables was used for the calculation of power of dependence. RESULTS: The foreign body was removed from 64.6% of the eyes after pars-plana vitrectomy. Foreign body caused serious damage of the intraocular structures in 37.5% of the eyes. The poor anatomical result was achieved in 18 (37.5%) and good in 30 eyes (62.5%). In twelve eyes (25%) was implanted an IOL. The final visual acuity of 6/9-6/6 was achieved in 18 (37.5%) and visual acuity lower than 2/60 was recorded in 21 (43.7%) of the eyes. The prediction of bad visual acuity (lower than 2/60) was significant related to: all lacerations except of corneal wounds smaller than 4 mm, IOFB size exceeding 3 x 2 mm. CONCLUSIONS: Management of retained intraocular foreign bodies should be individual and is dependent on the extent of the initial injury and the characteristics and location of the IOFB. The final outcome depends mostly on the extent of the primary injury and of the occurrence of the peroperative and postoperative complications.

[Problems in diagnosis of intraocular foreign bodies in the posterior eye segment].

BACKGROUND: To evaluate the application of the available diagnostic procedures in intraocular foreign bodies impacted in the posterior segment of the eye. MATERIALS AND METHODS: Between 1989-1993 were treated 51 patients and between 1994-1998 were treated 74 patients with intraocular foreign bodies in the posterior eye segment at the Department of Ophthalmology, Medical faculty of the Comenius University in Bratislava. It was evaluated: time of the first medical examination, cause of the injury, time of the right diagnostic and used ancillary testings. In the second period (prospective study) was evaluated also foreign body trajectory in the posterior segment of the eye. The influence on the final visual acuity was evaluated in no proper diagnosed patients. RESULTS: 50.9% of patients in the first period and 55.9% of patients in the second period were injured while at work. Foreign body was metallic in 96% and 94.7% respectively. In the first period 12 patients were injured by various explosions and in the second period 16 patients were injured by working with some circulating tools. The most common ancillary testing was plain film testing and Comberg localisation. In the second period was intraocular foreign body trajectory defined as indirect in 24 eyes (32%). The foreign body achieved its final intraocular position after simple or double contact with the retina. Foreign body was unproperly diagnosed in 14 patients. The final visual acuity was negative influenced by this fact in 8 patients. CONCLUSIONS: A history and ocular examinationare are still most important in diagnosis of the penetrating eye injury with intraocular foreign body. The basic ancilary testing is simple plain film. Ultrasonography is helpful in diagnosis of other intraocular pathological changes. In justified cases is also possible to perform computed tomography. The peroperative intraocular diagnostics is very important for treatment of the all injured intraocular structures.
