[Management system for patients requiring stereotactic macrobiopsies of the breast: report on 100 patients].

PURPOSE: The authors describe and evaluate the management system for patients requiring large core vacuum-assisted biopsies at a dedicated breast center. This process includes mandatory review of all requests by a multidisciplinary team. MATERIAL: and method. Patients were provided questionnaires to evaluate their degree of satisfaction with the management process. Results from biopsy and surgery were retrospectively reviewed. RESULTS: From the 100 patients included in the study (106 macrobiopsies), 95,28% of biopsies were informative. The degree of correlation between the histological diagnosis from biopsy and surgery was 86,95% for the 46 operated cases. The degree of acceptability by patients as well as medical and non-medical staff was satisfactory. CONCLUSION: The original feature of this process is the retrospective review of all charts after the histological results becomes available for final validation of the biopsy results and management. The results of the biopsy and the post biopsy management are provided by the initial referring physician, allowing optimal patient care and follow-up.

Treatment of cells with detergent activates caspases and induces apoptotic cell death.

Due to their amphiphilic properties, detergents readily disrupt cellular membranes and cause rapid cytolysis. In this study we demonstrate that treatment of cells with sublytic concentrations of detergents such as Triton X-100, Nonidet P-40, n-octylglucoside and the bile salt sodium deoxycholate induce typical signs of apoptosis including DNA fragmentation and cleavage of poly(ADP-ribose) polymerase molecules. The detergent concentration required for apoptosis was below the critical micellar concentration. Induction of apoptosis was not restricted to human cells but similarly occurred in a variety of other vertebrate cell lines. Unstimulated peripheral blood mononuclear cells were susceptible to apoptosis induction by detergent suggesting that apoptosis in this circumstance is not mediated by CD95. Cell death was not due to influx of calcium from the medium. Apoptosis was blocked and cytolysis prevented by treatment with peptide inhibitors of caspases. These findings suggest a process of apoptosis that is initiated upon nonspecific alterations at the cell membrane level. Physiologic correlates of this process still have to be defined.

Per operative localization of a carcinoid tumour of the breast using indium-111 pentetreotide and a nuclear surgical probe.

A 69-year-old woman presented with a single enlarged lymph node in the left axilla. Clinical examination and other investigations, including various imaging methods, failed to reveal the primary tumour. However, indium-111 pentetreotide scan revealed a site of uptake in the anterior region of the left thorax. Peroperative imaging with 111In-pentetreotide confirmed the tumour uptake and use of a nuclear surgical probe allowed precise localization of the tumour, which was completely resected.

Malignant lymphoma of mucosa associated lymphoid tissue: a new etiology of amyloidosis.

Non Hodgkin Malignant lymphomas (NHML) of mucosa associated lymphoid tissue (MALT) are known to have multiple involvement of the digestive tract. We report one case, presenting with an infiltrative process of the jejuno-ileum, associating lymphoplasmacytoid proliferating cells and amyloidosis. The plasmacytoid cells expressed Alpha and scarce Mu heavy chains, and lambda light chain. Lympho-epithelial lesions were more obvious at the second site of involvement, in the gastric mucosa. The amyloid substance was negative with the Amyloid A component antibody and gave a background noise with Alpha, Mu and lambda chains. No similar report of amyloidosis associated with MALT NHML has been found in the literature.

Plasmacytoid CD8 lymphoma: morphologic and immunologic characterization.

This study describes a 79 year-old female suffering from fever and lymphadenopathy. A lymph node biopsy showed diffuse infiltration of medium-sized cells with plasmacytoid features. The immunologic phenotype determined by cytometric analysis and immunochemistry showed suppressor T antigen (CD2, CD3, CD7, CD8) expression. Absence of CD5 expression characterized this abnormal T cell proliferation. Gamma T cell receptor rearrangement without immunoglobulin heavy-chain gene rearrangement confirmed the T lineage of these plasmacytoid cells and their clonality. Immunological studies are necessary for identification of this T lymphoma with B microscopic findings.

HIV-1 gp120 receptor on CD4-negative brain cells activates a tyrosine kinase.

Human immunodeficiency virus (HIV-1) infection in the human brain leads to characteristic neuropathological changes, which may result indirectly from interactions of the envelope glycoprotein gp120 with neurons and/or glial cells. We therefore investigated the binding of recombinant gp120 (rgp120) to human neural cells and its effect on intracellular signalling. Here we present evidence that rgp120, besides binding to galactocerebroside or galactosyl-sulfatide, specifically binds to a protein receptor of a relative molecular mass of approximately 180,000 Da (180 kDa) present on the CD4-negative glioma cells D-54, but not on Molt4 T lymphocytes. Binding of rgp120 to this receptor rapidly induced a tyrosine-specific protein kinase activity leading to tyrosine phosphorylation of 130- and 115-kDa proteins. The concentration of intracellular calcium was not affected by rgp120 in these cells. Our data suggest a novel signal transducing HIV-1 gp120 receptor on CD4-negative glial cells, which may contribute to the neuropathological changes observed in HIV-1-infected brains.

Restricted synthesis of the fusion protein of measles virus at elevated temperatures.

The elevation of culture temperatures from 35 degrees C to 39 degrees C led to the cessation of the synthesis of the fusion (F) protein of measles virus. This effect was caused by inhibition of the translation of the corresponding mRNA rather than by a decrease in the synthesis or stability of the mRNA or by increased degradation of the F protein at elevated temperatures. The haemagglutinin (H) and F mRNAs were distributed differently in gradients on which polysomes were fractionated. The H mRNA was present almost exclusively in the largest polysomes whereas the F mRNA was more evenly distributed over large and small polysomes. The distribution was not affected by a temperature shift. The inhibition of F protein synthesis thus appeared to be related to a cessation of elongation of the nascent polypeptide chain rather than to a defect in initiation of the translation of the F mRNA at 39 degrees C.

[Pleural thymoma. Apropos of a case and review of the literature].

The authors report a case of pleural tumor discovered in case of thoracic pain in a 72 year old woman. This lesion, discovered on chest X-Ray, developed in the left costo-vertebral groove and was seen as a homogeneous opacity, multilobate, well-defined. CT confirmed the pleural origin, showed a thickening of posterior pleural, a polylobate tumor well-defined on its lung side, non-invasive and of tissue density. Histopathological examination of the operative specimen showed it to be a thymoma with an exceptional localization.

Does diethylstilbestrol (DES) change the stress fiber organization in C6 rat glioma cells?

C6 Rat glioma cells acquire an astrocyte-like morphology (cytoplasmic shrinking) after exposure to beta-adrenergic compounds (e.g., isoproterenol) in serum-free medium. This morphological response is mediated by elevation of the intracellular cAMP level and possibly by activation of a kinase phosphorylating and inactivating the myosin-L-kinase in analogy to observations with smooth muscle cells. The result is a disturbance of the stress fiber function, which depends on a cooperation between actin and myosin. Diethylstilbestrol (DES) induces an identical morphological response in these cells under serum-free conditions. However, under the influence of DES no increase of the cAMP level was observed. In addition, at concentrations not inducing these morphological responses, DES inhibits the isoproterenol-induced effect when administered simultaneously or prior to the beta-receptor agonist. DES does not inhibit the isoproterenol-mediated morphological response when added after isoproterenol treatment. Furthermore, if serum is added to cells showing the isoproterenol- or DES-mediated astrocyte-like morphology (DES or isoproterenol present all the time) the cells regain their normal fibroblast-like morphology within 30 min. In view of these results the question arises whether DES interferes with myosin-L-chain phosphorylation or whether it directly interacts with the cytoskeleton stress fiber components, as cytochalasin B1 does. Thus it remains to be established whether the observed effects are related to the estrogenic activity of DES. Similar effects were observed with Syrian hamster embryo fibroblasts under the influence of DES and the naturally occurring steroid estrogen 17-beta-estradiol.
