Short report: comparison of two doses of balsalazide in maintaining ulcerative colitis in remission over 12 months.

In a four-centre prospective double-blind trial, 108 patients with ulcerative colitis in remission were randomized to receive balsalazide in doses of 3 g or 6 g/day for 12 months. The patients were assessed at 3-monthly intervals clinically, sigmoidoscopically and with routine haematology and biochemistry. Remission rates of 77% (3 g/day) and 68% (6 g/day) at 12 months were not significantly different. Intolerance reactions leading to withdrawal from the study occurred in only 9 patients (8%), all occurring in the first 7 weeks of the study. Balsalazide is therefore both highly effective in maintaining remission in ulcerative colitis and well tolerated in both conventional and high dosage (the latter equivalent to 5.5 g/day of sulphasalazine). In this study no distinct advantage in maintenance of remission has been found for the higher dose of balsalazide.

A method for comparing two systems of acute 24-hour psychiatric care.

A quasi-experimental method was developed to evaluate the cost-effectiveness of a public system of 24-hour acute psychiatric care in Santa Clara County, California, before and after a new treatment setting was introduced. The original system relied on a 54-bed psychiatric unit in a county general hospital; the new system consisted of a 20-bed unit in the general hospital plus a 45-bed nonhospital psychiatric health facility. The study demonstrated that the per diem cost of the psychiatric health facility was approximately 60 percent that of the original general hospital unit, but the average difference in cost per episode between the two systems was only about +25, primarily due to longer lengths of stay in the new system. In addition, patients treated in the new, combined system appeared sicker at discharge than those treated in the old system. The findings suggest the importance of simultaneously evaluating both cost and treatment effectiveness to make sure that one element does not dominate program direction at the expense of the other.

Further data on the effects of subliminal symbiotic stimulation on schizophrenics.

This investigation further examined the effects of activating unconscious symbiotic fantasies in schizophrenics. One hundred twenty-eight hospitalized schizophrenic men who qualified as "relatively differentiated" on the Adjective Rating Scale were randomly assigned to four groups. Each group was assessed for pathological thinking, pathological nonverbal behavior, and self-esteem before and after the subliminal exposure of an experimental and control stimulus. The control stimulus for all groups was the message PEOPLE ARE WALKING and the experimental stimuli were the messages MOMMY AND I ARE ONE, MOMMY IS ALWAYS WITH ME, MOMMY FEEDS ME WELL, and I CANNOT HURT MOMMY (one for each group). One half of each group was subliminally exposed to verbal messages only and one half to verbal messages accompanied by congruent pictures. The first stimulus (MOMMY AND I ARE ONE) was intended to activate unconscious symbiotic fantasies that in a number of prior studies reduced pathology in groups of relatively differentiated schizophrenics. The other stimuli were intended to activate reassuring unconscious fantasies about "mommy" that were not specifically symbiosis-related. Only the MOMMY AND I ARE ONE stimulus led to more adaptive behavior and did so on all three dependent variables. This supported the supposition, also borne out in two other studies, that it is specifically symbiosis-related gratifications that are ameliorative for schizophrenics. The above results were considerably stronger for the subgroup that was exposed to a picture accompanying the MOMMY AND I ARE ONE message. This was viewed as probably the result of the pictorial representation serving as a concretization of the more abstract verbal message and as such being more relevant to the relatively primitive mode of thinking in schizophrenia.

Positive Coombs test in Hodgkin's disease: significance and implications.

To clarify the clinicopathologic characteristics of Coombs' positivity in Hodgkin's disease, the records of 71 cases were reviewed. The direct Coombs test was positive in seven. Mean age of the seven was 22 yr (range 11-33). All were males. All had extensive disease (pathologic stage III or IV) and six had systemic (B) symptoms. Four had mixed cellularity; three had nodular sclerosis. The positive Coombs test was detected at original diagnosis in three and at time of relapse in four. Although all were anemic, only three had evidence of overt hemolysis. The antibody responsible for Coombs positivity was characterized in three and fulfilled the criteria for IgG anti-It. The presence of a positive direct Coombs test in the patient with Hodgkin's disease suggests active and advanced disease. The presence of IgG anti-It may represent a unique antibody in the Coombs-positive hemolytic anemia associated with Hodgkin's disease.
