[Rear and fore-foot pathologies].

A good knowledge of the anatomy of the foot is indispensable in order to orient the diagnosis. The clinical examination of the foot need to be done in a static (sitting and lying) as well as in a dynamic (walking and running). The foot is divided in three regions : rear-foot, mid-foot and fore-foot. The pathologies that are developed (rear-foot and fore-foot) are the ones that are most frequently seen during the consultation. They are described taking into account their aetiology, the clinical aspects, the imaging and the treatment.

Palmar metacarpophalangeal joint dislocation.

Palmar dislocations of the long finger metacarpophalangeal joint are extremely rare and easily missed at the first clinical examination. We describe a palmar metacarpophalangeal dislocation of the long finger following a hyperflexion injury. The presentation, aetiology and treatment are discussed.

Magnetic resonance imaging (MRI) of the ankle and hindfoot.

While Magnetic Resonance Imaging (MRI) has become a routine diagnostic method to deal with pathology of the knee, little has been published about foot and ankle lesions. This is probably due to the anatomic complexity of bone and ligamentous structures of these small joints, necessitating the use of very thin slices from various orientation planes (orthogonal and oblique planes). A special technique is needed allowing a 3-dimensional (3D) analysis, using inframillimetric slices and more sophisticated equipment than for 2-dimensional (2D) MRI (high fields with good homogeneity, specialized image processors...). An initial potential indication of the method is the precise diagnosis of lateral ligamentous components in severe sprains. MRI may determine if surgical therapy is needed. The ligamentous components are reconstructed along their specific planes thanks to the 3D method. Furthermore, assessment of the ligamentous damage is also possible in chronic ankle instability (elongation, fibrotic appearance...). Thanks to the multiplanar approach, associated lesions of articular cartilage can be revealed in ankle sprains. The high sensitivity of MRI in slight modifications of medullary bone allows the early diagnosis of "occult" fractures (not seen on conventional radiographs), osteonecrosis, osteochondrosis and reflex sympathetic dystrophy syndrome. Finally, MRI easily displays all ankle and foot tendons and fascia and is particularly helpful in depicting partial tears and postoperative complications.

[Dissection of the sub-astragalar ligaments and magnetic resonance imaging].

The aim of this study is to find the different ligamentous structures in the subtalar region through dissection and to compare them with images obtained on MRI. MRI allows visualization of the different ligamentous structures with greater precision than with computerized tomography. This investigation should permit a more precise diagnosis of injuries often poorly understood in this region.

[Electronic podometry--2-year experience: preliminary report].

The authors relate their experience with the EMED system for the dynamic assessment of plantar pressure used in clinical practice for 2 years: how the measurement device was chosen; how the examination is performed; the usefulness of this type of examination in foot surgery. They discuss the specific problems related to the application of this new diagnostic tool in clinical practice.
