Taiwan's perspective on electronic medical records' security and privacy protection: lessons learned from HIPAA.

The protection of patients' health information is a very important concern in the information age. The purpose of this study is to ascertain what constitutes an effective legal framework in protecting both the security and privacy of health information, especially electronic medical records. All sorts of bills regarding electronic medical data protection have been proposed around the world including Health Insurance Portability and Accountability Act (HIPAA) of the U.S. The trend of a centralized bill that focuses on managing computerized health information is the part that needs our further attention. Under the sponsor of Taiwan's Department of Health (DOH), our expert panel drafted the "Medical Information Security and Privacy Protection Guidelines", which identifies nine principles and entails 12 articles, in the hope that medical organizations will have an effective reference in how to manage their medical information in a confidential and secured fashion especially in electronic transactions.

A Web-based short messaging service system to enhance family-centered surgical patient care.

The purpose of this study was to develop a web-based short messaging service (SMS) system in the operating room (OR). At the same time, we integrated and analyzed the limits of SMS in the OR. A cross-sectional study with quantitative methods and convenience sampling was used to gather data in this study. In the SMS project, we approached the content of SMS with operation diagnosis, patient sources and time series (pre-, intra-, and post-operation). We also default the contents of SMS by patient individual conditions. In this study, 177 participants received 346 text messages and 139 participants answered satisfaction questionnaires (78.53 %). The findings show the usability of SMS as applied to clinical care, especially for reducing family anxiety, for real time information, physician-patient communication, medical care processes and patient safety. Therefore, it is suggested to exploit the effectiveness of a personal medical care information database under global goal in the future and to develop comprehensive management of the surgical patient relationship.

Attitudes of nurses towards End-user Development.

The purpose of this study is to explore nurses' attitudes toward developing self-made information tools. In order to understand nurses' attitudes, we studied the factors that affect nurses' attitudes toward End-user Development (EUD). The investigation of nurses in Taiwan hospitals showed that nurses had positive attitudes toward EUD. A framework of nurses' attitudes toward EUD is presented. A gap was found between the functions provided by the nursing information system and self-assessment required by the job. The nurses were inclined to be dissatisfied with support provided by the information centers. Nursing managers were advised to enhance on-the-job training for computer applications, and to include computer competencies in the nursing ladders program. The results indicated that nurses were willing to develop their own tools. We believe that nurses with computer application skills who are provided with appropriate tools can become nursing application developers. The results encourage nurses to develop software by themselves. Further research and development is required to enhance the impact of EUD and establish a sharing environment for EUD applications.

Qualitative analysis of end user computing strategy and experiences in promoting nursing informatics in Taiwan.

The purpose of this study was to analyse end user computing strategy and experiences in promoting nursing informatics in Taiwan. In February 2004, an 8-day NI technology training campaign was held in Taipei for 60 clinical nurses. Excel VBA was used as the tool to teach the clinical nurses, who had never written any programs, but were very interested in informatics. Three projects were determined after detailed discussion and evaluation of clinical needs and technical feasibility between the nurses and the technical support team, which was composed of one experienced informatics professor and one clinical NI assistant. A qualitative analysis was used to interview the three pairs of programming clinical nurses and their direct supervisors with a structured but open questionnaire. Representative concepts were categorized from the data until all were categorized. The concepts were organized under three categories: the purposes, the benefits and the challenges of system development. According to this study, end user computing strategy with Excel VBA was successful so far.

Intelligent mobile voice information centre system: Taking the emergency triage support system as an example.

According to research by the International Telecommunications Union (ITU), Taiwan has the highest percentage of people holding mobile (cell) phones, and every one has one or two cell phones. We usually use mobile phones to make contact with hospitals or dial 119 in an emergency. We have developed a new call flow style and combine traditional passive call flow. The system is designed with a voice menu and users can command services actively. The telephone connects to a computer server to enter the call flow module with ASR and TTS. The recognition term comes from a voice term database. The voice management center is an ER triage decision system. We have tested three scenarios which are DOA, a large number of patients and critical trauma. We measured the completion time and recognition rate for all the scenarios. The recognition rate in all three cases was above 90%. The average completion time for DOA is 38.6 seconds, for the large number of patients is 20 seconds and for the critical trauma case, the average is 252.7 seconds. It is too slow to input one patient's data but the communication transfer of the message is fast and convenient; the DOA notice only takes 38 seconds to inform medical staff, which is very efficient and medical staff believe that this can help their communication. We will continue to extend our system and further apply it to other domains. Future studies may combine the Internet platform with the WiFi phone and soft phone (Skype) and broaden the scope of the application in Taiwan.

Developing and evaluating a wireless speech-and-touch-based interface for intelligent comprehensive triage support systems.

Continuous speech recognition (CSR) technology appears promising in mobile nursing but is not yet well studied. We developed and evaluated bimodal CSR and touchscreen triage support systems in the Emergency department (ED) of a medical center with 2700 beds in 2004-5. Evaluation results show that the average accuracy rates of systems ranged from 94 to 98%. Results suggest that the ED nurses were significantly more willing to use CSR combined with touchscreen systems than the others, such as PDA and CSR alone. A more flexible interface, not efficiency, might be the main reason for this finding.

Evaluation to the informatics system for health promotion management with MedModel simulation project.

The purpose of this study was to evaluate the efficacy of the MedModel simulation project system based on the real approach in health promotion management. Used the MedModel simulation project in the physical exam department of a medical center, it's truly promoted the client satisfaction and reduced run-time about 30 minutes, and raised the client attendance about 30%. Actual application in the real health examination, also obtained the same improvement.

Performance and acceptance evaluation of a self-made excel nurse scheduling supporting system.

The purpose of this study was to evaluate our self-developed scheduling plan support system in terms of performance and nurses' perception of easiness to use and usefulness. The results showed that thought the head nurse needed to averagely take half hour to modify the system scheduling plan, it was much better than four hours needed before. Nurses were also satisfied with the system. We believed nurses could design very useful and easy-to-use systems if trained and equipped with the right information tools.

The development of a heuristic-based excel scheduling support system for nurses.

The purpose of this study was to develop a nurse-self-made scheduling support system based on the heuristic approach. A Pediatric ICU unit, staffed with 35 nurses, was taken as the case in this study. Nurses' preferred scheduling principles were surveyed. The Excel VBA was used to develop the support system by an informatics nurse. The system was composed of five components: personal scheduling request setting, auto-scheduling, shift modification, report generating and inquiry. The scheduling results were widely accepted by the nurses and the time to plan a monthly schedule was reduced from 4-5 hours to 30 minutes.

Demands and expectation for the e-learning model in clinical nursing education.

The constructed "Nursing e-college" in this study is an online self-learning educational platform developed in a particular medical center. This study was conducted in survey design, and the purpose were (1) to prove the computer skills ,the ability and level of preparation of nursing professionals in learning and using computerized information; (2) to understand the demands and expectations in the clinical nursing curriculum of e-learning. The study utilized randomized sample for data collection during May to December,2005. And then the reliability and validity of the questionnairs in the study were examined. Results indicate that the investigative inventory developed in this study has significant reliability and validity. Also, through statistical calculations (included descriptive statistics, ANOVA, Pearson correlation etc.), results have certain amounts of statistical significance, and demonstrating the functional nature of the study design.

SIGMA simulation for health promotion management.

Health promotion is the key task to achieve global health 2010. Periodical physical check-up have been the most basic manifestation for maintaining personal health care and promotion inverted exclamation markC Sufficient health check facility was provided in a medical center with annual service of 9200~9500. Sixty customers receive around 20 check ups in this physical check up center daily. Smoothness, efficacy and customer satisfaction of the process will be caretakers inverted exclamation mark| concern and target for promotion.

A feasible strategy of promoting nursing informatics by End User Computing.

The purpose of this study was to report our experiences in promoting NI training and professorship using the strategy of End User Computing in Taiwan. An interview-based survey was made to better understand the pros and cons of this strategy from three successful projects. The results appear promising and cost-effective.

Application of graphical relational representation techniques in healthcare website: taking SARS website information query and representation as an example.

In this study, the website information in the SARS period was taken as the example. By setting attributes of information of each article, the relations between articles are established. The relation,presented by an interacted graphic interface based on SVG language, could very with users query. The results show that the new approach could help users to find and understand the contents and relationship among different information source more efficiently.

The development of a Patient-Identification-Oriented Nursing Shift Exchange Support System using wireless RFID PDA techniques.

The objectives of this study were to technically testing the feasibility of combining RFID and PDA technologies in nursing care and to develop a support system for the nursing shift exchange, which featured with "Positive Patient Identification" and "Point of Care" for patient's safety and security. The most challenging part for the future work would be to embed the system into the real workflow. Future study would be to examine the practical effectiveness of the system.

Automatic appropriateness-evaluation and consultation-suggestion of antibiotics usage via mining of previous prescription data in hospital information system.

Inadequate infection subspecialty is main problem in most local hospital of Taiwan. Appropriate prescription of antimicrobial agents is an important issue in daily clinical practice. In order to avoid possible side effect and cost after inappropriate usage of antibiotics, we developed a system for automatic appropriateness evaluation and consultation suggestion of antibiotics usage via mining of previous prescription data in hospital information system.

The development of intelligent, triage-based, mass-gathering emergency medical service PDA support systems.

The support systems for the Emergency Medical Services (EMS) at mass gatherings, such as the local marathon or large international baseball games, are underdeveloped. The purposes of this study were to extend well-developed, triage-based, EMS Personal Digital Assistant (PDA) support systems to cover pre-hospital emergency medical services and onsite evaluation forms for the mass gatherings, and to evaluate users ' perceived ease of use and usefulness of the systems in terms of Davis ' Technology Acceptance Model (TAM). The systems were developed based on an established intelligent triage PDA support system and two other forms the general EMS form from the Taipei EMT and the customer-made Mass Gathering Medical form used by a medical center. Twenty-three nurses and six physicians in the medical center, who had served at mass gatherings, were invited to examine the new systems and answer the TAM questionnaire. The PDA systems were composed of 450 information items within 42 screens in 6 categories. The results supported the potential for using triage-based PDA systems at mass gatherings. Overall, most of the subjects agreed that the systems were easy to use and useful for mass gatherings, and they were willing to accept the systems.
