[A case of metachronous carcinoma of the colon diagnosed and radical operated with restorative anastomosis after more than 5 years from the first left hemicolectomy].

The authors described their observation about the female patient with an advanced carcinoma (T4) of the descendent colon where left hemicolectomy was done in the beginning of 1998. The patient is operated again after more than 5 years because of the cancer in the site of the anastomosis. Radical reresection with restorative anasomosis is performed and for this reason the colon is putting via mesenteric radix. The case is analyzed and there are suggestion about is this a local recurrence or "de novo" carcinoma It is important to follow up the patients during lifetime for early uncovering of local recurrence. Endocoloscopy and scener are preferable for this aim.

[Factors influencing successful rate of intrauterine insemination].

Intrauterine insemination (IUI) is a method for the treatment of marital infertility involving the intrauterine or fallopian deposition of washed spermatozoa, depending on the amount of inseminated semen. Numerous factors may be affect of pregnancy rates after IUI.

[Use of frozen human semen].

The necessity of establishing of monitored donor's genetic bank in Bulgaria is of exceptional necessity at the moment, when health care in our country is in reorganization and is set on a new base. Our studies were directed towards the organization of bank for cryopreservation of spermatozoa taken of: Donor's genetic material for the needs of obstetric and gynaecological laboratories From men that are expected to go through an X-ray or chemical treatment because of tumors or are planned for operation of the testicules. The genetic bank in IBIR is organised accordance with Bulgarian Legislation.

Effect of pentoxifylline on motility and longevity of fresh and thawed dog spermatozoa.

The effect of pentoxifylline on fresh and frozen-thawed dog spermatozoa was evaluated. In Expt 1, 0.0036 mol l-1 or 0.0072 mol l-1 pentoxifylline was added to fresh dog spermatozoa. Semen was incubated at 25 degrees C or 39 degrees C. In Expt 2, pentoxifylline was added either before freezing or in the process of thawing at a final concentration of 0.0072 mol l-1. Addition of pentoxifylline significantly increased the percentage of progressively motile spermatozoa in fresh semen. The most beneficial effect of pentoxifylline on frozen-thawed semen was observed (P < 0.001) when it was added to semen in the process of thawing.

Characterization of a cancer cachectic factor.

Cancer cachexia is a syndrome of progressive wasting which has been suggested to be mediated by tumour-necrosis factor-alpha, interleukins 1 and 6, interferon-gamma and leukaemia-inhibitory factor. It has proved difficult to correlate levels of tumour-necrosis factor-alpha and interleukin-6 with cancer cachexia, and the weight loss induced by leukaemia-inhibitory factor may be due to toxicity. In the murine adenocarcinoma MAC16, cachexia is mediated by circulatory catabolic factors, which we have now isolated using an antibody cloned from splenocytes of mice transplanted with the MAC16 tumour, with a delayed cachexia. The material is a proteoglycan of relative molecular mass 24K which produces cachexia in vivo by inducing catabolism of skeletal muscle. The 24K material was also present in urine of cachectic cancer patients, but was absent from normal subjects, patients with weight loss due to trauma, and cancer patients with little or no weight loss. This suggests that cachexia in mice and humans may be produced by the same material.

Suicide by self-inflicted burns.

Eighty-nine patients who had made an attempt on their life by self-burning were reviewed. For 12 years (1983-94) these patients were treated at the Institute of Emergency Medicine 'Pirogov', Sofia (Bulgaria). The gender distribution was almost equal (48 males: 41 females); the most vulnerable age was 20-40 years (57 patients) (64.05 per cent). Flame was the preferred thermal agent of attempted suicide (chosen by 72 patients), followed by electricity (11 patients). Thirty-one of the injured persons died (34.8 per cent). Forty-six of those who survived were operated on (116 surgical interventions: an average of 2.52 operations per patient) the mean length of treatment was 56.3 days. Eleven of the operated patients died subsequently (average stay in the hospital 8.20 days). There were some difficulties in collecting data about this method of self-harming. Twenty-three of the patients had recognized psychic disorders, in 43 of the patients the accident was preceeded by an agitated depression; in 15 by severe alcoholism. Emphasis is placed on the role of joint teams of physicians, psychologists and sociologists, for the treatment of these difficult patients.

[Streptococci and staphylococci in raw cow's milk on the market].

Studied were a total of 649 batches of raw cow milk produced on 42 farms, and 1100 flushing samples taken from 370 containers for the presence of pathogenic streptococci and staphylococci. It was found that 26.5 per cent, resp., 18.03 per cent of the batches contained coagulase-positive staphylococci and pathogenic streptococci (in 8.94 per cent--Str. agalactiae, in 6.0 per cent--Str. uberis, and in 3.08 per cent--Str. dysgalactiae ). In 28.3 per cent of the batches there were both strepto - and staphylococci that had pathogenic character. The study of the flushing material showed that the cleaning of containers for shipping the milk from the farms to the dairy processing enterprises was effective--only 1.62 per cent of the cisterns were contaminated with pathogenic strepto - and staphylococci.

What is emergency? Analysis of a population presenting to an emergency room.

The population presenting at emergency rooms is increasing in Belgium. What is emergency in 1983 and why do people come? From a study of the emergency room population in a relatively new university hospital it emerges that more than 30% of those patients are in critical conditions or require in hospital treatment and about 50% are in acute discomfort. Our results confirm the impression that inappropriate use of emergency rooms is more an impression than a real problem. For socio-economic and cultural reasons, people go to emergency rooms when they need or think they should have medical help. Some emergency rooms are well able to cope with this situation. Others, less well-equipped in staff and facilities are dangerous and should be improved or closed. Unfortunately education and training programs for emergency physicians are still unknown in many parts of the French-speaking Belgium.

[Microbiological studies of Mladost sour milk].

Studied were a total of 239 productional batches of Mladost sour milk to establish the occasional contamination with coliform bacteria. It was found that 97.9 per cent of the batches had coli titers of 0.1 and over 0.1, and 2.1 per cent of the batches--over 0.01. Fourteen productional experiments were carried out to ascertain the sources of contamination with coli forms, yeasts, and moulds. It was found that the productional starter used for this type of sour milk did not contain coliforms and moulds in 0.1 cm3 but in about 43 per cent of the batches it contained 20 to 470 cm3 yeasts. It was also established that both yeasts and moulds that have found their way to the product multiplied in it to a various extent, depending on the activity of the cultures and the temperature of storage. Some of the stored batches deteriorated in terms of their organoleptic indices. At 11,000 cm3 yeasts the deterioration becomes stronger following the cummulation of greater amounts of biomass.

[Proximal duodenopancreatectomy in the treatment of tumors of the bilio-pancreatic junction].

Between 1978 and 1983, 32 patients underwent a proximal duodeno-pancreatectomy for malignant lesions of the ampullary region on head on the pancreas. Pre-operative diagnosis was made by endoscopic cholangio-pancreatography, echography, tomodensitometry and arteriography. 14 patients underwent a pre-operative endoscopic bile drainage procedure. The operative mortality was 6,2% and the morbidity 43%; the intensity of the icterus as well as the preoperative bile drainage procedure did not change these factors significantly. The pre-operative diagnostic procedures permitted a clear definition of the extent of the tumor and retrospective analysis demonstrated their prognostic significance with regard to long-term survival. 11 patients are still alive. The presence of histologically proven metastases reduce significantly the duration of survival. Surgical resection is the best treatment for malignant tumors of the ampullary region and the head of the pancreas without local or distant metastases.
