Voltage-gated potassium channel antibodies in limbic encephalitis.

We found voltage-gated potassium channel (VGKC) antibodies in 4 of 15 patients with limbic encephalitis (LE). Two patients with idiopathic LE had high VGKC antibody levels (>800 pM; controls <100 pM), that fell in parallel with a clinical response to immunotherapy. Two patients with lower VGKC antibodies (170 pM, 300 pM) had lung cancer (radiological evidence only in one) and the LE improved with immunotherapy in one. The other 11 patients without VGKC antibodies had paraneoplastic LE and eight onconeural antibodies (Hu in 6; Ma2 in 2). VGKC antibodies do not unambiguously discriminate between idiopathic or paraneoplastic LE but probably indicate a good response to immunotherapy.

Alternative Therapies for Headache.

Although alternative therapies are widely used for the treatment of headache disorders, physicians often know little about the available options or about the risks and benefits of the treatments their patients may already be using. Alternative treatments or complementary medicine can provide significant benefit for the headache sufferers, be neutral, distract the headache patient from using more effective therapies, or be harmful. Physicians should take a rational, open-minded approach to the use of alternative and complementary headache treatments.

Reversible white matter alterations in encephalopathy associated with autoimmune thyroid disease.

Encephalopathy associated with autoimmune thyroid disease is an underdiagnosed clinical condition. No specific disease marker exists and to date, imaging techniques have not proven useful. The disease is characterized by seizures, stroke-like events and psychiatric symptoms. Here, we present the report of a patient with encephalopathy associated with autoimmune thyroid disease in which a reversible white matter disorder was detected by MR imaging. This encephalopathy was alleviated and later disappeared following corticosteroid treatment. We conclude that the white matter is the principal brain tissue involved in this condition and that brain MRI studies may prove to be a useful tool in the diagnosis of patients suffering this treatable disease.
