[The use of the Heidelberg Retinal Tomograph in glaucoma prevention].

Altogether 2436 eyes of 1218 healthy persons from the ophthalmological point of view were examined in the screening glaucoma action by means of Heidelberg Retinal Tomograph (HRT II). In 103 persons (8.5%), the result of the examination was at least in one sector of the optic nerve head graded as "suspicious" of "out of normal range". In those individuals, the complete ophthalmological examination including visual field examination with the Humphrey perimeter Threshold 24-2 program was performed. Monocular changes in HRT II examination were noticed in 83 patients (in 24 out of them also visual field changes were noticed), while bilateral changes in HRT II examination were noticed in 20 persons (in 5 of them also changes in the visual field). Medical treatment of glaucoma was started immediately after the complete examination in 10 persons, in 6 others the treatment started after next 12 months. The remaining 87 persons (7.1%) with the abnormal finding in the HRT II examinations are followed up. The HRT II use for the glaucoma screening helped to make the diagnosis of this disease in the early stage in 16 persons (1.3%), who probably will not otherwise visit the ophthalmologist.

[Antiglaucoma drugs--content of preservatives and the patient's compliance].

The presence of preservatives in eye drops, may have, depending on the concentration, cytotoxic influence on the corneal and conjunctival cells, and may cause the patients' subjective inconveniences. The authors compare the benzalconium chloride (BAC) concentrations in individual drug used for glaucoma treating. The daily dose of BAC was calculated according to the drop's size and BAC concentration. The daily dose of BAC differs depending on the selected therapy markedly--doses in the range from 0 to 10 micrograms were found. The BAC concentration is one of important factors affecting the patient's compliance in the treatment of the primary open angle glaucoma.

[Correlation between various methods of examination in glaucoma].

The authors investigated a group of 42 patients (84 eyes) with primary glaucoma of the open angle and with glaucoma of a low tension. In addition to the basic examination they applied in all patients fluorescent angiography focused on the papilla of the optic nerve, static computer threshold perimetry using a Humphrey Field Analyzer, strategy 30-2, examination of the colour sense (Farnsworth-Munsell 100 HUE test and the desaturated HUE test with computer analysis) and examination of contrast sensitivity by means of the Cambridge low contrast test. They describe the results in the initial stage of an at least 5-year study. In patients aged 41-50 years (n = 10) they found small paracentral scotomas in the visual field in 90% and arcuate scotomas in 10%, minor absolute defects of filling of the papilla in 85%, in 15% only relative defects. In patients aged 51-60 years (n = 15) they revealed acute scotomas in 73% and marked quadrant losses in 27%. Changes in the fluoroangiogram were more marked than in the former groups. In patients aged 61-71 years (n = 15) the changes were analogous. Defects of colour sense of the blue-yellow area were recorded in 63 eyes. Changes of the colour sense and absolute defects of filling in fluorescent angiography correlated with the degree and site of affection of the visual field and the patient's age in primary open angle glaucoma. Conversely, in glaucoma with normal tension the values of colour sense remained normal even when the visual field was affected.

[Glaucoma in the Czech Republic].

The authors investigated the conditions for the diagnosis and treatment of glaucoma in the Czech Republic using questionnaires dispatched to 88 departments. The total number of patients suffering from glaucoma is 50,000, the highest rate being in Prague and Moravia (à 9,000). In relation to the number of population the frequency is greatest in Prague (1 per 140 population) and lowest in Southern Bohemia (1 per 318). In 12 departments routine gonioscopic examinations are not made, in 30 departments applanation measurements of the intraocular pressure are not made. Statistical computed perimetry can be performed only in six departments. In 1989 about one tenth of the total number of 9,000 patients hospitalized on account of glaucoma were hospitalized on account of acute glaucoma. In that year 952 primary and 320 secondary glaucomas were operated, the greatest number in Prague and Eastern Bohemia. It is essential to improve the standard of ambulatory diagnostic and therapeutic care at the expense of hospital admissions.

[Partial trabeculoplasty using the argon laser in the microsurgery of glaucoma].

Authors refer one-year-results of their original modification of the partial trabeculoplasty using the argon laser Britt 152 in the treatment of the primary open-angle glaucoma. In 48 eyes of 35 patients the laser trabeculoplasty (ALT) was performed, using parameters 50 microns, 750-100 mW, 0.1 sec, 120 degrees of circumference of the anterior chamber angle. Influence of ALT on the compensation of the intraocular pressure and visual functions were followed in week intervals (1.5, 6-15, 16-25, 25-35, 36-45, 46-55, 56-70) after the performed ALT. The mean decrease of the intraocular pressure was 8.4 mmHg (1.12 kPa), the mean success was in 87%.
