In our in vitro fertilization program, only clomiphene/human chorionic gonadotropin-stimulated cycles have been used, yielding 1.6 preovulatory oocytes/woman at follicle aspiration. The cleavage rate was significantly influenced by the percentage of abnormal spermatozoa and by the percentage of motile spermatozoa in the spermiograms. At embryo replacement, a tilted uterus obtained by a full bladder in the woman simplifies the replacement procedure. In the last 12 months, 47 embryo transfers have been carried out, resulting in five clinical pregnancies. Of these pregnancies, one has proceeded to term, one resulted in a miscarriage, and three resulted in ectopic pregnancies.

Fifteen human oocytes incubated with spermatozoa for 24 hr and cultured for 1-3 days were examined by scanning electron microscopy. Five of the oocytes were preovulatory (3 uncleaved and 2 cleaved after culture) and 10 nonovulatory. There was a wide variation in the number of spermatozoa attached to or penetrating the zona pellucida of the oocytes. On preovulatory oocytes, spermatozoa were observed, ranging in numbers from a few to about 50, while on nonovulatory oocytes they numbered from a few to several hundred. Oocytes may differ with respect to sperm-binding sites on the zona pellucida. After incubation in vitro, spermatozoa with abnormal forms were rarely observed on the oocytes or in between corona cells. Selection of spermatozoa may take place on their interaction with oocytes/cumulus corona cells.
Review about the second and third step of in vitro fertilization and embryo transfer. The success rate is influenced by many factors of culture conditions (water, culture medium, plastics and glassware etc.) and by technical details of embryo transfer.
Embryo transfer after in vitro fertilization of oocytes obtained from unstimulated or stimulated women is usually carried out earlier than when an embryo developed in vivo reaches the womb. The possible asynchrony between the developmental stage of the embryo and that of the endometrium at transfer might reduce the prospects for implantation; so also might any secondary effect of stimulation. Preimplantation endometrium dated by plasma hormone analyses of 17 beta-estradiol and progesterone in five unstimulated and eight stimulated women was surveyed. Three features were examined, namely, the frequency, distribution, and appearance of ciliated cells, nonciliated cells, and apical protrusion. Wide regional variations were observed, but no continuous or consistent changes or differences were found from ovulation to the time for implantation in unstimulated and stimulated cycles. Therefore, from the morphologic point of view, the surface might be ready to accept a transferred embryo at any time within a range of days after ovulation.

Human oocytes in different stages of maturation were obtained by follicular aspiration from women given Clomovid and Gonadex. Particles similar to type-C virus were observed in three out of 16 oocytes. The particles were irregularly distributed along the oocyte membrane. They were seen both in a state of budding and lying free in the perivitelline space. Reverse transcriptase activity was detected in three out of nine samples of follicular fluid obtained from women other than those donating the oocytes. The supernatants from bat lung cells and dog thymus cells cultivated with oocytes or follicular fluids were tested for reverse transcriptase. An increased activity was observed only transiently in one case. It is assumed that these findings indicate the expression of endogenous retroviruses in human oocytes.
Seventeen infertile patients were stimulated with clomiphene citrate and human chorionic gonadotrophin (hCG). During laparotomy or laparoscopy, follicles were aspirated. At least one fertilizable oocyte was obtained from 13 patients, who had normal preovulatory plasma 17 beta-estradiol (mean 2.2 nmol/l) in contrast to the 4 patients with infertilizable oocytes (mean 0.4 nmol/l). A close association was found between the success rate of fertilization and the increment of plasma progesterone from the day before to the day after the operation. If the concentrations of 17 beta-estradiol and progesterone in follicular fluid were below an arbitrary limit (17 beta-estradiol less than 500 nmol/l in association with progesterone less than 2,000 nmol/l), no oocytes could be fertilized. Apparently aspiration of oocytes and follicular fluid did not disturb ovarian steroid production during the luteal phase. The results suggest that plasma 17 beta-estradiol and progesterone determinations may serve as useful predictors for oocyte fertilizability in an in vitro system, following clomiphene-hCG stimulation and multiple oocyte recovery.
Human oocytes in different stages of maturation were obtained by follicular aspiration from women given Clomovid and Gonadex. Particles similar to type-C virus were observed in 3 out of 16 oocytes. The particle were irregularily distributed along the oocyte membrane. They were seen both in a state of budding and as free lying in the perivitelline space. Reverse transcriptase activity was detected in 3 out of 9 samples of follicular fluid obtained from women other than those yielding the oocytes. It is assumed that these findings indicate the expression of endogeneous retroviruses in human oocytes.
Ovarian follicles were aspirated at midcycle from 193 women admitted to the hospital because of infertility or for legal sterilization. Eighteen women had not received prior hormone stimulation and 175 had been stimulated either with Clomiphene citrate and hCG with hCG alone. Preovulatory oocytes could not be obtained from any of the women in the non-stimulated group, but were obtained from 86 (49%) in the stimulated group. Of these 86, oocytes from 63 women were cultured successfully and from 47 (75%) cleavage occurred. The most advanced stages observed were about 10-cell embryos, but no attempts were made to culture them further. Almost all the oocytes which cleaved were obtained from women in whom the progesterone level started to rise from the basal level within 24 hours of the operation. Eight cleaved oocytes were taken for transmission electron microscopy (TEM). Early cleavage stages showed spheroidal mitochondria with a few peripheral cristae, while the number of elongated mitochondria with transverse cristae increased in later stages. Mitochondria-vesicle complexes, consisting of a vesicle filled with a dense substance and surrounded by 5-15 mitochondria, were observed in early (but not in more advanced) embryos. The complex is probably part of a transport system from the nucleus to the mitochondria. In the later stages, a type of granulated vesicle appeared. Thus, some changes occurred in tact with the degree of development, while no differences could be seen between oocytes with differing cleavage rates.
