Predictive factors of efficacy of periradicular corticosteroid injections for lumbar radiculopathy.

BACKGROUND AND PURPOSE: Steroid periradicular infiltration is a common nonsurgical sciatic pain treatment of inconsistent efficacy. The purpose of this study was to identify factors for predicting the efficacy or failure of this procedure. METHODS: Two hundred twenty-nine patients with lumbar radiculopathy were prospectively followed up at 2 weeks and 1 year after percutaneous periradicular steroid infiltration. The intensity of radicular pain was scored on the visual analog scale (VAS). Pain relief was classified as "excellent" when the pain was completely resolved or had diminished by 75% or more, "good" for a diminution of 50% to 74%, "fair" for a diminution of 25% to 49%, or "poor" for a diminution of less than 25% or an increase in pain. RESULTS: The mean VAS scores were 6.5 (range, 3.1-9.5) before and 4.2 (range, 0-9.5) 2 weeks after the procedure. Pain relief was graded as excellent in 45 patients (19.7%), good in 48 patients (21%), fair in 45 patients (19.7%), and poor in 91 patients (39.7%). Cause of pain, conflict location, and pain intensity were not predictive factors of radicular pain relief, whereas the symptom duration before the procedure was highly correlated with the pain relief outcome. Patients with excellent results 2 weeks after the procedure had a mean duration of symptoms of 3.04 months (SD 3.28) versus 7.96 months (DS 9.04) in the group with poor pain relief. CONCLUSIONS: Periradicular infiltration is a simple, safe, and effective nonsurgical procedure that should be performed quite early in the course of the illness to provide radicular pain relief, because corticosteroid infiltration is less beneficial for patients with more chronic radicular pain.

Multidetector CT of bowel obstruction: value of post-processing.

The value of imaging in patients with suspicion of bowel obstruction is dependent on the ability to answer questions relevant to the clinical management of patients. Is there mechanical obstruction? Is it a small bowel obstruction (SBO) or a large bowel obstruction (LBO)? What is the transition point? What is the cause of the obstruction? What is the severity of the obstruction? The results of studies published more than 10 years ago using axial and single-slice helical CT gave rise to findings based on axial slices that enables CT to answer these different questions. With the recent advent of multislice CT, large numbers of thin sections can be generated with short image intervals, which is well suited to postprocessing. Postprocessing techniques include standard reformatting methods such as sagittal, coronal and oblique reformatting, curved reformatting, maximum and minimum-intensity projection, variable thickness viewing, and volume and surface rendering. This pictorial review illustrates the added value of postprocessing for answering different questions concerning patients with suspicion of bowel obstruction.

[Breast MRI in invasive lobular carcinoma: diagnosis and staging].

PURPOSE: To assess the use of breast MRI for the diagnosis and staging of invasive lobular carcinoma and its impact on surgical management. MATERIALS AND METHODS: Retrospective study of 35 invasive lobular carcinoma, histologically diagnosed in 34 patients who underwent clinical exam, mammography, ultrasonography and magnetic resonance imaging. RESULTS: Enhancement at MRI was seen for all 35 cancers. It was focal for 24 patients, regional for 10 and diffuse for 1. The kinetic of the enhancement was characteristic of malignancy for 33 patients. For 11 patients the MRI staging was positive, finding 8 news cancers. For 3 patients MRI lead to biopsy of benign lesions but improved the surgical management in the 8 cases of new cancers: wider excision for 3 cases, planned breast conservation converted to mastectomy in 3 cases and excision of contralateral lesion in 2 cases. CONCLUSION: Breast MRI is useful in diagnosis, staging and surgical management of invasive lobular breast carcinoma.

Interstitial pregnancy: role of MRI.

We report the MRI features of two cases of interstitial pregnancy. In both cases, MRI was able to localize the ectopic pregnancy by showing a gestational structure surrounded by a thick wall in the upper part of the uterine wall separated from the endometrium by an uninterrupted junctional zone. Because US may confuse angular and interstitial pregnancies and because interstitial pregnancy has a particular evolutive course, MR imaging may play a key role in the diagnosis and management of women with interstitial pregnancy.

[Adenomyosis: what imaging modality in the diagnosis and staging?].

Adenomyosis constitutes the most frequent myometrial disorder with leiomyoma. Its clinical diagnosis is often difficult. After giving a histopathologic definition, the semiology, the limits, and the diagnostic role of hysterography, transabdominal and endovaginal sonography and MRI will be discussed.

[Imaging of acute urinary obstruction: non-enhanced CT or KUB and US].

The use of non-enhanced CT as a primary imaging technique for acute lumbar pain is now widely accepted in many centers instead of the traditional association of KUB and ultrasound. This review will present the different features of the disease according to each technique, emphasising on respective performances, advantages and disadvantages. Finally, a strategy will be proposed based on clinical presentation.

[Disorders of the peritoneum].

The peritoneum is an important actor in diseases of the abdomen: defects of the peritoneum or mesos may lead to internal hernias; developmental anomalies of the peritoneum may lead to volvulus and bowel obstruction; diffuse or localized primary or secondary tumors of the peritoneum may develop; infectious, inflammatory or ischemic primary diseases of the peritoneum may cause abdominal pain, sepsis or result in the formation of fluid collections. The purpose of this presentation is to illustrate the role of imaging in the diagnosis of these different diseases and to provide some guidelines allowing their detection and characterization.

[Complicated diverticulitis during pregnancy].

The authors report a case of perforated diverticulitis with presence of a fistulous tract between a peridiverticular abscess and the uterus during pregnancy with favorable outcome under medical treatment. The purpose of this case report is to illustrate specific imaging findings and clinical management of diverticulitis during pregnancy.

[Carcinoma with false negative mammogram: when and how?].

Breast carcinomas with false negative mammogram correspond to carcinomas diagnosed with means other than mammogram when the mammogram, at the time of diagnosis, showed no significant abnormality. The rate of false mammogram is difficult to ascertain because few studies have been published on this subject. The absence of mammographic abnormality is related to histopathological characteristics of the tumor and mammographic features of the patient's breast tissue. The small size of the tumor, the lack of microcalcifications often due to tumor necrosis and the absence of any significant associated desmoplastic reaction are the main histopathological factors encountered, particularly for dense breasts.

Pre-operative arterial embolisation of a uterine rhabdomyosarcoma in a 14-year-old girl.

We report a young patient suffering from a uterine rhabdomyosarcoma. Particular features of the present case are: accuracy of the tumour spread evaluation performed by MRI of the pelvis; and the use of pre-operative arterial embolisation. The present multimodal management highlights the usefulness of cooperation between surgeons and radiologists in lowering operative bleeding and finally permitting uterine conservation.

[Nocardiosis--is it frequently observed after the introduction of new immunosuppressive agents in renal transplantation?].

In our series of 1374 renal transplantations performed between February 1970 and December 2002, we observed 6 cases of infection due to Nocardia asteroides. There were 4 males and 2 females, aged 49.8 +/- 12 years (29 to 63 years). One patient received his first transplantation and the 5 others retransplants. Three patients had PRA > 80%, one 28% and one 40%. One patient was diabetic and two had HCV infection. Two of 6 patients experienced acute rejection episodes. Nocardiosis localisation was pulmonary in 5 cases, cerebral in two and mediastinal in one. All patients recovered after reduction of immunosuppression and appropriate antibiotherapy with trimethoprim-sulfamethoxasole (TMP-SMX). When we analyzed the role of immunosuppression, we observed that only two cases were observed in the 933 recipients transplanted between 1985 and 2002 and receiving cyclosporin, contrasting with 4 cases among 174 recipients transplanted between 1996 and 2002 and receiving tacrolimus. Our data suggest that high immunologic risk patients, heavy immunosuppression, and perhaps tacrolimus-based immunosuppression are risk factors of nocardial infection. Early diagnosis of this severe infection, reduction of immunosuppression and appropriate therapy with TMP-SMX resulted in complete recovery in all our patients.

Efficacy of transforaminal versus interspinous corticosteroid injectionin discal radiculalgia - a prospective, randomised, double-blind study.

A prospective, randomised, double-blind study was carried out to compare the respective efficacies of transforaminal and interspinous epidural corticosteroid injections in discal radiculalgia. Thirty-one patients (18 females, 13 males) with discal radicular pain of less than 3 months' duration were consecutively randomised to receive either radio-guided transforaminal or blindly performed interspinous epidural corticosteroid injections. Post-treatment outcome was evaluated clinically at 6 and 30 days, and then at 6 months, but only by mailed questionnaire. At day 6, the between-group difference was significantly in favour of the transforaminal group with respect to Schober's index, finger-to-floor distance, daily activities, and work and leisure activities on the Dallas pain scale. At day 30, pain relief was significantly better in the transforaminal group. At month 6, answers to the mailed questionnaire still showed significantly better results for transforaminal injection concerning pain, daily activities, work and leisure activities and anxiety and depression, with a decline in the Roland-Morris score. In recent discal radiculalgia, the efficacy of radio-guided transforaminal epidural corticosteroid injections was higher than that obtained with blindly-performed interspinous injections.

[MRI study before and after extracorporal shock wave therapy in calcifying tendinitis of the shoulder].

PURPOSE: Extracorporal shock wave therapy has recently been proposed with good results (70%) for treatment of persistent painful calcifying tendinitis of the shoulder. The aim of this study was to evaluate the early impact of shock wave therapy on the anatomic structures of the shoulder. MATERIALS AND METHODS: Eight patients (7 women and 1 man, with mean age of 48 years) were prospectively followed up after undergoing shock wave therapy (1500 pulses with 0.28 mJ/mm2 energy). MRI (T1: 500/12 [TR/TE] and STIR: 7200/60/180/180 [TR/TE/TI/alpha]) were obtained 2 hours before and 15 days after the procedure; in addition for 5 of them one more examination was carried out 6 hours after extracorporal shock wave therapy. RESULTS: There was no significant signal change of the humeral bone or rotator cuff, and the calcification size, when seen (6 times), was unchanged at successive MR examinations. One patient had subcutaneous fat signal change (STIR) next to the zone of impact, which resolved 15 days after the extracorporal shock wave therapy. No bursitis or joint effusion was found. CONCLUSION: Shock wave therapy has no early complications or significant impact on the anatomic structures of the shoulder.

Non-traumatic abdominal emergencies: imaging of acute intestinal obstruction.

The purpose of this paper is to give an overview of the main clinical questions in bowel obstruction, to discuss the value of various imaging modalities, including conventional radiography, ultrasound, and CT, to underline the impact of imaging in the management of patients with suspect bowel obstruction, and then to suggest a diagnostic triage in such patients.
