Metronidazole in the treatment of tropical phagedenic ulcers.

In an uncontrolled preliminary trial, metronidazole administered orally to 30 patients was effective in resoving tropical phagedenic ulcers within a period of one to two weeks. The efficacy of this drug indicates the important role played by fusospirochetes in causing tropical ulcers.

Ichthyosis with unusual hair shaft abnormalities in siblings.

An autosomal recessive disorder characterized by lamellar ichthyosis, an usual structural defect of the scalp hairs and amino aciduria occurred in two siblings in a South Indian family in Madras. In addition the female sibling was physically and mentally retarded. The disorder differs from typical Netherton's syndrome in several ways. The hair abnormality specific for this disorder appears to be splitting of the shaft. The possible mechanism of production of the split hairs is discussed.

An unusual presentation of Trichophyton violaceum infection.

A case with encapsulated Tricophyton violaceum abscesses in the skin is recorded. Defective cell mediated immunity was shown in the patient and his two cousins. This defect combined with malnutrition, hypoadrenalism and griseofulvin resistance gave unusual lesions and chronicity. A probable genetic factor for the defective cell mediated immunity is postulated.

Cutaneous infection by Cryptococcus laurentii.

Cryptococcus laurentii was isolated twice from cutaneous granulomas in the leg and foot of a 40-year-old man. Histologically the cells of C. laurentii were found in groups in the dermis and also inside giant cells. There was epithelioid cell infiltration in the dermis and subcutis. An atypical mycobacterium was isolated in addition to C. laurentii. Therapy with anti-tuberculous drugs after an initial amphotericin B infusion was found to be satisfactory.

Perinevoid alopecia. An unusual variety of alopecia areata.

We report three cases of alopecia areata occurring around central, pigmented nevi. The histologic appearance of the central nevus was that of the halo nevus. Removal of the nevus did not appear to alter the natural course of the alopecia. Histochemical studies showed a substantial increase of acid mucopolysaccharides in the dermis in each case.

Black grain mycetoma--2 case reports.

Two cases of black grain mycetoma, one owing to Madurella grisea and the other due to M. mycetomii have been reported. The clinical features appear to be less florid than that occurring in actinomycotic mycetoma. There was no bone involvement in either of the two cases. These two are the first cases of black grain mycetoma from our institution.

Tylosis following post-corrosive stricture of the oesophagus.

A case of tylosis following corrosive stricture of the oesophagus in a male of 26 years is recorded. It is suggested that there is probably a connection between the state of the oesophagus and the state of palms and soles and that an oesophageal abnormality may precede tylosis of the late onset type.

Familial syringoma.

Syringoma occuring in two siblings in a South Indian family is reported. A familial occurrence of such an uncommon tumour is of considerable interest and might indicate a genetic influence in its causation.
