Ventriculosubcutaneous shunt for temporary treatment of neonatal post-IVH hydrocephalus: a technical note.

A technique for temporary ventricular drainage using a subcutaneous pouch is described for use in neonatal hydrocephalus after intraventricular hemorrhage. The advantages include continuous, valve-regulated ventricular decompression, no exposed tubing, avoidance of electrolyte imbalance and of the loss of CSF, obviation of the need for repeated tapping, and the ability to convert to a peritoneal shunt on an elective basis.

Cerebral Hemorrhage

Giant cerebral Echinococcus cyst with galactorrhea and amenorrhea.

A case of giant left-sided frontal cerebral Echinococcus cyst causing headaches, galactorrhea--amenorrhea, secondary sterility, and gain in weight in an adult female patient is reported. The operative removal of this huge cyst led to complete neurological and endocrinologic recovery.

Brain sarcoma of meningeal origin after cranial irradiation in childhood acute lymphocytic leukemia. Case report.

The authors report their experience with an unusual case of intracerebral sarcoma of meningeal cell origin in an 8 1/2-year-old girl. This tumor occurred 6 1/2 years after cranial irradiation at relatively low dosage (2200 rads) had been delivered to the head in the course of a multimodality treatment for acute lymphocytic leukemia. The tumor recurred approximately 10 months after the first surgical intervention. Macroscopic total excision of the recurrent growth followed by whole-brain irradiation (4500 rads) failed to eradicate it completely and local recurrence prompted reoperation 18 months later. This complication of treatment in long-term childhood leukemia survivors is briefly discussed, as well as the pathology of meningeal sarcomas.

Brain Neoplasms

Acute central nervous system changes due to intoxication by Manzidan (a combined dithiocarbamate of Maneb and Zineb).

A 42-year old man sprayed Manzidan (a combined dithiocarbamate of Maneb and Zineb) on a cucumber plantation, twice during a week. Behavioural changes appeared after the first application, and loss of consciousness, convulsions and right hemiparesis after the second application with diffuse slow rhythm in the EEG. Both the behavioural and CNS symptomatology disappeared spontaneously after a few days. The previous history of the patient, the sudden appearance and spontaneous disappearance of the symptoms and a dose-response relationship strongly suggest that Maneb and Zineb were the cause of the illness.

Chronic eosinophilic leukemia complicated by epidural myeloblastoma.

Epidural myeloblastoma, which compressed the spinal cord, was the first evidence for chronic granulocytic leukemia, eosinophilic type, Ph chromosome negative. This manifestation was preceded by 3 years follow-up of a patient with persistent eosinophilia of 60% mature eosinophils. The only clues for the diagnosis of leukemia were splenomegaly and high serum vitamin B12, most of which was bound to transcobalamin I. The latter finding presents a useful diagnostic criterium in myeloproliferative disorders.

Febrile convulsions in an adult as presenting symptom of intracranial dermoid cyst: a case report.

A 32-year-old man had generalized tonic-clonic epileptic seizures associated with episodes of recurrent high fever for 6 years. Repeated physical examinations including neurological status, EEG and nuclear brain scan were negative. Brain CT showed a non-homogeneous parasellar cyst of low density and fat-fluid levels in the lateral ventricles. The diagnosis of intracranial dermoid cyst was confirmed at surgery and histopathologically. Recurrent febrile convulsions and chemical meningitis may be the only clinical manifestations of ruptured dermoid cyst. The CT features of intracranial dermoid cyst are pathognomonic.
