[Ultrasonographic assessment in differential diagnosis of non-palpable breast tumors].

In this study we analyse the value of high frequency ultrasound as diagnostic tool in non-palpable breast nodules (NPBN). We selected 121 women with NPBN revealed by ultrasound (US). The size of the nodules varied from 3 mm to 32 mm. Women with NPBN detected by US underwent sonographically-guided fine-needle aspiration biopsy (FNAB) and/or histological examination. Based on the US examination findings, the disease was diagnosed as malignant in 6 cases, as benign in 90, and in 28 cases it was unclassified. 15 patients underwent surgery. In 8, the disease was histopathologically confirmed as carcinoma (US 2 unclassified), 7 were benign lesions. US with FNAB as tool in diagnosing NPBN is an accurate technique for distinguishing between benign and malignant NPBN observable sonographically.

A double-blind, prospective, randomized, multicenter group comparison study of iopromide 240 vs iohexol 240 in myelography.

The aim of this study was to evaluate the safety and efficacy of iopromide 240 mgI/ml in comparison with iohexol 240 mgI/ml in myelography. A total of 421 patients in seven centers and four countries received an average of 11.9 ml of either iopromide 240 (278 patients) or iohexol 240 (143 patients) for X-ray and/or CT myelography in a randomized (2:1), prospective, double-blind study. All patients were followed up 3-4 h after the procedure, and 327 patients remained hospitalized for 24 h. In 82 patients an EEG was recorded prior to as well as 3-4 h and 24 h after myelography. Physical examinations, including measurement of vital signs, were performed in all patients at these time points. The results were subject to statistical analysis with the primary variable being the incidence of adverse events. Both contrast media (CM) were equally effective in terms of opacification. The rating for opacity was "good" or "excellent" in 88 % for both CM. Four patients (iopromide group: n = 3; iohexol group: n = 1) had transient EEG changes but did not show clinical symptomatology. The overall rate of patients experiencing any adverse event (AE) was 16.9 % for iopromide 240 and 14.0 % for iohexol 240. Equivalence testing was inconclusive; however, the results indicated equivalence. The rate for AEs considered as study-drug related was slightly lower with iopromide 240 than with iohexol 240 (7.2 vs 7.7 %, respectively). Neither unknown nor unexpected AEs known for myelographic X-ray CM nor serious adverse events were observed. Iopromide 240 and iohexol 240 are equally safe and effective and can be recommended for myelography.

[Fine needle biopsy in diagnosis of lung tumors].

Fine-needle biopsy is one of the method used in diagnosis of lung tumors. In this study the results of 192 biopsies performed by x-ray apparatus inspection were analysed. There was shown that lung cancer was recognized in over 50% of patients studied. In another cases non-specific inflammation (31%), lung fibrosis (1%), tuberculosis (1%), purulent inflammation (1%), blood (7%) were recognized. These results indicate for usefulness of this method in diagnosis of not only peripheral, but also central localized lung tumors.

[Intravascular embolization of intracranial arteriovenous hemangiomas].

UNLABELLED: The paper presents the technique of the procedures as well as the results of treating patients with operable difficult and non-operable intracranial angiomas. MATERIAL AND METHOD: During 26 months (1997, 1998 and 1999) 13 patients (7 women and 6 men) with intracranial arteriovenous haemangiomas underwent intravascular embolisation. The angiomas were embolised with coils (COOK) and Histoacryl with Lipiodol. RESULTS: Among the angioma patients, 2 were treated successfully during four procedures, 2 were treated during 3 procedures. The treatment was fully successful in 6 patients following the 3-rd and 4-th procedures. 5 patients are still being treated. Complications, during both the treatment and follow-up, were observed in two patients. CONCLUSIONS: The intravascular embolisation of intracranial angiomas is an effective treatment modality. It carries, though, a certain risk of complication development. Using this method in neuro-surgically non-operable cases is a method of choice.

[Intravascular embolization of intracranial aneurysms].

UNLABELLED: The paper presents indications for and the technique of the intravascular procedures as well as the results of treating patients with intracranial aneurysms. MATERIAL AND METHOD: Between 1997 and the May 2000, 25 patients (14 women and 11 men) with cerebral aneurysms underwent intravascular embolisation. The aneurysms were embolised with MDS-N BALT with coils. In 5 patients with aneurysm (about 2 cm), the procedure was performed in two or three steps. RESULTS: Aneurysms were embolised successfully in 19 patients. Three patients revealed intra- and postoperative complication manifestations. CONCLUSIONS: Intravascular embolisation of intracranial aneurysms is an affective treatment modality. It carries, though, a certain risk of complication development. Using this method in neurosurgically non-operable cases is a method of choice.

[Value of computerized tomography in the diagnosis of discopathy in the L4-L5-S1 segment].

The aim of this analysis was demonstration of the usefulness of CT in the diagnosis of lumbar discopathy. In 110 patients with backache, CT of the L4-L5-S1 part of the spine was done. The observed abnormalities were analysed, including presence of nucleus pulposus herniation, pathological changes of osseous structures of the spine, width of the vertebral canal and lateral recesses, and presence of other lesions there. Comparing the results of (CT) and myelography with intraoperative observations it was possible to confirm the high usefulness and value of CT in such cases. CT may be an important diagnostic method preoperatively in patients with iodine hypersensitivity and those with vertebral canal stenosis.

[Radiological diagnosis of iatrogenic stenosis of the lumbar spine in patients after surgical treatment of intervertebral disk displacement].

Iatrogenic stenosis of the vertebral canal arises as a result of inflammatory changes of the arachnoid with formation of adhesions following irritation of the meninges with chemicals, such as drugs and contrast media, or extension of the intracranial scar tissue, usually after operations. This stenosis is a serious diagnostic and therapeutic problem being often the cause of lacking improvement or recurrence of radicular pains after operations for herniated nucleus pulposus. On the basis of a literature review and own experiences the authors discuss the aetiology, pathogenesis, radiological diagnostic methods and methods of treatment in cases of iatrogenic postoperative stenosis of the lumbar vertebral canal.

[Three month treatment with budesonide in patients with corticoid dependent bronchial asthma--personal experience].

The experiment aimed at evaluating a three-month therapy of asthmatic patients with budesonide inhalations. The study involved 32 patients with long-lasting bronchial asthma depended on steroids. Therapy with budesonide (Pulmicort Astra) in a mean daily dose 1.6 mg enabled withdrawal of depot steroids and inhalation treatment only. In patients receiving prednisone in a mean daily dose 5.0 +/- 4.8 mg, budesonide therapy enabled a reduction of the oral dose by 50 percent. A significant improvement in the psychophysical status of patients has been noted despite a lack of significant differences in the results of lung functioning tests. Adverse reactions such as relaxation, insufficiency of the vocal cords at phonation and hoarseness were seen in 20 percent of patients, and in one of them required cessation of the drug. An increase in the number of Candida albicans colonies in throat swabs (over 20 colonies per plate) was seen in 7 treated patients but clinical symptoms which required an additional treatment were noted only in 1 patient.

[Evolution of the growth of multiple intracranial tumors in a case of type 2 neurofibromatosis].

A sporadic case of neurofibromatosis type 2 in a 23-year old woman is described with clinical and neuroimaging (CT or MRI) 4 years follow-up. Together with bilateral acoustic tumours multiple intracranial tumours and solitary orbital meningioma were present, the latter being confirmed surgically and histopathologically. Ocular findings of congenital cataract but lack of other external stigmata of neurofibromatosis completed the clinical picture of the disease. Diagnostic criteria of NF2, difficulties in establishing the optimal management options are described and the importance of evaluating of family members is underlined. Neuroimaging pictures showed tumour expansion over 4 years leading to severe disability with total blindness, deafness, paresis and cerebellar syndrome.
