Short report: neonatal screening pilot study of Toxoplasma gondii congenital infection in Mexico.

Congenital toxoplasmosis is an obstetric problem in Mexico, but its actual frequency is unknown. Using a network for screening of non-infectious disorders, we performed a pilot study to determine the frequency of IgM antibodies to Toxoplasma gondii in 1,003 infants (53.1% male, mean +/- SD age = 18.3 +/- 13.0 days, birth weight = 3.116 +/- 0.453 kg) in Mexico City from March to April 2003. Blood samples embedded in filter paper were assayed for IgM antibodies using a capture enzyme-linked immunosorbent assay and results were confirmed by Western blot. Two asymptomatic newborns, one of them premature, had IgM and IgG antibodies in a serum sample taken from both the infant and the mother and were clinically followed. Our data suggest a frequency of approximately two cases of congenital T. gondii infection per 1,000 newborns in Mexico City.

Discoid lupus erythematosus in children: clinical, histopathologic, and follow-up features in 27 cases.

Among 27 pediatric patients with a clinicopathologic diagnosis of discoid lupus erythematosus (DLE), 15 had localized cutaneous lesions and 12 had disseminated lesions. During a mean follow-up period of 36 months, seven patients (26%) developed systemic lupus erythematosus (SLE). Four of these patients were less than 10 years of age. No correlation was found between localized and disseminated lesions and evolution to SLE. Three of four patients with a positive family history for rheumatoid disease developed SLE (p < 0.05). Hyperpigmentation was significantly more frequent (p < 0.04) in children less than 10 years of age. There was a female predominance of 5:1 among patients less than 10 years of age. Our findings suggest that onset of DLE prior to 10 years of age does not indicate a greater risk of developing SLE. The occurrence of localized or disseminated lesions does not seem to influence the outcome.

[Neonatal sepsis morbidity and mortality in a tertiary care hospital].

OBJECTIVE: To compare the epidemiological, clinical and microbiological profiles between patients with neonatal sepsis who lived or died. MATERIAL AND METHODS: The medical records of patients with neonatal sepsis were retrospectively reviewed at Instituto Nacional de Pediatría (National Pediatric Institute) of Secretaría de Salud (Ministry of Health) in Mexico City, between 1992 and 2000. Neonatal sepsis cases were classified as surviving or not after 90 days of postnatal follow-up. The survivor and decreased groups were compared using Mann-Whitney's U test for continuous variables, and the chi-squared test or the Fisher's exact test for categorical variables. Significantly associated variables were included in a Cox proportional hazards model. A p-value < 0.05 was considered statistically significant for all analyses. RESULTS: A total of 116 patients with neonatal sepsis were included (65 live and 51 dead). Multivariate analysis showed that fetal distress, respiratory distress, a delayed capillary fill up, a low platelet count, and a positive hemoculture for Klebsiella pneumoniae were significant risk factors for death. CONCLUSIONS: Epidemiological, clinical, laboratory, and microbiological variables are significant predictors of death in newborns with neonatal sepsis. [Evaluation of cisplatin ototoxicity by the audiometric curve in retinoblastoma].

BACKGROUND: To evaluate hearing loss severity according to Brock's gradient and to compare it with the audiometric curve during cisplatin treatment in children with retinoblastoma. METHODS: This was an observational retrospective and retrolective study. Twenty children with the diagnosis of retinoblastoma under cisplatin treatment were included. Audiometric testing was performed before treatment, after the second and fourth doses, and after the final dose. RESULTS: Decreased audition was observed in 100% of the cases. Ototoxicity can be observed with cumulative doses of 240 mg/m(2) and higher. No improvement in audition was observed and the audition loss progressed from high to medium frequencies; 15% of the patients showed a grade II loss of high frequencies at the end of the second dose. This level was observed in 95% of the cases at the end of treatment. Two years after completion of therapy, no patient showed auditory recovery. Area below the curve showed higher sensitivity to identify initial auditory loss. CONCLUSIONS: Evaluation of audition with Brock's gradient can be performed. The area under the curve is a useful method to identify minor changes in serial conventional audiometry.
