Direct vesicoureteral scintigraphy: quantifying early outcome predictors in children with primary reflux.

UNLABELLED: This study quantifies some of the outcome predictors in a group of children with primary vesicoureteral reflux who were initially managed medically. METHODS: We studied 133 patients with primary reflux for 7.1 +/- 2.2 yr. Direct vesicoureteral scintigraphy (DVS) was used to prospectively measure the absolute bladder volume at which reflux began and the maximum volume of urine refluxed into the ureters during the filling and voiding phases of their first two DVS studies. Findings were related to outcome as defined by spontaneous resolution or the eventual need for reconstructive surgery. RESULTS: Medical management eventually failed in 35% of this sample. Patients who did not begin to reflux until their bladders had been filled to more than 60% total bladder capacity had a substantially smaller risk of surgery than those who began to reflux at smaller bladder volumes. Patients who refluxed a volume of urine back into their ureters that was less than about 2% of their total bladder capacity had a substantially smaller risk of surgery than those who refluxed more than 2%. The difference between groups was significant for both DVS variables (p < 0.001). CONCLUSION: Quantitative DVS contributes to the assessment of prognosis in children with vesicoureteral reflux who are managed medically.

[Blood transfusion preparations with a low leukocyte count. I. Preparation of thrombocyte concentrates from buffy coats].

The preparation of platelet concentrates from "buffy coat" makes it possible to obtain a platelet concentrate with a low leucocyte and erythrocyte content. Concurrently prepared resuspended red cell concentrates "buffy coat" removed have moreover a low leucocyte and thrombocyte content. The mean number of thrombocytes in 1 TU of thrombocyte concentrate was 0.45 +/- 0.2 x 10(11), the leucocyte content was 0.03 +/- 0.03 x 10(9) and the red cell content was 0.8 +/- 0.7 x 10(9). The described method makes it possible to process in a rational manner blood samples into resuspended red cell concentrate a thrombocyte concentrate and plasma content. The administration of transfusion preparations with of preparation of platelet concentrates in common practice.

[A registry of potential bone marrow donors].

Transplantation of bone marrow is relatively successful treatment in some haematological diseases. The most suitable donor of bone marrow is a HLA genotypic identical sibling; however, roughly only 30% of patients have a suitable related donor. In order to find a HLA identical non-related donor for a maximum number of patients it is necessary to create as extensive registers as possible of potential bone marrow donors. The authors mention the possibility to obtain these donors from the ranks of regular blood donors and they mention the advantages of obtaining a register from this group of subjects.

The magnitude of left ventricular myocardial hypertrophy related to the degree of its dysfunction in patients with dilated cardiomyopathy (DCM).

The aim of the study was to analyze the relationship between the magnitude of left ventricular (LV) hypertrophy and selected haemodynamic parameters reflecting LV systolic and diastolic function. The "hypertrophy-function" relationship was evaluated in 22 patients with dilated cardiomyopathy (DCM) and in patients with left ventricular dilatation resulting from volume overload due to valve disease (DVOL). The parametres of systolic and diastolic left ventricular function were obtained from right- and left-heart catheterization and quantitative angiocardiography, DCM patients were divided into subgroups depending on the magnitude of hypertrophy and degree of dilatation: Ia- moderate hypertrophy (100 g/m2 < LVMI < 175 g/m2). Ib- massive hypertrophy (LVMI > 175 g/m2); IIa- mass/volume ratio (M/V) < 1.1, and IIb - M/V > 1.1. It was found that the magnitude of myocardial hypertrophy and the M/V ratio do not affect the degree of systolic and diastolic dysfunction in patients with DCM. Myocardial hypertrophy accompanying dilatation due to DCM and DVOL showed very similar progression of impairment of isovolumetric systole and relaxation. Significant differences in EF, LVMDP and LVEDP may result from a different degree of total LV volume stiffness as a consequence of various mechanisms of hypertrophy in DCM and DVOL.

[Comparison of the value of echocardiographic examination in relation to hemodynamic studies in atrial septal defect of the ostium secundum type].

The aim of the study was in comparative analysis of the results obtained using the cardiac catheterization and the echocardiographic parameters in patients with atrial septal defect of the ostium secundum type. The studies have been performed in the group of 31 patients aged 16 to 58 years (mean = 31.9) in whom the right heart catheterization of the two-dimensional or Doppler's echocardiography has been performed. The following parameters have been analysed: pulmonary flow (Qp), systemic flow (Qs), shunt size (S), the Qp/Qs ratio, the systolic gradient via tricuspidal valve (TPG). The highest correlation in the invasive and the not invasive examination has been shown for TPG (r = 0.93). Other parameters showed no statistically significant differences.

A contribution to the incidence of nucleoli in normal human blood monocytes.

BACKGROUND AND OBJECTIVE: Normal blood monocytes were studied in blood donors to provide more information on the presence of nucleoli and nucleolar types in these cells. METHODS: Nucleoli in monocytes were visualized using cytochemical procedures that detect RNA and characteristic nucleolar proteins, i.e. nucleophosmin, nucleolin, fibrillarin and AgNOR proteins, in peripheral blood smears. RESULTS: Nucleoli were detected in all blood monocytes. The nucleolar coefficient (number of nucleoli per cell) was 2.6 and no differences were found between men and women. Concerning the incidence of nucleolar types, monocytes from both male and female blood donors possessed mainly only inactive micronucleoli characteristic of mature or advanced maturation stages of blood cells; however, 16-20% of monocytes also contained functionally dominant ring-shaped nucleoli, which reflect a reversible decrease of rRNA transcription and in blood cells are intermediate stages between actively transcribing large nucleoli in highly immature cells and inactive micronucleoli in terminal nucleolated maturation stages. Monocytes containing large nucleoli with a relatively uniform distribution of RNA characteristic of immature or stimulated blood cells were rare (< 2%). INTERPRETATION AND CONCLUSIONS: Nucleoli are present in all normal blood monocytes. The incidence of the main nucleolar types represents a very convenient complementary marker for evaluating the maturation and possibly the stimulated state of these cells.
