Chondromyxoid fibroma of bone (CMF) is a rare benign primary bone neoplasm accounting for less than 0.5% of all primary bone neoplasms. The spine is an uncommon site for this tumour, with forty-two cases reported in the modern English literature. They have clinical features similar to CMF arising at other sites. Local recurrence is well documented. We report an incidentally discovered lytic lesion of the C2 vertebra. The patient underwent stereotactic CT guided trans-oral curettage of the lesion with iliac bone graft and anterior fusion of C2 and C3. Microscopic examination of the surgical specimen demonstrated CMF. This is the second reported case of this rare tumour in this location. We review the literature and the unique radiological and pathological features and management of spinal CMF. Local recurrence of spinal CMF and its management is also discussed in light of the five previously reported cases of local spinal recurrence.

Parathyroid carcinoma (PTC) is a rare pathological entity, with fewer than 400 reported cases. Intracranial metastasis of a PTC is exceptional; only five other cases have been identified in the English literature. The authors present a unique case of a patient with a solitary intracerebral metastasis of a PTC in an individual presumed to have hereditary hyperparathyroidism who had concomitant papillary and follicular carcinomas of the thyroid. The literature relevant to the management of these rare lesions is reviewed and discussed.
The Physiome Project of the International Union of Physiological Sciences (IUPS) is attempting to provide a comprehensive framework for modelling the human body using computational methods which can incorporate the biochemistry, biophysics and anatomy of cells, tissues and organs. A major goal of the project is to use computational modelling to analyse integrative biological function in terms of underlying structure and molecular mechanisms. To support that goal the project is establishing web-accessible physiological databases dealing with model-related data, including bibliographic information, at the cell, tissue, organ and organ system levels. Here we discuss the background and goals of the project, the problems of modelling across multiple spatial and temporal scales, and the development of model ontologies and markup languages at all levels of biological function.
A case of glioblastoma with leptomeningeal spread after control of the primary lesion by radiation therapy. A discussion is made regarding leptomeningeal metastatic disease with reference to the literature.
To understand the relationship between pituitary adenoma and carcinoma, four adrenocorticotropic hormone-producing pituitary adenomas and corresponding metastatic carcinomas were studied. All were functional macroadenomas (three cases of Nelson syndrome and one of Cushing disease) that initially invaded the sella turcica and occurred in women ranging in age from 17 to 66 years (mean 45 years). Metastases (two craniospinal and two systemic) occurred after latency periods of 6 to 13 years. Histological specimens were immunostained for pituitary hormones, Ki-67 antigen (MIB-1), p53 and p27 proteins, D-type cyclins, and glucocorticoid receptor messenger (m)RNA. The DNA content of the specimens was assessed using Feulgen stain. Reactivities were quantified by digital image analysis. Primary/recurrent lesions and metastatic tumors differed according to their respective mean mitotic indices (1.2/10 hpf compared with 4.3/10 hpf), MIB-1 labeling (1.7% compared with 8%), p53 staining (37.3% compared with 49.9%), and p27 labeling (48% compared with 25%). Cyclin D, immunoreactivity provided no prognostically significant information. Glucocorticoid receptor mRNA was detected in all cases. Results of a ploidy analysis were variable and nonprognostic. In keeping with the 2000 World Health Organization classification of endocrine neoplasms, our findings support the concept that primary tumors that exhibit mitotic activity, an increased (> 3%) MIB-1 labeling index, and/or p53 immunoreactivity should be termed "atypical adenomas" to denote their aggressive potential and the possibility of future malignant transformation.
