[Role of Gadolinium-DTPA in the assessment of renal tumors with magnetic resonance].

To investigate the role of Gd-DTPA in the MR study of renal cancers, 30 patients affected with primary (28) or recurrent (2) renal cancers were submitted to high-field MRI (1.5 T). T1- and T2-weighted and Gd-DTPA-enhanced T1-weighted sequences were always acquired. Based on qualitative and quantitative criteria (calculation of the signal-to-noise ratio in the tumor mass and of the signal-difference-to-noise ratio between tumor and renal parenchyma), Gd-DTPA influence was studied on the following variables: signal intensity and tumor demonstration, detectability of morphostructural features of tumor tissue and staging accuracy. As for the latter variable, MR results were compared with the results of anatomical and surgical staging (28 cases). Relative to unenhanced T1- and T2-weighted sequences, on Gd-DTPA-enhanced sequences 80% of the lesions were markedly hypointense relative to renal parenchyma and they were better demonstrated in 93% of cases. The S/N and the SD/N ratios were higher on Gd-DTPA-enhanced than on unenhanced images. After Gd-DTPA administration, the intratumoral necrotic areas, the walls and the septations of the cystic masses and the boundaries between tumors and renal parenchyma were better demonstrated. Staging accuracy was the same (90%) on both Gd-DTPA-enhanced T1-weighted and unenhanced images. Therefore, Gd-DTPA, although failing to increase staging accuracy, improves tumor depiction and demonstrates the morphostructural features of the mass. Since Gd-DTPA caused no side-effects, Gd-DTPA-enhanced T1-weighted sequences can replace T2-weighted sequences, whose acquisition time is definitely longer.

[Osteosarcoma: magnetic resonance imaging of the effects of preoperative chemotherapy].

The effects of preoperative chemotherapy for osteosarcoma of the extremities were investigated with MR imaging. Forty-six patients affected with high-grade osteosarcomas of the extremities underwent MRI before and after two cycles of preoperative chemotherapy. At the end of treatment all tumors were resected and the rate of tumor necrosis was assessed on histologic sections. The MR images obtained before and after treatment were compared. Decreased tumor size, perilesional intramedullary edema and more marked lesion outline correlated well with positive histologic response to preoperative chemotherapy. Soft tissue edema and joint effusion did not change significantly and no clear correlation with histologic response was observed. Decreased signal intensity on T2-weighted images is likely to indicate response to chemotherapy. Likewise, poor response correlated with unchanged/increased signal intensity. Therefore, MRI appears a useful means to evaluate chemotherapy-induced changes in osteosarcomas of the extremities.

[MR in the characterization of benign ovarian masses].

To investigate the role of MRI in characterizing benign ovarian masses, the MR images of 64 patients affected with benign ovarian masses were retrospectively reviewed. The benign nature of the masses was proven at surgery (42 cases), fine-needle biopsy (10 cases), laparoscopy (6 cases) and follow-up (6 cases). MRI correctly characterized 56 of 64 masses (87.5%). In particular, all the cases of simple serous cyst (9), of hemorrhagic cyst (10), of fibroma (4), of dermoid cyst (18) and of tubo-ovarian abscess (7). MRI misdiagnosed 4 endometrial cysts--2 of them as hemorrhagic cysts and 2 as dermoid cysts-, 1 serous cystoadenoma as cystoadenocarcinoma and 1 suppurative mucinous cystoadenoma as tubo-ovarian abscess. Moreover, 1 angiofibroma and 1 hematosalpynx were misdiagnosed as endometrial cysts. In conclusion, MRI can be considered a second-choice diagnostic tool which can replace CT in the evaluation of the patients in whom US alone fails to yield an unquestionable diagnosis.

Ticlopidine lowers plasma fibrinogen in patients with polycythaemia rubra vera and additional thrombotic risk factors. A double-blind controlled study.

Thirty-seven patients affected by polycythaemia rubra vera (PRV) and with at least one additional thrombotic risk factor (overt vascular disease, diabetes mellitus, treated hypertension, smoking habit, plasma hyperviscosity, hyperfibrinogenemia) were enrolled in a double-blind randomized placebo-controlled study, and 18 were given ticlopidine 250 mg, b.i.d., for 60 days. All the patients had previously been submitted to cytoreduction, and PRV was under control in all cases at the start of the study. During the study, the haematological parameters were controlled every 15 days, and venesection was performed if haematocrit was greater than 46%. Whole blood viscosity, at low and high shear rates, plasma viscosity, and fibrinogen were measured on days 0 and 60. In the ticlopidine group, we recorded a significant 13.14% reduction of the mean fibrinogen level after treatment (390 +/- 63 vs. 449 +/- 97 mg/dl, p less than 0.01). All the other haemorheological parameters were not significantly modified by ticlopidine treatment, nor were there significant modifications recorded in the placebo group. Our study shows that ticlopidine may reduce a probable thrombotic risk factor (hyperfibrinogenemia) in PRV patients.

Radiographic and computed tomographic findings in hydatid disease of bone.

Twenty-six patients with hydatid disease of bone were evaluated by means of radiography and conventional tomography. Fourteen patients underwent high resolution computed tomography (CT). In two patients with vertebral disease, magnetic resonance imaging (MRI) examination was also performed. Seventeen patients underwent surgery with histological examination in all. Based on the surgical data and gross examination of the specimen, the radiographic and CT findings have been reviewed to identify the most characteristic radiographic features and to assess the role of CT. Radiographic finding of hydatidosis are rarely typical in bone, and only in a few patients can CT contribute to the diagnosis. On the other hand, the local extension of the lesion both in bone and in soft tissues, which is essential in planning surgery, is always demonstrated well by CT.

The value of computed tomography in the diagnosis of soft-tissue swellings of the hand.

Twenty patients with palpable swellings of the hand were investigated by computed tomography. The results, when compared with the pathological findings, lead us to consider this a technique of considerable value in the assessment of this kind of disease. The importance of angiography and, occasionally of magnetic resonance imaging, are also stressed.

CT in established Volkmann's contracture in forearm muscles.

Differential diagnosis between true and pseudo-Volkmann's contracture should not be based on clinical grounds alone, because clinical data are often insufficient in determining the severity and the extent of muscle damage. CT has been utilised in 11 cases where a flexion contracture of the forearm muscles was present and corresponded closely with the surgical findings. True and pseudo-Volkmann's contracture can thus be distinguished and the severity and extent of the muscle damage determined.

Long-term effects of ticlopidine on fibrinogen and haemorheology in patients with peripheral arterial disease.

The effects of ticlopidine treatment (250 mg b.i.d. for 21 months) on fibrinogen and other rheological variables, as compared to placebo, were studied in 44 patients with intermittent claudication due to peripheral arterial occlusive disease. Blood samples were collected every 3 months during this double-blind, randomised placebo-controlled trial which lasted 21 months. Consistently lower values of fibrinogen, haematocrit and whole blood viscosity at high and low shear rate levels were found in the ticlopidine group; the intergroup differences were statistically significant at most but not all follow-up examinations. A significant time-related variance was observed in the ticlopidine group for the measured variables, also after correction for the variability found in the placebo group. Thus, the observed changes in the ticlopidine group are mainly treatment related. These effects on fibrinogen and haemorheology may contribute, besides the known antiplatelet activity of the drug, to the clinical improvement reported in a larger group of claudicants to which the present subset of patients belong.

Effect of slimming on metabolic and haemorheologic patterns in a group of obese subjects.

In order to evaluate slimming effects on certain metabolic (cholesterol, triglycerides, basal insulinaemia) and haemorheologic (haematocrit, fibrinogen, whole blood viscosity and plasma viscosity) rates, we studied 24 obese subjects (15 female and nine male) aged 25-58, with BMI ranging from 35.5 to 67, before and after a hypocaloric diet period involving a 20-kg weight loss. All subjects underwent blood sampling to assess: OGTT, plasma proteins, serum total cholesterol, HDL-cholesterol, serum triglycerides, haematocrit, whole blood viscosity at high and low shear rate with and without correction of 45 per cent haematocrit, plasmatic capillary viscosity and fibrinogen level. Our study showed no significant changes in plasma proteins, serum HDL-cholesterol, haematocrit, fibrinogen and whole blood viscosity at high and low shear rate, while basal insulinaemia, total cholesterol, triglycerides, low shear rate corrected at 45 per cent haematocrit blood viscosity and capillary plasmatic viscosity are significantly reduced. However the analysis of possible relation between the differences of assessed rates before and after slimming has shown no significant correlations. In conclusion, we can say that the slimming of very obese subjects improves blood and plasma viscosity, but the mechanism by which this improvement occurs is not the one which usually affects the determination of these rates.

CT diagnosis of lumbosacral conjoined nerve roots. Findings in 19 cases.

The authors report the observations derived from CT evaluation of 19 cases of lumbosacral conjoined nerve roots; 11 of these have been confirmed by lumbar myelography and/or at surgery. They conclude that CT without intrathecal metrizamide allows the recognition in most cases of the presence of conjoined nerve roots and to differentiate them from a herniated disk fragment; this is especially useful to avoid surgical damage of anomalous roots.

[CT evaluation of sacrococcygeal chordoma].

From June 1985 to September 1986, eight patients affected by sacrococcygeal chordoma were examined by means of conventional radiography and tomography first, and then by high-resolution CT. All patients underwent surgery and an histologic examination of the surgical specimen was carried out. CT always allowed to detect the sacro-coccygeal neoplasm, which looked like a solid non-homogeneous mass; in 50% of cases coarse calcifications were found. CT made it possible to evaluate the growing side of the tumor, its relation with the adjacent anatomical structures-especially the longitudinal extension and the involvement of the sacral hiatus; these findings proved to be very useful in order to choose the most appropriate surgical technique. CT findings also suggested the nature of the tumor but rarely allowed a definitive diagnosis. CT is, at any rate, mandatory in order to evaluate the real extent of the tumor and to plan surgery.

[Value and limitations of computerized tomography in the study of benign tumors of the bone].

In order to stress the role of CT in the evaluation of benign tumors of the bone the authors studied by means of a high-resolution CT scanner 74 patients that later underwent surgery (70 cases) and histological examination (all cases). CT findings were compared with the surgical and histological data. The results prove that the diagnostic contributions of CT are less important in the evaluation of benign than in malignant bone tumors. The major contributions of CT were:--the capability of detecting the presence of osteoid osteoma not found by means of conventional radiographic techniques;--differentiation between benign osteochondroma and peripheral chondrosarcoma by measurement of the peripheral cartilaginous cap: this criterion, on the other hand, does not seem to be always reliable and useful;--differential diagnosis of different types of cystic and fibrocystic diseases (simple and aneurysmal bone cyst, fibrous dysplasia);--evaluation of soft-tissue tumoral masses and of their extra-osseus extent in giant cell tumor and chondroblastoma; this allows us to establish the real extension of the tumor and to plan surgery more accurately.

[Computerized tomography in spinal osteoblastoma. Observations in 10 cases].

In order to stress the role of computed tomography (CT) in the evaluation of benign spinal osteoblastoma, the authors examined by CT 10 patients with surgically and histologically proved osteoblastomas of the spine. Basing on surgical and histopathological findings CT proved to be a very important method for attesting the presence of the tumors (usually showed by radiographic examination), and specially for their staging and follow-up. CT in fact showed the real extent of the tumor into the spinal canal and towards the paraspinal soft tissues, and allowed the early detection of post surgical relapses. The authors conclude that CT is a primary diagnostic modality for preoperative planning and follow-up of benign osteoblastoma.

[Computerized tomography in the assessment of soft-tissue neoplasms. Observations in 124 cases].

In order to evaluate the role of Computed Tomography (CT) in prediction of nature, staging and follow-up of soft-tissue tumors, the authors examined by CT 124 patients with soft tissue neoplasms who later underwent surgery (116 cases) or fine needle biopsy (8 cases). Comparison between CT and surgical or anatomical results showed that CT was able to correctly predict the benignancy or malignancy of the masses in 76% of cases but it was very seldom able to allow an histological prediction. On the contrary CT was found to be a very useful tool for pre-therapeutic staging and follow-up of the tumors, because it gave many diagnostic information which influenced therapeutic choices and strategies.

[Computerized tomography and scintigraphy in the evaluation of osteoid osteoma].

The authors report their experience of bone scintigraphy and computed tomography in 19 patients, with suspected osteoid osteomas, who underwent surgery. Basing on surgical findings, the role of these techniques in diagnosis, surgical planning and follow-up of osteoid osteoma is stressed and the results are compared with those of conventional radiography.

Pulmonary vascular reactivity and hemorheology in patients with chronic cor pulmonale: responses to pentoxifylline at rest and during exercise.

This study was carried out in order to evaluate changes in pulmonary vascular reactivity and in hemorheology induced by pentoxifylline infusion (100 mg) at rest and during standardized exercise in patients with chronic cor pulmonale secondary to chronic obstructive pulmonary disease. The administration of pentoxifylline at rest was associated with reduction in mean pulmonary artery pressure (p less than 0.01), pulmonary vascular resistance (p less than 0.01) and right ventricular stroke work index (p less than 0.02). Standard exercise performed after pentoxifylline infusion was also associated with significant reduction in mean pulmonary artery pressure and pulmonary vascular resistance. Rheologic tests showed less evident changes. Our data suggest that pentoxifylline significantly improves pulmonary hemodynamics at rest and partly reverses its changes, after exercise, in patients with chronic obstructive pulmonary disease.
